- After viewing the MRI and talking with Elaine, Dr. Leutz strongly advised her to have reconstructive surgery on the ACL and to use conservative therapy on the partially torn medial collateral ligament. First, though, he wanted her to "pre-hab"-that is, take physical therapy for several weeks-in order to have the knee and leg in the best possible shape before the surgery. She was also instructed to use a more substantial brace. Dr. Leutz described the procedure: either a cadaver tendon OR the middle third of her patellar tendon would be removed and used to replace the ACL, which would be removed. Elaine opted to use her own tendon. Five weeks later, Elaine went in for surgery. She was given a drug used for conscious sedation as well as a general anesthetic. Through the arthroscope the surgeon noted significant injury to both the medial and lateral menisci, which he repaired with sutures and screws. The pieces of the torn ACL were removed. - At first, Donnie’s wife accused her once-active 25-year-old husbandof trying to get out of housework by constantly complaining aboutpain and stiffness in his lower back. But over the next 5 months, thepain and stiffness increased and seemed to be spreading up his vertebral column. The family doctor referred Donnie to a rheumatologist, who diagnosed ankylosing spondylitis (AS). AS, a chronic inflammation of joints at points where ligaments, tendons, and joint capsuleinsert into bone, causes fibrosis (the development of scar tissue), ossification, and fusion of joints. Combine your knowledge about bonegrowth, repair, and anatomy from chapters 6 and 7 and joint structureand function from this chapter to identify the category of joints primarily affected by AS, and explain how chronic inflammation ofDonnie’s joints led to their fusion.arrow_forwardJoseph was frantically texting his friend before he had to store his phone for class. (a) What kinds of movements can normally occur at the ankle joint? (b) Was the doctor telling Harry that his bones were broken? (c) What is reduction of a joint? (d) Why is it often necessary to repair ligaments surgically? (e) How will the use of arthroscopic surgery minimize Harry’s recovery time and his suffering?arrow_forwardAn 18-year-old high school football player suffered a torn anterior cruciate ligament (ACL) in the state playoffs. After undergoing repair, he found it difficult to ambulate with the knee brace. Weighing close to 140 kg (approximately 300 pounds), the athlete found it difficult to stand and balance on one leg. His mother was unable to support him by herself. After 1 week, the young man began to have increased pain and swelling in the calf of his affected leg. In the middle of the night, the teen called out to his mother, complaining of a shortness of breath and unexplained anxiety. His mother called 9-1-1, and he was taken to a local hospital. In the emergency department, he was diagnosed with a deep vein thrombosis and a pulmonary embolism.
