INTRODUCTION: This incident involved the attempted battery by Suspect Kevin Hendrix on Pasadena Fire Dept. Captain Josef Konrad. INJURIES: Suspect Kevin Hendrix sustained the following injuries: -abrasions to the face and right knee Suspect Kevin Hendrix also had abrasions on top of his head from hitting his head on the metal lattice work in the patrol vehicle. INVESTIGATION: On Thursday, 05/18/2017, at about 9:27 p.m., partner Officer Griffin (#3135) and I were on uniformed patrol in a marked patrol vehicle (Unit #21) as 3A21. We were dispatched to an injury traffic collision at Lake Ave. and Maple St. Upon our arrival, we saw three vehicles that appeared to be involved in a traffic collision. While we were assisting with the scene, we saw Victim Captain Konrad struggling with the suspect, later identified as Kevin Hendrix. Konrad stated he saw Hendrix yelling at a victim of the collision. Konrad said he told Hendrix to step away and let the paramedics attend to the victim. Konrad said Hendrix walked away but a short time later Konrad found Hendrix yelling at the same victim. Konrad told Hendrix he needed to leave. On Thursday 12/24/2015 at approximately 2307 hours. Security Officer Omar Alonso (420) was contacted by E.D. Charge Nurse Sharey Selover about an uncooperative intoxicated male patient, Jose D. Gonzalez (DOB: 03/30/1977; FIN# 85006354), come in through the EMS Offload area. Officers Alonso and Ayuso reported to the call and observed an intoxicated male being wheeled into the Special Care Unit (SCU) E.D. room # 39. According to his assigned Nurse Sara Lopez, the patient had been involved in a physical altercation and had been kicked hard in the groin area. On March 17, 2016, at 1:41 P.M., officer T. Cramblett CPD#1191 was dispatched to he area of Sharon Woods Blvd. and Brookhurst Ave on a narcotics complaint. Radio was advised by a known caller that a white Chevrolet Malibu, occupied by two white females, was parked on the street and were involved in the sales of narcotics. On Tuesday, June 6, 2017, at approximately 2134 hours, Trooper Jason Brown, Unit 522, conducted a traffic stop at the junction of US 460 and Camargo Leevee Road in Montgomery County. Jimmy White was pulled over for driving with no headlights. When Trooper Brown made contact with Mr. White, dispatch advised Mr. White had active arrest warrants. When Trooper Brown advised Mr. White that he had active warrants and was under arrest, Mr. White began to flee on foot. Trooper Brown chased him approximately 300 feet giving him loud verbal commands to stop running. Mr. White tripped over some debris in the yard he was running through and fell to the ground. When he got back up to continue running, Trooper Brown attempted to place him in hand cuffs.
Officer Jimenez believed that Anderson locked herself in the room because she was afraid of Hendricks. Anderson stated that she got into a physical altercation with Hendricks. Officers asked Anderson the details of the incident. Anderson told Officer Jimenez that Hendricks has an addiction to alcohol. Anderson stated Hendricks was drinking earlier that night. Anderson stated that Hendricks wanted to have sex with her but she refused. Anderson then stated that it was then when she tried to lock him out of the room but he got inside before she was able to lock him out. Anderson stated that after Hendricks got inside the room he started throwing miscellaneous items inside the room and acting belligerent. Anderson advised Officers that it was then when Hendricks delivered a closed right fist towards the left side of her face. Anderson then said that she fell backwards on the bed and Hendricks got on top of her. Anderson advised Officer Jimenez that her daughter Presley Martin (W/F-DOB09/13/2004) got in between them. Anderson stated that she then bit and scratched Hendricks in order to protect herself from his acts of physical family violence. It was then when Hendricks left the room in order to call the Police. Figueroa arrived Cape Fear Valley Medical Center 1638 Owen Drive Fayetteville, North Carolina 28304 in-reference to an additional victim suffering from a gunshot wound. The individual was identified as Kenneth Alexander Shaw (black male, 7/5/1988) who arrived in a gray Ford Fusion North Carolina Registration PAB5727. Detective J. Figueroa was contacted by Officer Catlett who advised an Amos rashaad Shaw (black male, 10/10/1989) was in the parking lot and had brought his brother Kenneth to the Hospital.

On Monday, 11/30/2015 at approximately 1755 hours, Officer Lauritzen and I responded to intersection of Holt Blvd and Plum Avenue to assist Sergeant Ortiz to subdue a suspect who was fighting him. Upon arrival, I saw other OPD officers trying to handcuff the suspect, later identified as Kevin Larry. Larry was on his stomach and he was yelling that he did not do anything. Larry was resisting arrest by turning his hip side to side and was trying to pull his hands to the front of his body. Police officer, Michael McKay, 36, of Westville Police Department, was reportedly scene giving a minor, from a youth group he mentored, a ride home on multiple occasions, in his police vehicle. Activity was reported suspicious by a witness, who witnessed Officer McKay, hugging and kissing the minor before exiting the vehicle, at the minor’s residence, 132 S. 4th Street. Once activity was reported, an unmarked car with two officers, and two patrol officers on foot, were assigned locations within visibility of the minor’s home. Officers A & B (in unmarked vehicle) were stationed at the corner of 3rd and 4th street, with an active dash cam. Officers C & D (patrolling on foot) patrolled 4th street keeping Officer McKay within sight. While on patrol, I was heading northbound on North Three Notch Street. While traveling, I observed a burgundy Chevy Tahoe traveling southbound in the northbound side of traffic, approaching my patrol vehicle. To keep from being involed in a head on collision, I pulled my patrol vehicle into the Sunoco on North Three Notch Street. I then engaged my lights and siren and attempt to catch up to the violator vehicle. The Tahoe then pulled into the Piggly Wiggly parking lot and that was the final. I then identified the driver as William Anthony Thomas. I advised Thomas that he alost hit me head on and he stated, "I'm sorry, I am too fucked up." Thomas had an strong odor of an alcoholic beverage coming from his expelled breath. Thomas also had red glossy eyes. I then advised Thomas multiple times to step out of the vehicle to perform FST's. I noticed that Thomas had unsteady balance as he was waling to the back of the vehicle.
On 9/12/2016 at approximately 0740 hours Deputy Chavez B1871 and I responded to a Assault DV in 9615 South 7th avenue Phoenix, Arizona 85003. On 07/25/2015, at approximately 0157 hours, Captain O'neill and I were on routine patrol. We were driving south on Gilbert street and at the intersection of West Mt. Vernon Street we observed a blue GMC pick-up (PA registration ZGT-5355) travel through a clearly and properly posted stop sign forcing me to slow my vehicle. I observed and identified the operator as Rafael CHAGOLLA-ESCOVEDO, who I am familiar with through prior contacts. I was aware that Rafael CHAGOLLA-ESCOVEDO does not possess a valid drivers license. I activated my emergency lights. The vehicle continued south on Gilbert, turned right (west) on Centre street and parked.
Off. Martin arrived before I did. when I arrived, Off. Martin was placing Schweitzer detaining Schweitzer in handcuffs.
