Chapter 23, Problem 13RQ
Summary Introduction

To explain: The physiological reason for advising diabetic patients who are under physiological stress to monitor blood sugar carefully in case there is a need to increase their insulin dose temporarily.

Introduction: Diabetes mellitus is the most common metabolic disorder of a pancreatic endocrine system. Diabetes is associated with abnormally elevated concentration of plasma glucose. It is known that when a diabetic person is under any stress, the chances of high plasma glucose level increases. Cortisol is a steroid hormone and promotes gluconeogenesis under stressed conditions.

What happens to the insulin-secreting capacity of a type 2 diabetic placed on insulin therapy earlier than recommended? Can the external supply of insulin improve the functional capacity of the insulin-secreting cells, to some extent by providing some rest to these cells? Is inhaled insulin a suitable substitute for injectable insulin? Is there, or will there soon be, insulin in the form of a tablet?
What kind of bioreactor should be designed for insulin production?
A woman affected by type I (insulin-dependent) diabetes miscalculates and injects herself with too much insulin. She soon begins to feel confused and sluggish. She calls for medical assistance and injects herself with the glucagon her doctor prescribed for such an emergency. When an ambulance arrives, she is given dextrose ( a sugar) intravenously. How did the excess insulin cause her symptoms. How did the glucagon injection and the intravenous sugar help reverse them?
