Piperacillin/tazobactum (Zosyn)–Intravenous (IV) 2.25g every eight hours.

Theophylline-300 mg every twelve hours; Maalox 30 mL prn (Gastrointestinal upset); and ibuprofen (oral) 400 mg prn for pain.

The physician orders for blood culture from two different sites and informs the staff nurse to begin the antibiotic regimen as soon as possible.
