WPP Torres consulted with WYP Ngo, the team can go see the youth on December 10th, 2015. Next meeting will be on December 10th 2015.
CM emailed Andrea Orlando (Daytop Program Coordinator) a copy of Dre’quan’s (youth) updated biopsychosocial (BPS) and court order. Ms. Orlando reported the clinical director will review youth’s documents and will determine admission date. Ms. Orlando will continue to keep CM updated regarding youth’s status. CM provided CM’s contact information.
The foster contacted WYP Ngo to cancel the one on one with the youth today, because the youth was placed in custody for robbery. The foster will keep WYP Ngo updated when the youth has court for his violation. WYP Ngo contacted the WCC (Wraparound Care Coordinator) as requested from the foster father. WYP Ngo updated the WCC about the situation.
Case management services were provided. The youth is in custody for 3-4 months. The youth was at court this morning. The mother declined to take the youth home. The youth is still pending court trial for another offense. The WCC Tran did not get permission for the team to visit the youth, due to the probation office is not responding. WYP Ngo is waiting for permission to visit the youth. WCC will update WYP Ngo. The next one on one will be scheduled as soon as possible.
CM was unable to reach Ms. Williams (caregiver) in regards to 30 Day CFT meeting for Nsilo (youth). CM left caregiver a detailed message and reported CM will be on vacation on Monday, 8/7/17 for upcoming court hearing. CM requested to meet with the family prior to court hearing to conduct 30-day CFT meeting at the youth detention center located in Teterboro, NJ. CM provided caregiver with CM’s contact information.
WCC Taylor updated WYP Ngo the youth was released from custody and advised WYP Ngo to not go out into the field. WYP Ngo will have to schedule a meeting with the youth secondary probation officer to meet on site with the youth in YRC (Youth Reporting Center). The youth will be living with his grandmother, due to his mother does not want to take him back. The youth is planning to go back to his mother. The next one on one will be on May 30th, 2017.
I presented Esteban’s case during case consultation because I noticed that I had countertransference when doing his intake. My expectation when presenting this case was to receive feedback that would help me to provide effective treatment to Esteban without my countertransference affecting his treatment. This section of the paper will talk about the feedback I received about my countertransference and how to support Esteban.
CM conducted a face to face meeting for Kiara Gelin (youth) at the Jersey City Medical Center Partial Hospitalization Program. In attendance were Jasmine Alexander (CM) and Kiara (youth). CM and youth discuss therapeutic services; youth reported she enjoys attending the program. Youth admitted to using substances and noted she will stop. CM and youth discussed employment. Youth is a part-time employee at Honeygrow restaurant in Hoboken, NJ. CM explained to youth Hudson Partnership CMO contact policy. CM requested for caregiver to contact CM to schedule CFT meeting and to discuss services. CM provided youth with CM’s contact information to provide to caregiver.
The interns, [Robert Chandler (Garth), Jose Morales (Tam)] and I created a presentation outlining recommendations with the Shared Services meeting procedure. This included how long the meeting last to possible break opportunities. Through observation, specifically with the Architecture, I noticed excellent facilitation techniques from Gary and the overall respectfulness of the team. Everyone was open to new ideas and passionate about their work and ultimately wanted the best delivery at hand as a team. The SharePoint sites collectively are easy to navigate. I could not provide necessary suggestions that I thought would be significantly helpful for improving the sites, however as I continue to navigate it-I am sure there is something that can
Sergio Paredes (Court Liaison) faxed CM, Dre’quan (youth) court order from today’s (1/20/17) court hearing. Youth has to continue with court GPS bracelet, follow BPS recommendations, and must obey Sasha Walker (caregiver niece) rules. Youth next court hearing is 2/10/17 at 1:30pm.
CM spoke to Sebastian Bliffeld (IIC) regarding services for Leandro (youth). IIC and CM discussed the youth current situation and different residential treatment programs. IIC stated that he will meet with youth on Saturday, 12/3/16 at 10:00am. CM informed IIC that youth scheduled f/f meeting is on 12/6/16. CM will continue to keep IIC update regarding the youth.
WYP Ngo discussed with the youth about coping tools. The youth would like some video games as a coping tool. WYP Ngo will discuss with the team about it. WYP Ngo discussed with the youth about boxing. The youth thinks boxing will keep him away from using substances. The youth is not using substances while under house arrest. WYP Ngo will inform the team about this. WYP Ngo discussed with the youth about employment. The youth will be released from house arrest on October 21st, 2017. WYP Ngo will assist the youth with looking for employment after October 21st, 2017. The youth is open to the idea. The next one on one will be on October 10th, 2017.
