Best Practice Recommendations
Best practices are recommended based off of reviews, interventions and policies with government, academic, health, public and community officials to increase public health care needs via evidence-based findings. Currently in health care, there is an emphasis on improving medical adherence and literacy, by the enhancement of health communication and social marketing materials. These methods will help to foster healthy behaviors, sustain cessation of unhealthy ones, and increase awareness involving disease and injury. The following are a set of recommendations currently being used, to improve health communication:

Because of language barriers, cultural barriers, and limited literacy, visual aids, such as pictures can help deliver health information. Many patients get confused in the numeracy aspect of health care. They pay attention too much attention to one part of a set of number/ratio(s), and much information is lost, making it difficult to make appropriate health decisions. This is known as a term referred to as “denominator neglect.” Visual aids help to reinforce material, rather than distract from the material itself. In addition to being consciousness of different barriers. Not only must the delivery be presented and made available to patients upon initial visits, follow-up should be done as well. Many people struggle taking information in, and lose it by the time they get home, have more questions but are afraid to ask, are pressured for time constraints, or simply just lack access in general. If information is delivered effectively, all bases should be covered before, during and after receiving care and information to prevent worsening conditions.
Improve Internet Tools. Internet tools need to be improved, to cater to those who rely on the world web. While many sites are updated annually, all of the information doesn’t cater to those with low reading levels, different backgrounds, or age groups. Websites should incorporate clear and plain information. If medical terms are needed there should be links, examples or visuals aid to make things more clear. Visual aids should also be used throughout websites period because they are useful for seniors as well. Pictures can help trigger things in seniors with memory/cognitive
The function of a health promotion brochure is to present information to the community on different topics with the intention of improving the health and well-being of individuals through the implementation of health education. In order for a brochure to be effective, the message must be clear, precise, and totally understood by the individual. Some health promoting brochures can provide incentives to maintaining wellness promoting behaviors. The “Health for All by the Year 2000” movement was developed by The World Health Organisation with the aim to promote health and provide an improved quality of life worldwide (WHO 1986b). The World Health Organisation later developed a set of standards for delivering health promotion. These standards state that patient’s must be provided with information on important issues regarding their illness or condition and health promotion interventions should be recognised in all service users care pathways. In my every day practice, though I work with a different patient population, the results of this study show the applicability of health education for family and patients in improving health outcomes. Even with health topics other than diabetes, health education is pivotal to improve knowledge, self-management, and patient compliance. With adequate knowledge in disease processes, patients can make better choices, maintain their health, and adequately prevent complications. Though the demographic aspects will be different, the outcome achieved in this study would remain relatively similar.
Disease prevention should be the center thoughts to everyone and more specifically the healthcare providers. The nurses and the nurse practitioners should promote a preventive measure to all patients that they have seen on a daily basis. We as NPs should be proactive in promoting health care promotions in the community that we are living. “Increasing the focus on prevention in our communities will help improve America's health, quality of life and prosperity” (Centers for Disease Control and Prevention, 2014, 2). Answer: As a part of a high-reliably organization, I am committed to 200% accountability and safety as the number one goal. Over the past year as an Informaticist, I have consistently advocated for patient safety and safe workflows. As an example, a new staff endocrinologist made a request to update the Insulin Basal Bolus Correction order set that was not evidence-based. I met with the endocrinologist to review the current practice guidelines per America Diabetes and Endocrinology Associations. During our discussion. the endocrinologist did not realize that there was the ability within the order set to make the desired changes to individualize the care and orders
Health promotion can be accomplished in various environments. Schools can be utilized for education purposes. Films, brochures, lectures, and class discussions can be routes of teaching about nutrition, hygiene, heart disease, diabetes, hypertension, and abuse. Health departments can distribute pamphlets concerning the importance of vaccinations and preventive strategies against sexually transmitted diseases. Hospitals and medical clinics can conduct health fairs that offer free blood pressure evaluations and cholesterol screenings. They can also educate the public about the serious signs and symptoms of a heart attack, stroke, dehydration, and heat exhaustion.
Due to the Americans with Disabilities Act being passed recently, there are many improvements that need to be made concerning social policy and individuals with developmental disabilities. A best practice for practitioners working with this target population would be a person centered approach. Falon et al conclude, “The aging service system has adopted the disability community’s ingrained philosophies of self-direction, person-centered planning, and participant- directed services” (p. 22). The quest for “best” practices in health care involves implementing available evidence into practice. Despite growing bodies of evidence, moving evidence from peer-reviewed journals and guidelines into practice remains challenging. Efforts have shifted from focusing on methods for rigorously synthesizing study results into practice recommendations and improving guideline quality toward implementation. We are only beginning to recognize processes involved, and many interventions need additional study. When designing online communication tools such as a patient portal, certain factors need to be considered if older patients are going to be successful in their use. Studies tend to show that older patients make more mistakes than younger patients when using computers and computer accessories such as a mouse and keyboard. Browsing for, locating, and retrieving online information can be difficult due to decreased cognitive and perceptual abilities. These changes can strongly affect an older person’s ability to navigate the Internet (Xie, 2003).
