The operator of the vehicle identified himself as Roger Robert Romansky with a California identification card. I questioned Romansky who the owner of the vehicle is. He said it's a friend of his. When asked for his friend's name he said he wasn't sure. He asked the other occupant in the vehicle for his friend's name. The passenger said Brittney. On Sunday, 04/29/2018, at approximately 1024 hours, I was in full police uniform, driving a marked Daly City Police patrol vehicle. I was driving south on Sullivan Avenue, north of the interstate 280 south on ramp. At that time my attention was drawn to a gray, 2015, Volkswagen, Passat bearing California license #5TUF434. I saw the vehicle was occupied by a male driver and female passenger. I conducted a records check on the computer in my patrol vehicle and it revealed it was a stolen vehicle out of Oakland.

Mr. Matthews said the male then got out of the truck and then began to walk down his driveway and fell and face first on the driveway. The male then got up and stumbled all the way down the street and disappeared in the darkness. While Officer Seachris was speaking with Mr. Matthews, neighbors advised there was moaning coming from down the street. Officer Seachris went to 628 S Vassar and located a light-skinned, bald, Hispanic male with no shirt lying face down on the cement porch. This subject was later identified as Javier Ramirez-Zarate. Officer Seachris could immediately smell a very strong odor of an alcoholic beverage on Mr. Ramirez-Zarate. Officer Seachris attempted to get information about the accident but Mr. Ramirez-Zarate kept repeating that he was at his house. Officer Seachris had Mr. Ramirez-Zarate walk down the street and observe that Mr. Ramirez-Zarate was having difficulty maintaining his balance. Officer Seachris instructed Mr. Ramirez-Zarate that he was going to administer sobriety tests and attempted to do so. Mr. Ramirez-Zarate could not perform the test and Officer Seachris took him into custody. During this time Officer Wheeler made contact with Mr. Ramirez-Zarate’s girlfriend who advised that Mr. Ramirez-Zarate had been driving the truck earlier and as far as she knew he

I made contact with the driver (Stanciel, Conswella N. DOB 06/19/86), who related that she did not possess a driver's license. Conswella was checked through LEADS via dispatch, which revealed a no valid driver's license and she was clear with no wants or warrants. She was unable to provide insurance for the vehicle. There was a passenger in the vehicle, who was identified as (Edwards, Shekida D. DOB 07/24/86), she was checked through LEADS, which revealed that she was valid with no wants or warrants. The vehicle was released to Shekida.

On 07/15/2015, at approximately 2305 hours, your Affiant was on routine patrol on Main Street (SR924) when I observed a silver Chrysler sedan parked in the One Hundred Block of South Main Street (westside of the street) near the intersection of Oak Street. The vehicle displayed a Pennsylvania registration of JRJ-5342. The registration was checked utilizing the Pennsylvania Department of Transportation vehicle record abstract which revealed that the license plate was assigned by the department to KYLE ROBERT & SARAH LYNN FOGARTY for a 1998 Chevrolet. I then verified the status of KYLE and SARAH FOGARTY'S driving licenses both of which were suspended by the Department of Transportation.

Officer Saunders, Officer Torres and I contacted Suspect Jose Guadalupe Arevalo at 13 W. Hammond Street. Arevalo told me the following information in summary: On 03-14-16 at approximately 0930 hours, Arevalo was reportedly approached by Parra in the rear parking lot behind his residence. Parra told Arevalo that he did not properly screw in her vehicle’s inner driver’s door panel.

Larry was transported by AMR to Chino Valley Community Hospital. At Chino Valley Hospital, Larry claimed that he was Tased and was complaining of pain to his stomach and back. Larry was not Tased by OPD officers. I looked at Larry’s stomach and back and did not see any redness nor any visible injuries. I read Larry his Miranda rights from my department issued Miranda card. He said “yes” to all four questions indicating that he understood them. Under Miranda, Larry told me that today in the morning, his girlfriend, Stephanie Perez, dropped him off at work and they got into an argument. Larry said he and Perez got into an argument. The argument was again about Perez wanting to leave him. Larry said he took the vehicle key from the vehicle and went inside the store, preventing Perez from driving away. I asked him if he saw the police unit. Larry said yes he did however he was scared and did not want to be detained or arrested because he did not do anything wrong. On Thursday, 05/18/2017, at about 9:27 p.m., partner Officer Griffin (#3135) and I were on uniformed patrol in a marked patrol vehicle (Unit #21) as 3A21. We were dispatched to an injury traffic collision at Lake Ave. and Maple St. Upon our arrival, we saw three vehicles that appeared to be involved in a traffic collision. While we were assisting with the scene, we saw Victim Captain Konrad struggling with the suspect, later identified as Kevin Hendrix. Officers Griffin, Riehl (#7454), Spatz (#8058) and I grabbed Suspect Hendrix and took him to the ground. I placed properly adjusted, double locked handcuffs on Hendrix. Officer Griffin and I placed Hendrix in our patrol vehicle.

On 26 June 2017 at 1516 hours, Officers were dispatched to the area of 305 East Penn Street for a theft of a vehicle. Officer were advised the actor was a Rhiannon Meyer and that she had stolen her dad Dodge truck green in color with a plow on the front. and was last seen heading towards Main Street. Officer began to check the area of Main Street, when Unit 191 called out with the vehicle at 100 Pennsylvania Ave, in Shenandoah Heights. Your Affiant (Patrolman William Moyer) along with Patrolman Tyler Dissinger responded to Unit 191 (Chief Livergood) location. At arrival Chief Livergood had MEYER in custody along with the stolen vehicle. MEYER'S was still yelling about someone having a gun in the window of the residence and was not making sense

On Wednesday 11/02/16 at 2125 hours I was dispatched for a theft of a motor vehicle from 1502 A ST SE in the City of Auburn, King Co, WA. Dispatch advised the reporting person, Melvin Harrison, stated sometime within the last hour his red 2006 Buic LaCrosse was stolen from the above address.

I exited my patrol vehicle and made contact with the driver of the vehicle Erica Manning 24 year old female, and the front seat passenger Brent Manning 30 year old male. I identified myself and asked Erica for her drivers license and proof of motor vehicle insurance. Erica removed her license from her billfold and handed it to me. I asked again for her motor vehicle insurance. Erica began looking around inside the cab of the vehicle. Erica began looking in the center console of the vehicle but was unable to locate the insurance information. Erica said she wasn't sure where the insurance is. Erica asked if I would accept the insurance in a digital form on her phone, but had difficulty finding words she wanted to use. Erica began using her phone trying to find her insurance information. Brent began looking into the glove box of the vehicle. While I was speaking with Erica and Brent I noticed they were both talking slowly with mumbled and slurred speech. At this time Officer Pearson placed Leigh under arrest. After Officer Pearson explained to Leigh why she was under arrest I helped Leigh into the rear passenger side of squad #22. As we were getting Leigh into the squad the male passenger, later identified as Matthew Paul Ginther DOB: 02-10-1976, got out of the vehicle and approached Officer Pearson and I. Officer Pearson instructed Matthew to stay where he was several times, but he refused asked what was going on.

Upon our arrival, we found a 2000 white Pontiac Grand Prix, bearing FL tag Z72 AKB, parked in the driveway. The vehicle was occupied by Taylor Lewis Jones, who was seated in the driver seat. The white male agreed to speak with Officer Holroyd. He identified himself as Taylor Lewis Jones with a Florida identification card.
