3J On 6/30/2015, client attended to her ILP meeting with her youngest son Keury. Child was well dress for the weather. Cm observed child very anxiety and he was telling client that he wanted to go to his unit because wanted to watch cartoon. Housing: HOH asked HS when she is going quality for any housing program. HS Ms. Taveras ask client if she added her husband to her PA case, if her husband starting to provide proof of saving, proof of pay stubs. Client stated that she all already added her husband to her case and he starting to work at the Kennedy airport, he had to provide his pay stubs to CM. Benefits/Entitlements: Client has an active PA case. Employment: Client Mr. Abreu currently got hired at Kennedy airport through the Back to work program. Client Mr. Polanco provides his pay stubs to place them in his file. Client started that she got confused with the day her son Keury supposed to attend to the summer camp. Child will be attending to the second session, which is July 13, 2015 to July 31, 2015. Cm reminded client to think of sending her son the summer camp, child don’t like to follow the rules and he likes to run when someone saying something or if had to follow rules. Client stated that her son love to do different activities and he will enjoy the summer camp. Client stated that her son is going to behave

On 7/30/2015, client walk in the social service office and CM completed Bi-Weekly ILP Review. In the meeting client appears she appeared her stated age and in good physical health. She was satisfactorily groomed & dressed. She constantly throb her forehead, she most of time space out and her affect is flat.
On 9/12/2015, CM met with the client to complete Bi-Weekly ILP Review. She was well groomed, and dressed with proper attire. She made eye contact appropriately. Her affect was appropriate. CM inquires if the client had any recent seizure disorder. Client replies “No” because she is not spending time in the shelter. She continue to relate during the day spend it with her children and grandbaby.
Social Service Meeting: On 02/28/2017 Ms. Williams met with her assigned Case Manager for the family weekly ILP Document Review. Ms. Williams’ next ILP Document Review appointment is on 02/07/2017. No issues or concerns were discuss during the meeting. Children were present at the time at the meeting.
On 3/24/2016, CM met with the client to complete Bi-Weekly ILP Review. In the meeting client was dressed with proper attire for the weather. She was cooperative and friendly in the meeting.
The worker was able to speak with Ms. Richardson advising her of her appointment with Preferred Children Services. Ms. Richardson told the worker that she was aware that the appointment was on 12/2/2016. The worker told Ms. Richardson that it was imperative that she attend the appointment.
Client Contact: The Housing Specialist conducted a HV (Home Visit) with Ms. Sims in her home and to also collect the applications for the WOW Fast Track program and the pay check stubs for her son Christopher Jones. Ms. Sims was in good spirits and her grandchildren were present. Ms. Sims was asked by the Case Manger have she enrolled in the writing classes at her local library. She stated that she has not enrolled in the writing classes due to not having a free moment to herself
CM was out on vacation for the period of 11/25/2016 to 12/12/2016. Client was scheduled to meet with CM on 12/13/2016, to complete Bi-Weekly ILP Review. Client was no show. CM inquire the reason client was no show. Client replies “she wasn’t feeling well and she went to LIJ Emergency Room. On 12/15/2016, CM met with the client to complete Bi-Weekly ILP Review. Client was dressed with proper attire for the weather. She was well-mannered and groomed. In the meeting client appears to be cooperative
On 8/4/2015, CM met with the client and completed Bi-Weekly ILP Review. Client arrived early for the meeting. In the meeting client appeared her stated. She was alert, satisfactorily groomed, and casually dressed. She was cooperative and appropriate in the meeting. She sometimes made eye contact. She appears to have intellectual functioning disorder. She doesn’t grasp information like everyone else. Her conversation is always one side geared towards her son. She blamed everyone including her lawyer, LHMC and Case Planner of doing anything to help her get her son back. She continues to ambulate with a cane as she continues to quote “her leg is broken” but she is refusing medical attention. She appeared depressed but again she is refusing referral.
On 6/22/2016, CM met with the client and completed Bi-Weekly ILP Review. In the meeting client appears cooperative and friendly. She continues to appear to have some cognitive impairment/short term memories.
She informed the writer "someone" applied for housing for her at the Owensboro housing authority. Writer spoke to her about a mental health provider. She informed the writer she sees Dr. Silva and has not been since December/January. Writer asked pt. if she wanted to schedule an appointment with RVBH to see the psychologist. Writer explained the process of attending a therapist before scheduling with the psychologist. RVBH scheduling department did not answer. She wanted to call RVBH back on Thursday.
On 8/8/2016, CM met with the client to complete Bi-Weekly ILP Review. Client appears to be alert, cooperative and friendly. She was dressed appropriately for the weather. CM inquires how the client has being doing since the last Bi-Weekly ILP Review. Client replies “ok”. CM inquires if the client is more active in participating in onsite recreation activities. Client replies; she is able to participate in some and not all due her psoriatic arthritis.
Mother explained that the client studies but takes the exam fast. PP and FF asked if the school makes adjustments for the client since he has an IEP. Mother shared she doesn’t know. PP encouraged mother to talk to the teachers and make sure the client has his academics needs. Mother agreed to talk to the teachers and once CFS comes back from leave he will go to the school too. FF and PP went over family concerns, ways the wrap team can support them and mission statement. Client shared that he would like to improve his grades to participate in a school sport. PP and FF praised the client for wanting to improve his academics and want to participate in a school sport. Mother expressed her mission is to close her case and not reopen. PP praised mother for sharing and wanting to close her case and leave in the pass. Mother expressed concerns over client behavior at home and school. Mother shared that client talks back and doesn’t follow directions at home. Client shared that he doesn’t like to be the only child doing chores at home. Client shared that he gets frustrated and talks back to mother. Mother expressed that she relies on the client a lot to do chores because he is the oldest. PP
Alex grew up mostly in Olathe, Kansas after his family moved from California when his dad took a job at the Federal Aviation Administration as an air traffic controller. He graduated from Olathe Northwest High School. He went on to play baseball at Mid-America Nazarene University. After one year of college, he decided to enlist in the United States Army.
Justin showed up the visit on time and immediately the children demeanor changed. All three of the children started to run, and be destructive to SOGI House property. EV Staff had to help Justin manage the children the whole visit.
