On 3/13/16 SO EMT Perez was notified that an engine had come on campus. SO EMT Cunningham responded along with SO EMT Perez and caught up with the engine by lakeside commons. The driver of the engine stated that he was responding for a lift assist to 3144/3146 apt 220 but that he wasn’t sure which one he was going too. SO EMT Cunningham and SO EMT Perez decided to respond to FR-220 first being that they were the engine came from gate 2 and they were closest to that building. SO EMT Perez and SO EMT Cunningham knocked and announce their presence at FR-220’s door and were both verbally invited in. The resident a Mrs. Helen Neff stated that she could not get out of her chair because it was stuck and needed help getting out of it. SO EMT Perez
On 3/6/2016 SO EMT Perez was dispatched to GV-204. SO EMT Perez knocked and announced his presence at the door and was verbally invited in by the resident. The resident, a Mrs. Gloria Krupin was sitting on the floor in the bedroom and stated that she was trying to use the bathroom when she slipped and fell and was unable to get herself back up on her own strength and crawled herself to the bedroom. Mrs. Krupin had a slight skin tear on her left elbow and her right knee which SO EMT Perez promptly bandaged. SO EMT Perez performed an assessment which revealed the following; Hip stabilization noted upon palpation presenting no pain, Blood Pressure 130/80, Pulse 90bpm and Spo2 94%. On 10/4/2015 S/O EMT Perez was dispatched to MG-T15. S/O EMT Perez knocked and announced his presence at the door and was greeted by the resident, a Mrs. Maxine Montgomery. Mrs. Maxine Montgomery stated that she needed some help re ajusting her sling and was unable to do it herself. S/O EMT Perez adjusted her sling and asked if Mrs. Maxine Montgomery needed any asistance with anything else and or needed any aditional care. Mrs Maxine Montgomery stated that she was fine and needed no adititional care and escorted S/O EMT Perez out. S/O EMT Perez cleared the scene without further incident.
This evening resident in unit 1115, Keenan Turner was doing a move out. Quin noticed a truck trying to enter but was well over the size to fit in driveway. Quin instructed resident that the truck was too big and would not be allowed to block the entrance gate. Residence stated that it would only be there to load two things and in 30 minutes it would be moved. He stated that guest can use exit gate or either just go around. Quin informed him that wasn’t his choice to make but Turner replied; fine him. When board members Morse and Oliver stepped in to try to assist with the removal of truck. The driver and resident began to argue and cause the problem to escalate more. Quin tried to talk to both drivers and residents in moving it but no luck.
After talking with Mr. Cunningham worker walked outside and meet with Mr. Danny Cunningham, son of Mr. and Mrs. Cunningham. Mr. Danny Cunningham agreed that his mother needed to be in the nursing home because no one was able to care for her at home. According to Mr. Danny Cunningham, he only wanted his mother to return to home if she was able to walk. However, since her admission to Shadescrest she has declined physically and now requires assistance with everything. Mr. Cunningham stated that his father and sister was unable to provide that level of care and he was not able to move into the home with them and provide care because he worker in Birmingham and isn’t around a lot due to work. On Friday 09/16/2016 at 2300 hours, Security Officer Michelle Gallegos was badging visitors in the E.D. Lobby when the sister of a patient in E.D. # 32 lunged over the counter, knocking down a computer screen and a cup of water, then striking Officer Gallegos in her left arm. Security Officer (SO) Omar Alonso, was inside the E.D. Security Office, and responded immediately verbally instructing the visitor that police were going to be notified and that she needed to leave property immediately, and he also called Dispatch for immediate assistance. The female visitor was with 5 other females and her mother so they all followed Officer Alonso, outside of the E.D. Lobby. They were screaming and hollering that they were being treated disrespectfully by the badging officer who would not let them go back and visit her sister. Security Officers Ariel Weiland, Demetrio Flores, and Supervisor Steven Evans all responded by 2303 hours and helped to keep all 6 visitors outside while SO Alonso, conducted an investigation into the visitor’s allegations that they were being unfairly treated and were given erroneous information. He spoke to the nurse that Officer Gallegos spoke to when she needed information on the patient and permission for visitors to be given a pass to visit. Upon arrival we noticed that there is a light on at 424 and ½ S F St. I made contact with a male and asked if someone had called 911. Male stated that his girlfriend called but hung up the phone. I asked if I could go inside and speak with the female while the male spoke with Officer Sullivan. He agreed.
