Client Contact: A HV was conducted by the Housing Specialist. The Housing Specialist gave Ms. Sims information about the free online writing course that she could attend at her local library. Ms. Sims seemed excited to enroll into the program and was very thankful for the information. Her home needed to be deep cleaned. All of her utilities were on and functioning. There were no needed home repairs and no record of destruction of the property. The visited went well.

Ms. Smith agreed to have a copy of her I.D. prepared for the next HV. She was asked by the Housing Specialist if she had any goals that she was ready to work on. Ms. Smith stated that she was ready to take her GED test, but wasn’t able to pay for the test due to having limited funds. The client was told that the Case Manger would find out if the agency had funds to pay for her GED test. After the discussion the client was informed about the Home Visit form that needs to be signed by her at the end of the visit for PSH (Permanent Supportive Housing). On Tuesday March 8th, 2016 Case Conference with Resident Ebony Rice #325 has been conducted with Janette Chirico from DHS , Program Director Felicita Rivera, Housing Program Supervisor Zenobia Garland and Senior Case Manager Ms. Arias. The purpose of this meeting was to address client noncompliance and to come with an exit strategy for this Ms. Rice. Client was asked the reason for her missing ILP meetings and her barrier to obtain permanent housing. Client has a LINCH voucher for the amount of $1515. Client stated that she hasn’t receives any help from facility staff so she is searching for apartment on her own. Ms. Rice has been receiving assistance by the onsite housing department but unfortunately she has been able to link to an apartment
Housing: Case Manager informed client that she must meet with assigned Housing Specialist on a weekly basis to discuss her housing opportunities.
The writer help the consumer to complete intake paperwork at Northeast Guidance Center the consumer is a DD and does have problem with reading, and writing. The consumer report during his assessment that he would like to have income, go back to WCCC and have a place of his own. The consumer also states that his mother was murdered when he was young and he live with his aunt whom he call mom and his cousin who often time treat him mean. The consumer is also unable to get around catching the bus because he does have problem reading. The consumer next appointment for his treatment plan is schedule on 01/03/16 at 9:00am. At 0824 this clinician met with the patient who reports her weekend was not unpleasant. She reports she is continuing to have problems locating housing. She reports her family members continue to refuse to allow her to live with them specifically her father and her uncle. She reports she has one friend who is considering allotting her the capability to reside with her under the condition that she speak with her Child Protective Services (CPS) caseworker first. She reports she does not want to reside in a homeless shelter with her son as she has done this before and the experience was not up to her standards.
New Hope Housing likely had strengths related to their employee development program, their effectiveness in housing people in need, and their niche for ensuring individuals who would typically be rejected housing. Some of their weaknesses likely related to their secured funding and current dependence on government funding. Some of their opportunities relate to finding solutions for permanent housing, as this is currently a struggle for the people they serve. Lastly, their threats likely also have to do with their government funding. As budgets change, money is moved around and can impact this organization for the better or for the worse.
On this date worker received a call from Tori Pettit. Ms. Pettit stated that Serv-Pro had visited Ms. Smith's home did an assessment and sent off the mold samples. The results came back that the house is not livable due to mold infestation. The mold is in the walls, ceiling, basement, and air vents. Ms. Pettis has spoken with Ms. Smith and Ms. Rachel Rodgers about Ms. Smith moving ASAP. Ms. Pettit stated that Tri-County could place Ms. Smith into a group home now due to her home being deemed unlivable. However, Ms. Smith is not interested in-group home placement and would rather live independently. Ms. Rachel Rodgers has agreed to assist Ms. Smith with apartment applications and any other thing need in order for MS. Smith to find residence.
REVIEW ILP AND CLIENT RIGHTS AND CLIENT CODE OF CONDUCT: Client is not compliance with the saving contract. CM advises the client to comply with the Shelter Rules and DHS Code of Conduct. On this date, Matthew Mitchell APS supervisor conducted a meeting on behalf of Mr. and Mrs. Beaird well-being. In the meeting, Mr. Mitchell presented Mr. Joey Beaird and Mrs. Julie Johnson with solutions that would allow Mr. Gail Beaird to return home. However, both Mr. Joey Beaird and Mrs. Johnson declined to take care of their parent independently. Each stated their home was not handicap assessable and they had jobs and could not dedicate that much time to their parents care. The other option discussed was hiring another service besides Visiting Angels since they are refusing to continue services if Mr. Beaird returns home. However, this is the only service offered in Jasper. When the discussion of Mrs. Johnson and Mr. Beaird hiring sitter independently they stated they had tried this before and it never worked out.
On 9/15/2016, CM met with the client for Bi-Weekly ILP Review. Client was dressed appropriately for the weather. She was well groomed. As usual client in the meeting was loquacious, and loud. Client continues to be hostile towards this worker. Client continues to repeat to this worker as quote: "I am not mad with you, and this has nothing to do with you. It's all about DHS and your supervisor." Client was hostile due to an Authorization of Release Form client signed for staff to contact her therapist Dr. Iris Yankelevich for a copy of the client psychosocial and psychiatrist evaluations. Client continues to report that she didn’t signed the Authorization of Release Form because the form stated that the form is for HRA 2010e and she doesn’t know how many time she need to repeat herself that she doesn’t meet the ‘CRITERAI FOR SRO”. CM tries to explain to the client that staff is requesting a copy of her psychosocial and psychiatrist evaluation, but then client continue to talk over CM and threat to contact the Coalition and DHS. Client continues to set in her way and refuses to work with staff to meet her unmet needs. CM observed that the client walk with a book bags and a small shopping cart. Client reported she walk with all her documents and letters from Adult Protective Services & Social Security Administration stating that she is capable of living independently.
The client states she was very focus and persistent in her housing search. Ms Quinn is 65 years old and she has health problems. However, she still worked hard to find housing. The consumer was intact functional, he understood about his condition however, was a little confused about his discharge date from the the hospital. The consumer did not understand why he was not able to go to rehabilitation home. The writer and social worker at the hospital explain to the consumer that because he is able to take care of himself does not qualified him for rehabilitation. However, the social worker did refer the consumer to Mollie Care which is an AFC Home on 1485 Holmur in Detroit, MI. According to Nadra social worker at the hospital the consumer is schedule to meet with Latina Kaigler to do intake assessment with the consumer. The writer did go and look at the home on Holmur St to make sure it is suitable for the consumer.

She lives with her 13 moth old child (NM) in a one-bedroom apartment. The client is unemployed and currently receives SSI benefits. AM resides in subsidized housing because of her social security income. Client did not finish high school and has no desire to do so.
