She then became agitated with staff, and threw over the cards that were on the table. She then walks and the bathroom that was open with a staff member at the door, as a peer was in the shower, and knock over the trash can, staff attempted to process with her as she just was sitting inside the tub. Staff gave Christina some time away, but she eventually came out and attempted to charge her peer, but staff was able to use proximity to keep her from hitting the target peer. During my first day of clinical, I encountered an issue that I believe is very significant. As a student nurse, our duty for this day was to follow our health care aide around the ward and assist in completing resident care. The resident required assistance in many of her daily tasks. The health care aide asked if I would perform one of those and do perineal care for her. I turned down her offer because I did not feel comfortable with my skill level. The resident had a bowel movement during the night. There was a significant odour in the room that overwhelmed me. I really wanted to leave the room because it was so unpleasant, but I stayed in the room so that the resident would not be embarrassed. This feeling of embarrassment, I assume,
The reporting party (RP) stated resident Arev Dabbagh DOB: 12/27/00 disclosed on 4/15/16 she inappropriately touched by a male peer. According to the Arev on 4/14/16 a peer Ibraheem Mamoori DOB: 9/2/98) placed his hand on her shoulder and continued downwards towards her breast. The peer touched her breast over he clothing. The resident was distraught while she recounted the incident. Arev alluded to their being other incidents with the peer and other unknown males. Arev didn't disclose details or clarification. Arev was placed voluntarily in the facility by her mother and will continue her placement for a few more weeks. The peer is currently placed on a 5150 hold and will not have access to Arev in the future.
During my clinical rotation during my last semester of nursing school, I was able to work one on one with a BSN degree nurse named Judy in the ICU. Judy had three years of experience in the ICU setting. She had been a medical surgical nurse prior to her ICU transfer. The ICU at this hospital consisted of two associate degree level nurses and two BSN level nurses on my shift. I rotated three days in this particular ICU. I worked with Judy all three days of my rotation. I was excited about being placed with her for she seemed knowledgeable and skilled. We were given a male post trauma patient to work with all three days. This patient was a 30 year old male admitted for trauma related injuries and was considered unstable and was to be monitored in ICU. This patient had been involved in a motor vehicle accident and
CCIB LPA Perryman-French received a call from a parent who was trying out this day care. She brought her one year old child to the program and then snuck around the corner to watch how the child adjusted. (Her child hung on to the gate for a few minutes and then wandered into the classroom. Caller then watch a Ms. Veronica grab a toy (or something) out of a little boy's hand (under two years old) and say "Give me that." Her grab was forceful enough and her tone loud enough that caller immediately reacted. Caller walked away for maybe a half hour, then could not handle the bad feeling she had. She returned and found her child had red swollen eyes and snot on the face, was clearly upset, but the teacher did not comforting, instead, just told
CCIB received a telephone call with an allegation against the facility. RP called to report that there is drug use and domestic violence happening in home due to the licensee runs a daycare. RP reported that on 10/9/2017 Veronica and Juan went to pick up children from school and Veronica asked staff at the school to call the police. RP reported that staff told her to contact the police to report what is happening. The RP reported that the police came to the school and arrested Juan in the parking lot. RP stated that it was stated on the police report that Veronica was hit in the face and kicked (unknown when incident happened). RP reported that the police report stated Veronica had a cut lip and red marks on her legs. RP also mentioned on 10/10/2017
On Thursday, August 27,2015, @ 5:29 pm ,Tyneice Daishae' Key( DJS Youth ) was redirected by Dennis Thomas ( Senior Residential Staff) to return to Tours Unit. Tyneice Daishae' Key( DJS Youth ) became verbally aggressive towards Telethia Menefee ( Residential Staff). Tyneice Daishae' Key ( DJS Youth) also began to curse and threaten to hit April Smith ( Supervisor of Tour Unit). Tyneice Daishae' Key( DJS Youth ) was redirected to her room to take time away. Tyneice Daishae' Key( DJS Youth ) was able to return to the unit and go to her room. Once in her room Tyneice Daishae' Key (DJS Youth) began to use her chair to hit the door with. During my clinical competency placement, I was working on a surgical ward when a registered nurse on duty asked me to assist Mr. A with his shower. This incident happened on the fifth day of my clinical practice. He was a dementia patient and had undergone right knee total joint replacement. She also informed me that the patient did not like too many people in his room because of his dementia. When I went into his room, his wife was there with him. I talked to the patient about having a shower and getting dressed to look smart and he agreed to have a shower. The patient got out of the bed and walked to the bathroom and sat on the shower chair to have his shower. Then I asked his wife if I needs to stay with him to assist with shower, she said she can help him as she was taking care for him at home since he has been diagnosed with dementia. Therefore, I left the patient with his wife to help with his shower and told her to ring the bell if she needs any help. After some time I left the room, the wife rang the bell. As soon as I entered the room, I heard him shouting at his wife and she started crying and left the hospital. So I had to stay with him. He was very capable of washing himself and I just had to help him wash his back as he requested. After he had washed, I asked him if he was ready to get out of the bath, he started shouting at me.
