CCIB LPA Perryman-French received a call from a parent who was trying out this day care. She brought her one year old child to the program and then snuck around the corner to watch how the child adjusted. (Her child hung on to the gate for a few minutes and then wandered into the classroom. Caller then watch a Ms. Veronica grab a toy (or something) out of a little boy's hand (under two years old) and say "Give me that." Her grab was forceful enough and her tone loud enough that caller immediately reacted. Caller walked away for maybe a half hour, then could not handle the bad feeling she had. She returned and found her child had red swollen eyes and snot on the face, was clearly upset, but the teacher did not comforting, instead, just told
As indicated by the file two (2) T.A.’s accuse Kerin of slamming a child’s head back against a bookcase following a time out. An LPN in the classroom did not observe anything wrong although it is unclear how much she actually saw. Kerin was placed on administrative leave and while she was out it was learned that Kerin was providing a protein supplement to another student for the past several months. Kerin says that she notified the parents about this but via daily notes, but there does not appear to be any written confirmation, although you would think the parent might have the notes. The decision to provide this supplement was discussed with the treatment team including the speech pathologist and possibly the MD. CCIB Intake received a call from licensee Gurpreet Kaur who stated on 8/29/16 at approximately 3PM day care child Arya Naik DOB: 12/14/13 during nap time (2:15PM) the child began to make strange noise. The licensee stated she called out the child's name and the child didn't respond. The child's eyes were open and she was breathing. The licensee stated 911 was contacted and emergency responders responded within 15 minutes. After contacted emergency services the licensee contacted the child's parents. The child's mother arrived at the home while the emergency responders were still at the home. The child was transported to the local hospital and diagnosed with double ear infections per the child's father. The licensee will submit the written report
The reporting party (RP) received information from Alta Regional Center regarding foster parents. It was reported the foster father threatened a foster child say "if you don't stop this I'm going to punch you in the face." According to the RP an in home support services employee was present and overheard the threat and contacted Kang Thao (916) 978-6456 with Alta Regional Center. The RP stated the exact date is unknown, however it was recent. The foster parent met with 2 social workers in their home after the visit the parents became upset at the outcome of the visit and began to swear in the presence of the foster children. Also present were the foster parents' two adopted minor children.
The reporting party (RP) is group home youth Laura Qi DOB: 4/11/03 DOP: 9/22/16 SCHOOL: Clyde L Fischer Middle, 1720 Hopkins Dr., San Jose CA 95122-1632 SW: Irene Lee (408) 501-6650. The reporting party (RP) stated she has been bullied for the past 4 months by both the group home youth and the staff members. According to the RP she has been called names, physically hit and punch by other group youth, and threatened with physical harm from staff members. These incidents has causes the RP to feel unsafe and place her at risk for harm in the group home. On 1/22/17 the RP disclosed she was hit by another foster youth and reported the incident to a staff member named Dan McGirt, however, Mr. McGirt didn't believe her and stated to her "you are full
UCM: CPSW did a home visit for Lance and Ashley. Both children were outside playing with their bicycle. Ashley seemed to be engaged little more than Lance during the visit. Both stated that they are excited that school will start soon for them. The foster parent ( Ms. Dawn) stated that Lance still sees his therapy and it seems to help him with his aggressive behavior at the day care. Ashley was standing next to the CP and quietly listening to the conversation. Ms. Dawn asked Ashley if she can go and play while grownups are talking. Very quickly Ashley got upset and started screaming. Ms. Dawn tried to help Ashley to calm down and it escalated more. CP gave little time Ms. Dawn so she can help Ashley to calm down. CCIB LPA C. Perryman-French received a call from Gina, her son (2 years old) attended the program from 02/01/15 until 0714/15. When Gina's son was admitted, Nova (licensee) assured her that the ratios would not be more that 1:8. Gina understands that CCL requires a 1:12, however, on several occasions in May, June and July there were more than 24 children to the two teachers. When she approached Nova, Nova stated that she counted in the ratios even though she was in the administrative office. Gina's son received a scrap from a cinder block that is used to hold up a play structure. He was in a wading pool near the block and apparenly slipped on the wet floor/ground and scratched him arm. On 1/6/16 at approximately 12:05PM the reporting party (RP) was alone with foster child Michael Martinez as they worked on a puzzle on the floor of the resource room. The RP questioned Michael regarding the prior day after school. Michael answered "stupid, I had to stay in my room until dinner." The RP asked why and Michael responded "I don't behave right at school." The RP