Attempt CC =1 CM was unable to reach Ms. Williams (caregiver) in regards to 30 Day CFT meeting for Nsilo (youth). CM left caregiver a detailed message and reported CM will be on vacation on Monday, 8/7/17 for upcoming court hearing. CM requested to meet with the family prior to court hearing to conduct 30-day CFT meeting at the youth detention center located in Teterboro, NJ. CM provided caregiver with CM’s contact information.
The newborn and infant physical examination, NIPE, is performed by a qualified NIPE practitioner (Department of Health, DoH, 2015; Public Health England, PHE, 2013), within the first seventy-two hours of an infant’s life. This screening consists of a head-to-toe examination of the infant, including, but not limited to: the heart; the eyes; and the hips. During the examination, the practitioner will take the time to promote good health and wellbeing of the infant, by teaching the family how to identify signs of a well and unwell infant (Someren, 2013). The intention of the examination is to assess whether the infant has any congenital abnormalities (DoH, 2015; PHE, 2013). The NIPE is a developmental examination which is replicated between six and eight weeks after the infant’s birth, to screen for congenital disorders that become prominent in their physiology within this time period.
CM spoke to Theresa (FSO partner) regarding FSO services for the family. CM informed FSO partner that the family is interested in FSO services and the scheduled 30-Day CFT meeting is on 12/12/16 at 3:30pm in the family’s home. FSO partner reported that she would not be able to attend today’s meeting due to a schedule conflict and I she will try to get someone to cover the meeting.
CCIB Intake received a call from owner/administrator Therese Kragness who disclosed that the local Law Enforcement was present at the facility. According to the caller, an employee who was having difficulty with her boyfriend received a text threatening to come to the facility. Fearing for her safety, the employee notified Law Enforcement and was escorted to the Police Station to file a restraining order against the boyfriend. Conversely the boyfriend did not appear at the facility. The Administrator will fax the hard copy of the report identifying the employee within the required time
Spoke to Stacy Anderson (DOB 10/24/77) who stated that her daughter’s father, Sandro Carpao (5/17/78) , refused to give her daughter this morning. Anderson had a copy of court order stating that her daughter’s father gets her daughter from Thursday to Friday Morning there was no time listed on the order. Spoke Carpao who stated that he planed to visit his family in 3 Even Ave Hightstown NJ with his daughter and that he wanted her until 1600 hrs today. Anderson and Carpao came to an agreement to do custody exchange at that location in Hightstown at 1500 hrs today. Anderson was advised to contact the Hightstown Police department for smooth exchange as Anderson stated that they have been issues with exchanges. Anderson was also advised to contact

This case was last before the court on 1/4/2017 for a report to the court. The hearing was rescheduled until 2/6/2017, as all the attorneys were not present. At that time, temporary custody of Issac Johnson remained with the Department. The case was continued until 2/6/2017 for a status report.
CM contacted Mrs. Rhiel (Caregiver) to inform her that CM is outside the family's home for f/f meeting. Mrs. Rhiel stated that CM can have the f/f meeting with youth due to caregiver not being home at the moment. Also, CM reported that she will be away on vacation beginning 12/16/16 until 1/3/17. CM stated if caregiver has any questions or concerns to contact CM supervisor, Alison Scarpignato. CM provided CMS contact information.
2. The previous NCOER NCOIC departed unexpectedly two months ago due to a compassionate reassignement, and he was out the majority of the previous sixty days prior to his departure due to medical and personal issues. As such, the corrections the battalion CSM required on the NCOERs were not done and the NCOERs were not processed in a timely fashion.
Jasmine Alexander (CM) conducted the 1st Discharge CFT meeting at Mrs. Edman’s home on August 8, 2016. In attendance were Destiny Tyler (youth), Mrs. Christine Edman (parent), Jasmine Alexander (CM) and Charlene LeBrun (CM). During the meeting, CM reviewed the family’s crisis, ISP, and completed the Strength and Needs Assessment.
Montgomery County DHR Child Support caseworker’s (CSW) testimony revealed that on September 11, 2015 the custodial parent (CP) telephoned the office and left a voice mail stating she received a letter regarding her children’s father, the non-custodial parent (NCP), George Johnson, who is in prison; she cannot do anything about that. The CSW returned the telephone call and informed the CP that the appointment was for the child in the case with the NCP, James Hall. The CP stated that she did not know a James Hall. The CP was informed that the information was received from the Temporary Assistance for Needy Families (TANF) caseworker. The CP stated that she made a mistake. The CP was informed that she could keep the appointment or be sanctioned
Sergio Paredes (Court Liaison) faxed CM, Dre’quan (youth) court order from today’s (1/20/17) court hearing. Youth has to continue with court GPS bracelet, follow BPS recommendations, and must obey Sasha Walker (caregiver niece) rules. Youth next court hearing is 2/10/17 at 1:30pm.
CM conducted a CFT meeting for Taina Ortiz (youth) at the family’s home. In attendance were Jasmine Alexander (CM), Taina Ortiz (youth) Catherine Quinones (caregiver) and Maureen O’Dea (IIC). The strength and needs assessment was completed and the Family Crisis plan was reviewed. CM continued to explain IIC services to the team. CFT discussed appropriate number of hours for IIC service and target areas for the provider to continue to focus on. CM asked the family if they would like to continue with same provider; the family were in agreement with the strategies discussed and will continue to work with the same provider.
Resident is 87 years old with moderate Dementia. The resident spends most of their time either watching TV from their wheelchair, or quietly watching the staff and other residents from the lobby.
CM received a call from Kim Shipmon (DCP&P) in regards to a follow-up for CFT meeting that took place on Friday, 3/10/17 at 1:00pm. DCP&P and CM discussed needs and strategies for Joseph (youth). CM informed DCP&P of youth’s improvements in school and in the home. CM and DCP&P talked about mentor services for youth; DCP&P will complete the application and follow-up with referral. DCP&P approved for youth to continue with therapeutic services. CM noted next CFT meeting is scheduled for Thursday, 5/11/17 at 5:00pm.
CM spoke to Sebastian Bliffeld (IIC) in regards to services for Leandro (youth). IIC informed CM of a program called FEDCAP (not-for-profit organization that provides vocational training and employment resources to those who face barriers to employment). IIC stated that this program would be a good fit for Leandro. CM reported that she will lookup the program and update IIC with more information regarding FEDCAP. Also, IIC provided CM with the family's new home address for tomorrow's CFT meeting at 9:30am.