Whilst on duty on a general ward I was asked by my senior nurse, if I could go down to the pharmacy to pick up some new medication for a new client, who would be needing them at lunch time. On my way to get them, I was approached by another health care assistant who requested my help with a client, who was lying in their own faeces. Quinn Hogan was sitting in the living room during his 5 minutes break. When the timer went off, staff Augustine Oloyede prompted Quinn Hogan to the gym for his exercise block. Quinn Hogan started to engaged in property destruction; banging on wall,window, flipping the table and attempting to pass through the living room window. Staff Augustine Oloyede blocked Quinn Hogan from passing through the window. Quinn Hogan then attempted to dart through the main door, and staff Augustine Oloyede called for assistance. When Martin N arrived, Martin N attempted to block Quinn Hogan from darting out of the house. Quinn Hogan started to engage in aggression (pushing, grabbing, hitting and scratching) towards staffs Augustine Oloyede and Martin Nganga.
This writer escorted the patient to Nursing Coordinator Kesley office as the Nursing Supervisor was not in her office. Upon entering Kesley office, Kesley was having a discussion with another nurse and this writer apologized for the intrusion. This writer addressed to the Nursing Coordinator that the patient is experiencing bedbeg and the patient is aware he will not be dose by the Nursing window, only curbside. It appeared that Nursing Coordinator was being abrasive towards the patient as she explained to the patient as to what is needed before the patient can reenter the clinic. The patient then became agitated and shouted at Kesley and says, " Kiss my Ass, " and then proceeded storm out into the lobby area.
CCIB Intake received a call from Katherine Wills (559) 786-5780 who disclosed that she is an epileptic. According to the caller she was diagnosed approximately 13 years ago and has worker for Senaida Garcia CDC for approximately 5 years. During her tenure at the Developmental Center they have provided her with an aide especially when she is outdoors due to her seizures. Consequently the caller has suffered seizure episodes during her care and supervision time with the children, fortunately an aide or other teacher has been available during those times. Recently due to schedule changes she is left alone daily from 3:15PM to 3:30PM when the aids are no longer available. At 3:30PM she joins the other class for the last hour of her day. The caller
Neuville peers approach Twyla about thepPhysical altercation that occurred early in the school day. Staff intervened quickly and removed Twyla off the Unit into the Neuville / Welbers hallway. Neuville and Tours students were transitioning from the center courtyard back to the Unit. Neuville students were in the process of going up the steps leading to the Neuville/Welbers units’ hallway. Tynoura C entered the stairway area, unauthorized and against staff advisement. Tynoura C ran up the stairway and was able to gain entry to the Neuville/ Welbers hallway. Tynoura did not appear to be upset with Twyla but when her peer stated “get her biddy,” Tynoura attached Twyla. This resulted into a physical altercation; Twyla was taken to nursing to
About 2:30 in the afternoon, M.E. was in her patient’s room and J.P. confronted her in front of the respiratory staff and students that were present about why she was/wasn’t doing certain things and what was making her so slow; J.P. caught M.E. off guard and was very abrasive, belittling and verbally abusive; M.E. felt attacked and embarrassed, especially since this behavior took place in front of colleagues. J.P. felt frustrated that M.E. was not more competent and efficient in her care and confronted her about it. After the confrontation, there were no words spoken between the two of them for days and even though months have passed, there is still an obvious tension and unresolved conflict between these two individuals. There is currently a noticeable effort being put forth by both women but the conflict they experienced is not yet fixed.