I was waved down by a female at the entrance to the Edge Apt. Alize Rivera advised that she had been involved in a motor vehicle accident on Elm St. she stated that the other driver involved had her boyfriend the stated driver of Unit 2, move from the scene of the crash to the Edge Apt, and then he drove off. Rivera advised that the driver of Unit 2 ,was stopped waiting to turn into the Troy Softball parking lot when a black Chevy Silverado struck her vehicle from behind. She stated that the Driver of unit 1 was a white male, around his mid 20, and had a blue hat on. Officer were unable to locate unit 1. On Saturday 10/26/2016 at approximately 2328 hours, Security Officers Christopher Paz, Ariel Weiland, Omar Alonso along with Supervisor Steven Evans were dispatched to the EMS Off load Ramp for an incoming (51S) Patient Standby In E.D. Upon arrival at 2328 hours Security met with E.D. Charge Nurse Johnathan Bacal who stated that there was a combative male patient being transported by the Orange County Fire Department. At 2330 hours, the patient, Alan Castillo (DOB: 08/03/84; Fin #86501337) was brought by Orange County EMS (Engine #83) with an escort from Orange County Sheriff's Deputy. I responded to 4198 AL HWY 87 in reference to a suspicious circumstance call. Upon arrival, I made contact with Gilbretta Thompson walking in the grass heading towards Troy on US 87. Dispatch advised that the subject had mental issues and possibly under the influence of drugs. I then attempted to get out the vehicle and make contact with Gilbretta, but she kept walking. I then notified Sgt. Wingard and he came out to the scene. Upon Sgt. Wingard arriving, Gilbretta was walking in the roadway. Myself, Sgt Wingard and Off. Irsik then pushed the subject on the ground to prevent Gilbretta from getting hurt by on coming vehicles. Gilbretta was then escorted back to her residence. At the residence, Gilbretta did advise that she was going to kill herself. East Central Mental Health was notified and Haynes ambulance came out to check Gilbretta out.
According to the incident report, R/A Moss observed though the camera that residents were arguing in the elevator. R/A Moss also observe that once the resident of unit #329 came out of the elevator started to kick the elevator door. After kicking the elevator door resident proceed to the Social Services office screaming and yelling that she was going to beat up resident Tessier in unit #204. On Monday May 30, 2016 at approximately 0031 FHEO Security was dispatched to ED room #27 for a (51S) Patient Standby in ED. Security Supervisor Steven Evans (407), Brandon Rodriguez (418), and Christopher Paz (408) responded. Upon arrival to ED room 27 Charge Nurse Sherry Selover informed security that patient, Jessica Daniel (FIN: #85701913) was attempting to stand and it was not medically safe to do so. The Charge Nurse Selover explained to the patient for her safety she needed to remain on her bed until the medical staff deemed it safe for her to stand and walk. The patient agreed to corporate without further incident. No hands on and no injuries incurred during this service call. Nothing further to report.
On 08/20/2017 at 1002 hours FTO Wheeler #4361 and I responded to St. Luke’s Hospital emergency room entrance, on 3555 Cesar Chavez St., regarding a women in a hospital gown trying to break the caller’s vehicle window with a brick. Officer Wheeler and I were in full police uniform driving a marked patrol unit when we responded.
S/L Walker was responding for a pendant alarm for CL -T21, but was told about a sick resident in teh dinner room. The call was pass to mobile as S/L Walker tend to the sick resident. S/L Walker was lead to Mr. Matisoff of PV-524 in Seasons dinning room. Mr. Matisoff was alert and oriented x4. Mr. Matisoff said, "I don't feel good. I am cold, weak, and sweating." S/L Walker asked if Mr. Matisoff was feeling any pain. Mr. Matisoff denied any pain. S/L Walker check Mr. Matisoff vitals sign. Blood pressure was 180/100 with a Pulse of 181, and O2 of 98% on room air. S/L Walker called for 501 due to the high pulse rate. When I arrived to Bldg C I went straight to the elevators and use the intercom to communicate with Mr. Gutierrez Gutierrez (51/M). I asked Mr. Gutierrez was he ok and help was on the way. I pushed the button to open the elevator and Mr. Gutierrez was laying on the floor of the elevator. Mr. Gutierrez stated he was looking for the hospital. Mr. Gutierrez stated his back was hurting.
