The reporting party (RP) stated on 11/10/16 the biological mother of David Jiron DOB: 3/6/11 reported he was transported to his visit with her on 11/8/16 without a booster car seat by social worker Liz Cardenas. The mother also stated the social worker grabbed David's arm and hand to prevent him from swinging a rake. Attached is an email from the social worker explaining the reason for not providing the booster car seat. The social worker stated due to the other two car seat there was not enough room for the booster seat therefore the child was secured with a seat

The RP stated the resident was issued a government cell phone, however the caregivers take it away from her and refuses to let her charge it. On 6/13/16 the police were called to the facility due to resident's inability to enter the facility. The North Hollywood Police Officers Winter (#38878) and Officer Radke (#39569) arrived to the home. The Officers removed the resident due to her being battered and concern for her safety.