asked what else did he do and Michael responded "got spanked, and then spanked myself in my room. I hit the crap out of my butt." The RP asked if he played with toys at which Michael responded "yes after I spanked the crap out of my butt because I'm bad." On 10/16/2015 I spoke with Nelda Shavers the director who stated she became aware of the incident when Brittany Shavers had came in the office around 9:30 am and stated Kaylee P. was having a seizure on the playground. Ms. Shavers stated Brittany grabbed the phone and called 911 while she went to tend to Kaylee on the playground. Ms. Shavers stated she rolled Kaylee over on her side. Ms. Shavers stated there was another class on the playground adjucte to the playground that Kaylee and her class was on. Once Brittany had called 911 she gave the phone to Ms. Shavers who continued to stay on with 911 while Brittany and D'Andrea Franklin took the children on both playgrounds inside. Ms. Shavers stated Kaylee's teacher Shana Myers had went in to
The reporting party (RP) stated a teacher named Charlotte will remove children's lunches as a form of punishment (no person listed on facility personnel report). According to the RP these incident are frequently observed. The RP stated the teacher in the "Orchid" classroom will turn the music up really loud to scar the children to become quite. The RP stated the teacher in the "Lavender" classroom is consistently out of ratio. The RP stated the teacher provides care to 13-14 children without assistance. The RP stated a child named Natalie who is allergic to cinnamon was given "Cinnamon Toast Crunch" cereal and broke out in hives. The RP stated the diaper changing schedule which provides that diapers are changed every two hours is not followed.
RP who is a neighbor next to the childcare home at 163 Avenida Victoria called CCIB on 7/31/15 stating that for about the past year the licensee has been letting the children at the child care home play outside for long extended periods at a time. RP states that the licensee will put the children outside in the front play area between the hours of 7:30am to 11:00am and will bring the children back outside at about 2:15pm to 6:30pm. RP reported that yesterday (7/30/15) there was one particular little girl (age and name not provided) who was screaming and crying for approximately five minutes. RP states that there were about three staff members outside at the time when the girl was crying
RP is Spanish speaking. RP stated that her son Phillip Cruz Orozco (age 4, DOB 6/28/11) is afraid to go to school. The child disclosed that staff Adam is always "bothering" him. RP stated that he is continuously chastising the child. When the child's father picked him on Friday 3/18/16, the child told asked his parent if he would have to go back to school. RP told the child that he would have to return to school in 2 days (after the weekend). RP disclosed that the child cried on Saturday, Sunday and on Monday morning when it was time to go to school. RP stated that the child has never acted this way and she did not send him to school that morning. RP stated that she was not able to go to work because the child was so upset about going to school.
The nursing role in this setting was screening of preschools and early head start children for developmental delays and abnormities. The different here is everything is brought to the children. The children/patients are pulled in groups of three, and some of them were pulled out one at time. The nursing students and Dr. Lee looked for signs like how do the children walk up steps compared to their age group. How do they hold a crayon? Can they throw a ball? Are they overweight for their age group? Do they have any signs of ear infection? We also assessed for lead toxicity in the blood. Many of these children did not like this. We found that distraction at this time results in the child not crying. The only requirements here was consent of the parent/ guardian and bring their child to daycare/school.
In a nursery a child minder is only allowed a certain amount of children at a time with them, due to having too many and not abiding to the ratio, if the child minder got distracted with one of the other children and another child had accidently swallowed an object this could cause the child to choke on the object they would have to try and get the object out of the young child’s mouth if this fails to work they would have to ring a ambulance , if the child minder had all the children in sight where she could see all of them this incident would have occurred. This could link to expose to danger.
This case began with couple Mr. and Mrs. Keller opening up a daycare in Austin Texas that was overwhelmingly positive and filled with fun activities for the kids that may attend. All of this came to a halt though, when one day a three year old girl who was already seeing a therapist for mental issues told her mother that Mr. Keller had assaulted her when she was at the daycare.
The actions that the two teachers had demonstrate can be describe as inhuman and horrible. A number of children was sent to hospitals for checkups on their physical and mentally health, as parents are very concerns. The preschool is now temporarily closed down as the police is still continuing investigate the incident and looking for proofs if this has happen in the past. Further announcement will be make after Mrs Taylor and Ms Kelly appear in court next month. L&M
