The Reporting Party (RP): A Case Study

Reporting party (RP) called CCIB to cross report that resident Gerald Gilliland (DOB: 4/22/41) reported that while he resided at this facility, unknown male staff locked him in a room against his will. It is unclear what room the resident was locked in because resident was unable to convey any details about the room. Resident then stated that it was all of the nursing staff who locked him in the room. Resident was hospitalized at Kaiser South Sacramento on 9/18/16 through 9/22/16 and is currently residing at Eskaton Greenhaven. RP stated that it is unclear if the resident has Alzheimer's or not. RP stated that the original reporting party is Sacramento Sheriff's Department (Report#16-286176) and RP will forward the SOC 341 to CCIB.

The RP stated that the resident is very lucid, alert, and was very uncomfortable about going in her diaper. The RP stated the resident is fond of sports and enjoys watching sports on the television. The RP stated the resident was enjoying the television when she was told she had to go to bed. When she complained a caregiver named Robert go into the resident's face and told her that she was going to bed. According to the RP the resident was intimidated by the caregiver. Although the resident no longer lives in the facility she fears that the caregiver will find and harm her. The reporting party stated on one occasion the resident was having respiratory problems and the facility contacted the RP. The RP questioned that staff and asked if the resident had a fever. The RP was told that the resident was agitated therefore the RP went to the facility to check on the resident. The RP asked the caregiver to take the resident's temperature using a digital thermometer. According to the RP the caregiver did not know how to operate the thermometer therefore requiring the RP to take the resident's temperature. The resident was taken to the hospital and diagnosed with

This case involves Ms. Brianna Mona Haggins a fourteen year old Minnie Howard School student. Briana left home January 6, 2016 and was returned on the same day by Officer Hudson. On January 11, 2016, a call of service for a domestic assault was reported for 6300 Stevenson Ave Apt# 611. In this incident (16-102714) Brianna and her mother (Sherry Haggins) were determined mutual combatants.

Reporting party resident Michale Dean Lopez (DOB: 11/3/58) stated that approximately 2 months ago, that he was lying down in his bed and felt something behind him. Resident stated that he jumped up and turned around and resident Chris (last name unknown) was standing at his bed side with his boxers down and his "thing" sticking out. CCIB received SOC 341 reporting that during a meeting with the resident, James Brown (DOB: 09/18/84), social worker from Solano Adult Day Healthcare Center, the resident stated that staff Marco has hit him on multiple occasions. The resident kept repeating that he did not want to return to the facility. It was reported that when staff Elizabeth Gomez of UCP-Work day program (Facility #425800283) arrived to Sloan Terrace, she noticed 3 bruises on client Thomas Pankratz's (DOB: 04/09/71) upper left arm. Two of the bruises were the size of a quarter and the other was a size of an egg. The bruises were yellowish and green in color. She had a large, blue in color, lump under her left eye. She was unable to open her left eye very much. I later identified her as Anna Williams by Arkansas drivers license. The reporting party (RP) stated on 11/10/16 the biological mother of David Jiron DOB: 3/6/11 reported he was transported to his visit with her on 11/8/16 without a booster car seat by social worker Liz Cardenas. The mother also stated the social worker grabbed David's arm and hand to prevent him from swinging a rake. Attached is an email from the social worker explaining the reason for not providing the booster car seat. The social worker stated due to the other two car seat there was not enough room for the booster seat therefore the child was secured with a seat

CCIB received a telephone call with an allegation against the facility. RP called to report that there is drug use and domestic violence happening in home due to the licensee runs a daycare. RP reported that on 10/9/2017 Veronica and Juan went to pick up children from school and Veronica asked staff at the school to call the police. RP reported that staff told her to contact the police to report what is happening. The RP reported that the police came to the school and arrested Juan in the parking lot. RP stated that it was stated on the police report that Veronica was hit in the face and kicked (unknown when incident happened). RP reported that the police report stated Veronica had a cut lip and red marks on her legs. RP also mentioned on 10/10/2017

The reporting party (RP) stated resident Arev Dabbagh DOB: 12/27/00 disclosed on 4/15/16 she inappropriately touched by a male peer. According to the Arev on 4/14/16 a peer Ibraheem Mamoori DOB: 9/2/98) placed his hand on her shoulder and continued downwards towards her breast. The peer touched her breast over he clothing. The resident was distraught while she recounted the incident. Arev alluded to their being other incidents with the peer and other unknown males. Arev didn't disclose details or clarification. Arev was placed voluntarily in the facility by her mother and will continue her placement for a few more weeks. The peer is currently placed on a 5150 hold and will not have access to Arev in the future.
