The definition of Personalisation means that every person receiving support either provided by the government or funded by him or herself will have choice and control in what they would like their care to be.

Person centred thinking is a set of values, skills and tools by the individuals in the personalisation of services who need support or health care. Person Centred Planning is for the Individuals with learning disabilities, Person Centred Planning also means that the person is at the centre, family and friends are full partners, Person Centred Planning concentrates on what is important now and in their future and specifies the support they require to live there life in the way they wish. The Person Centred Approach is there to help the individuals live as independently as possible to have the right to choose to control the services that they need and to have access to both wider public, community services and employment and education. They are all there to help
The main principles of person centred model are the core conditions of empathy, congruence and unconditional positive regard. They are vital for building a safe therapeutic relationship, in which the client can explore fully.

Person centred planning is a set of approaches designed to assist someone plan their life with support. It is most often used as a model to enable people with disabilities or otherwise requiring support to increase their personal self determination and improve their own independence. It discovers and acts on what is important to the person.
Of course there will be important conflicts of interest between service users and their carers which will need to be faced up to at an individual level. But services should not be provided by exploiting the personal commitment and dedication of carers. Families who care for individuals whose behaviour presents challenges are subject to considerable and continual stress, both physical and psychological. Although staffs is not permanently on duty as family carers can be, they are still subject to the same stresses when supporting people whose behaviour challenges. This is true even in settings which aim to do no more than contain people. Similar concerns apply to staff as to carers. The higher aspirations often found in progressive services impose extra strain on staff and here too it is important to balance the demands of working constructively with people who can be very difficult, day in and day out, with the needs of individual staff for emotional and practical respite and ongoing support. Person-centred is about providing care and support that is centred or focused on the individual and their needs. We are all individuals and just because two people might have the same medical condition, for example, dementia, it doesn’t mean that they require the same care or support because they are two different people which means their needs will be different. Another strategy that has been developed for dealing with challenging behaviour is person-centred planning. Person centred planning is used in a variety of health and social care setting with service users of all ages from cradle to grave. It is used in elderly care homes to provide the best care for that individual and within families that have children of different ages to help parents give children the best care possible and it is used for individuals with physical impairments such as Down’s Syndrome.
It is important to get to know the individual to ensure the individuals needs are met and to put together a person centred plan. It is important to involve the individual when writing care plans and development plans to enable them to establish their needs and enables the individual to express their wishes and preferences and to make them aware that they have choices.
Instead offering people a choice of what is currently available and finding what best fits their needs, person-centred working looks at someone’s needs and built the support package around them. One of the important aspects of person-centred planning is to look at what people are able to do for themselves and to ensure that services are not taking over aspects of a person’s life that they could perfectly well manage without support.
Valuing People (Department of Health 2001) it was introduced by the labour government who were keen to promote independent living. This white paper on learning disabilities was for the first in England in 30 years. It made direct payments available to more people with a learning disability and was the first paper where we officially come across the term ‘Person Centred Planning’. It stresses the importance of Personal Centred Planning in helping people with learning difficulties take charge of their own lives. This paper has been ‘refreshed’ in 2009 titling it Value People Now and is a new three year strategy for people with learning disabilities, and will lead to better lives for people.
It is important to review care and support plans as people’s needs change. By including the person, their family then everyone knows what is happening and the family can help to monitor mood swings and behaviour. The individual and the family can express their views and preferences and any relevant risk assessments may be done with everyone involved. By monitoring the individual, a decision can be made as to whether the changes are effective and if the best care is being given to encourage independence and promote dignity.
