The Reporting Party (RP): A Case Study

The reporting party (RP) stated resident Arev Dabbagh DOB: 12/27/00 disclosed on 4/15/16 she inappropriately touched by a male peer. According to the Arev on 4/14/16 a peer Ibraheem Mamoori DOB: 9/2/98) placed his hand on her shoulder and continued downwards towards her breast. The peer touched her breast over he clothing. The resident was distraught while she recounted the incident. Arev alluded to their being other incidents with the peer and other unknown males. Arev didn't disclose details or clarification. Arev was placed voluntarily in the facility by her mother and will continue her placement for a few more weeks. The peer is currently placed on a 5150 hold and will not have access to Arev in the future.

On 08/20/2017 at 1002 hours FTO Wheeler #4361 and I responded to St. Luke’s Hospital emergency room entrance, on 3555 Cesar Chavez St., regarding a women in a hospital gown trying to break the caller’s vehicle window with a brick. Officer Wheeler and I were in full police uniform driving a marked patrol unit when we responded.

The RP stated that the resident is very lucid, alert, and was very uncomfortable about going in her diaper. The RP stated the resident is fond of sports and enjoys watching sports on the television. The RP stated the resident was enjoying the television when she was told she had to go to bed. When she complained a caregiver named Robert go into the resident's face and told her that she was going to bed. According to the RP the resident was intimidated by the caregiver. Although the resident no longer lives in the facility she fears that the caregiver will find and harm her. The reporting party stated on one occasion the resident was having respiratory problems and the facility contacted the RP. The RP questioned that staff and asked if the resident had a fever. The RP was told that the resident was agitated therefore the RP went to the facility to check on the resident. The RP asked the caregiver to take the resident's temperature using a digital thermometer. According to the RP the caregiver did not know how to operate the thermometer therefore requiring the RP to take the resident's temperature. The resident was taken to the hospital and diagnosed with

This case involves Ms. Brianna Mona Haggins a fourteen year old Minnie Howard School student. Briana left home January 6, 2016 and was returned on the same day by Officer Hudson. On January 11, 2016, a call of service for a domestic assault was reported for 6300 Stevenson Ave Apt# 611. In this incident (16-102714) Brianna and her mother (Sherry Haggins) were determined mutual combatants.

I then went through the family violence packet with Odelia Butler. Odelia Butler told me the during the physical altercation Rodeny Butler struck her on the right shoulder blade region causing her physical pain and injury. Odelia Butler then filled out a voluntary statement form which will be attached to this report. Odelia also stated she did not want an emergency protective order and was not going to the hospital. Photographs where taken and will be attached to this report, while taking photographs I did not locate any discoloration to Odelia Butlers right shoulder blade

CCIB received a telephone call with an allegation against the facility. RP called to report that there is drug use and domestic violence happening in home due to the licensee runs a daycare. RP reported that on 10/9/2017 Veronica and Juan went to pick up children from school and Veronica asked staff at the school to call the police. RP reported that staff told her to contact the police to report what is happening. The RP reported that the police came to the school and arrested Juan in the parking lot. RP stated that it was stated on the police report that Veronica was hit in the face and kicked (unknown when incident happened). RP reported that the police report stated Veronica had a cut lip and red marks on her legs. RP also mentioned on 10/10/2017

The reporting party (RP) received call from Diego Baca with Kings County CPS, 1400 W. Lacey Blvd., Hanford, CA 93230 (559) 852-2345 who reported that on 12/21/16 at approximately 3PM while conducting a placement visit for youth Andrew Yanez DOB: 12/18/01 the youth and the social worker took a walk. During the walk Andrew disclosed that he and a staff member named Shawn Wilcox were involved in an altercation. According to Andrew at approximately 2:45PM he was in the office of the ground home with 2 other staff members named James Boston and Nicole Juarez. Andrew was requesting the staff members to take him to the store to use is Christmas gift cards, when staff member Shawn arrived and asked Andrew to exit the office. Andrew said no because

I entered the residence and was directed to a bedroom on the north side of the residence. I entered the bedroom and observed a white female standing next to the bed. She had a large, blue in color, lump under her left eye. She was unable to open her left eye very much. I later identified her as Anna Williams by Arkansas drivers license. I asked Anna what had happened and she said she was trying to leave the residence and they hit her. I asked Anna who "they" was and she said Gilbert and Judy. Anna said they hit her with their fists. Requested to 605 West Penn Street for an unknown situtation. Schuylkill County Communication Center advised they could hear a lot of screaming in the background. Mrs. Drew stated “Saniya was at her great-grandmother Norma Drew’s home Mrs. Drew noticed Saniya had a knot on her forehead being concerned she asked Saniya how she got the knot on her forehead. And Saniya said she was hit. Norma the asked Gerald Daniels how Saniya got a knot on her forehead and Gerald said she fell playing with her cousin. It was reported that when staff Elizabeth Gomez of UCP-Work day program (Facility #425800283) arrived to Sloan Terrace, she noticed 3 bruises on client Thomas Pankratz's (DOB: 04/09/71) upper left arm. Two of the bruises were the size of a quarter and the other was a size of an egg. The bruises were yellowish and green in color. Staff asked the client what happened and the client stated that he was horse playing and a staff member (unknown) from Novelles Day Program hit him in the arm. No additional details were

The reporting party (RP) stated on 11/10/16 the biological mother of David Jiron DOB: 3/6/11 reported he was transported to his visit with her on 11/8/16 without a booster car seat by social worker Liz Cardenas. The mother also stated the social worker grabbed David's arm and hand to prevent him from swinging a rake. Attached is an email from the social worker explaining the reason for not providing the booster car seat. The social worker stated due to the other two car seat there was not enough room for the booster seat therefore the child was secured with a seat

RP who is a neighbor next to the childcare home at 163 Avenida Victoria called CCIB on 7/31/15 stating that for about the past year the licensee has been letting the children at the child care home play outside for long extended periods at a time. RP states that the licensee will put the children outside in the front play area between the hours of 7:30am to 11:00am and will bring the children back outside at about 2:15pm to 6:30pm. RP reported that yesterday (7/30/15) there was one particular little girl (age and name not provided) who was screaming and crying for approximately five minutes.
