On 7/30/2015, client walk in the social service office and CM completed Bi-Weekly ILP Review. In the meeting client appears she appeared her stated age and in good physical health. She was satisfactorily groomed & dressed. She constantly throb her forehead, she most of time space out and her affect is flat. Client reported she does have family members here NYC. She has an uncle residing in the shelter systems. She doesn’t have any community resources. CM advises the client to enroll in ACE Job readiness program.
Education Update:
Client would like to pursue getting a GED but she suffers from Clinical Depression which gets in the way of completing projects and programs. CM advised the client to try ACE Job Readiness

Medical Update: Client has no current medical issues. CM observed that the client is having problem with the eyes. CM on numerous occasions referred the client to Laser Eyes but the client was no show. Mental Health Update: Client was diagnosed with Clinical Depression and client reported she wasn’t attending her mental health counseling at BATF (“Bridging Access to Care”).
Substance Abuse Update: Client has no history of substance abuse. Housing Update: Client GP housing package was submitted and circulating.
Client is a possible 2010E, CM will update client psychosocial evaluation and re-submit housing package to PACT. Psychosocial will be initiated on 8/6/2015.
Review ILP and Client Rights and Client Code of Conduct Violations. Facility Rules Violations. Indicate Plan of action to correct violation/s and identify it in the ILP.
Client curfew violated numerous times. CM inquires where the client goes when she CV’D. Client reported for the weeks that she CV’D her uncle was hospitalized, and that the reason why she will come to the shelter after 10pm, missing her curfew. CM reiterates the shelter rule and regulations and for the client to adhere to the
I: CM guided client through ISP goals. CM inquired about updates related to the client’s housing goals. CM used open ended questions to inquired about the client most recent drug use. CM reminded client that starting Monday the 2nd he would have to meet with CCM for weekly case management going forward. CM continued to assess for PTSD symptoms, substance abuse, and medication compliance.

Mental Health Update and Substance Abuse: Client has been diagnosed with clinical depression and she was referred to Woodhull Mental Counseling to address her depression. Client initial assessment is scheduled for 8/25/2015.
On Tuesday March 8th, 2016 Case Conference with Resident Ebony Rice #325 has been conducted with Janette Chirico from DHS , Program Director Felicita Rivera, Housing Program Supervisor Zenobia Garland and Senior Case Manager Ms. Arias. The purpose of this meeting was to address client noncompliance and to come with an exit strategy for this Ms. Rice. Client was asked the reason for her missing ILP meetings and her barrier to obtain permanent housing. Client has a LINCH voucher for the amount of $1515. Client stated that she hasn’t receives any help from facility staff so she is searching for apartment on her own. Ms. Rice has been receiving assistance by the onsite housing department but unfortunately she has been able to link to an apartment
Bensalih reported that she has completed her rule 25 assessment at Tubman and has upcoming treatment sessions with her therapist and with her group. Also, CPSW asked Ms. Bensalih her process regarding Associate clinic psychology. Ms. Bensalih reported that she has not seen her therapist lately and planning to go back and schedule appointment with Associate clinic associate asap. Ms. Bensalih reported that she will let this writer know her up coming appointments with tubman and ASC. CPSW encouraged Ms Bensalih to go back at Associate Clinic Associate for the mental health assessment. CPSW asked about her supervision and needing to schedule appointment asap. Social Services: On 12/08/2016, client Lissy Figueroa met with assigned Case Manager Ms. Gilgen for Intake Assessment and Initial Independent Living Plan (ILP). Client is 21 years Hispanic female. On 7/11/2015, CM met with the client for Bi-Weekly ILP Review. Client arrived early for the meeting. In the meeting client appears to be well groomed with good hygiene and dressed appropriately for her age. She was calm, cooperative and well related. She discussed typical issues re: her political and activism work and automobile accident disbursement. She continues to relate her paralegal was in the neighborhood and she wanted this worker to meet her. Unfortunately, there was no parking and she left. She also mentioned the paralegal brought her documents that she need to filed with the state and the federal for her automobile entitlement.
On 8/7/2015, CM met with the client and completed Bi-Weekly ILP Review. Client arrived early for the meeting; she was alert, satisfactorily groomed, and casually dressed. In the meeting she was cooperative and friendly. CM inquires how client is doing since the last Bi-Weekly ILP Review. Client reported she is exhausted from running around obtaining relevant documents to be submitted to Lemle & Wolfe, Inc. by 8/3/2015. She continues to reports she has Straight Medicaid and she is searching for a Managed Cared that cover oxygen tanks.
On 1/5/2016, CM met with the client to complete Bi-Weekly ILP Review and housing plan. In the meeting client was cooperative and friendly. She was dressed appropriately for the weather.
On 8/30/2017, CM met with the client to complete Bi-Weekly ILP Review. Client was dressed with proper attire for the weather. In the meeting client appears to be sad and quiet. CM inquires the reason client appears to be quiet and sad. Client looked at CM and didn’t say a word.
CM was out on vacation for the period of 8/15/2015 to 8/24/2015. On 8/27/2015, CM met with the client to complete Bi-Weekly ILP Review. Client arrived early for the meeting. She was alert, satisfactorily groomed, and casually dressed. She barely made eye contact and when she did it was rapid. The client’s mood was balanced and her affect was flat. CM inquires how the client has been doing Since the last Bi-Weekly ILP Review. The client reports “not to good” she reports she applied for SSI because of what she is going through. CM requested for the client to describe what she is going through but the client refuses to disclose.
CM was out on vacation for the period of 8/15/2015 to 8/24/2015. On 8/29/2015, CM met with the client to complete Bi-Weekly ILP Review. Client arrived early for the meeting. In the meeting client was dressed in poker dot sweat pants, and green jugging sweater. She was well groomed. She appears to be in pain. Client reports arthritis pain in her shoulder and back and she takes prescribed medications. She also mentioned Asthmas flare up and she uses the pump as needed. She was cooperative and alert in the meeting.
CM was out on vacation for the period of 5/31/2017 to 6/14/2017. On 6/17/2017, CM met with the client to complete Bi-Weekly ILP Review. Client appeared her stated age and appeared in good health. Client was cooperative in the meeting. She was alert, satisfactorily groomed and dressed. Client made eye contact appropriately. Client affects and mood was appropriate. Client was not delusional nor did presented flight of ideas.
It appears that there were delays in requesting medical records and in this case a psychological evaluation was necessary. The medical records were 18 months old, and the consumer had not received treatment for depression since that time. A psychological evaluation could have provided a better understanding about the consumer’s current status and impediments to employment.
