After talking with Mr. Cunningham worker walked outside and meet with Mr. Danny Cunningham, son of Mr. and Mrs. Cunningham. Mr. Danny Cunningham agreed that his mother needed to be in the nursing home because no one was able to care for her at home. According to Mr. Danny Cunningham, he only wanted his mother to return to home if she was able to walk. However, since her admission to Shadescrest she has declined physically and now requires assistance with everything. Danny Cunningham agreed to meet with the nursing home on Friday and provide them with as much of the documentation he could gather. Mr. Cunningham’s only concern about leaving Mrs. Cunningham in the nursing home was losing the home. Worker explained to Mr. Danny Cunningham the home would not be placed up for sale as long as his father was alive and living in the home. Worker also instructed him to speak with Ms. Morrow about the home selling after his father pass away because of his sister living in the home. Because she has lived in the home for 30 plus years, she may be able to remain in the home after their parents passing. R/s Alene Collins suffers from arthritis, depression, hypertension, a history of fracture arm, a history of right hip fracture, and she is hard of hearing. R/s Ms. Collins lives alone and needs assistance with some of her ADLs, she has 10 hours of care from CLTC. R/s Ms. Collins has a history of falling so she has a lifeline. R/s Ms. Collins is having difficulty breathing due to mold and mildew in the home. R/s the roof has a leak and according to Shala Martin (daughter), they don’t have insurance to fix it.
On December 30th, 2015 at 0357 hours Deputy Akins responded to a battery of a family member. When the Deputy arrived on the scene he was approached by Kimberly Corp and Tayia Corp who advised that John Paul Branfield Corp had punched them. Deputy Akins asked the victims to explain in detail what had happened.
On this date worker visited Walker Rehabilitation in Carbon Hill Al., for the purpose of monitoring Ms. Phillips placement. When worker arrived Ms. Phillips was in bed sleeping but woke when her name was called out. Ms. Phillips was dressed appropriately with good personal hygiene. Ms. Phillips did not remember worker from previous visits but was friendly with worker. During visit, Ms. Phillips talked about her roommates and appears to have adjusted well to her new living arrangement. This was a concern for Ms. Phillips because she went from a room for two into a room of four. Ms. Phillips stated she enjoys the food but doesn't participate in activities. On October, 24, 2004, Bradley Harrison was driving a rented a vehicle from Vancouver to Toronto. However, the rented vehicle did not have a front license plate, which is an offence if the vehicle is registered in Ontario. An Ontario Provincial Police officer, constable Bertoncello, pulled over the rented vehicle. Bertoncello realized that the vehicle was from Alberta and decided to do a background check on the vehicle if it was rented in Alberta. Bertoncello learned that the vehicle was properly registered and no offence was committed.
I represent the children of Maudie Miller Oliphant, Dianne Oliphant, Jan Oliphant and Lloyd Oliphant who are concerned about Maudie Miller Oliphant’s health, safety, and her ability to live alone in her house. They feel that Ms. Oliphant needs around the clock care, and she is reluctant to agree to that. Moreover, she is confused and does things which puts her safety in jeopardy such as, carrying open flames around her house to light a candle.
D/A: Maurice Brown had one appointment this week. On 12/14, he was seen by Doctor Nat Ramani of the GI Associates of Delaware. According to Doctor’s note, consumer tolerated procedure well. He did not have fever, rash, or chills post procedure, but still complains of having periumbilical abdominal pain frequently for 2-3months. He describe pain as sharp in nature, rates it 7 out of 10, wakes him up from sleep at night, no exacerbating factors, radiates to both sides of his abdomen, alleviating on passing gas and having a BM. After a couple of day at home he took Ms. Brooks to Senior Care in Winfield. There Ms. Brooks was discharged to Eagle Rest Group Home. R/s Rachel Morton recently had a pace maker placed in her and will need physical therapy at home. R/s Ms. Morton’s children won’t allow anyone in the home. R/s the home is not in livable conditions. R/s Mr. Morton is a hoarder and she can’t use the walker due to the clutter. R/s Ms. Morton’s sons, Terry and William call the police wherever family comes to visit Ms. Morton. R/s Terry has a mind of a 7-year-old and William has a history of alcohol and drugs abuse. R/s William suffered a brain injury and has brain damage.
On this date worker visited the residence of Mr. William Teske, for the purpose of monitoring his situation. When worker arrived, Mr. Teske was heating up a microwave dinner. He stated he was his favorite, spaghetti and meatball. Mr. Teske remembered worker from previous visits. Mr. Teske was appropriately dressed with acceptable hygiene. There was mail lying around the apartment and there the odors of cigarette smoke. During visit Mr. Teske keep O2 on and did not smoke. Mr. Teske thanked worker for helping him get Medicare part A. He stated he still receives Elder Care and home health but HH switched from Amedysis HH to Alacare HH and Alacare is in the process of changing him from HH to Hospice. The resident faces the medical problem of being a dementia patient. The result of this has caused anger, physical assault and incontinence after restraining the resident. The social issues that are presented is Mrs Alston lives in an aged care facility that is not only sectioned for residents with high care needs but also other residents with the same degree of illness. Another problem is that she has been taking other people’s belongings.

Both of the gentlemen stated they had been in other homes and that this was the nicest home they had been in. Kenneth complained of his first home ¡°smelling more like a zoo than a nursing home.¡± John told me that his last home was on the South Side and that the neighborhood was filled with gang bangers and hoodlums. When his family visits here though, they wheel him through the Lincoln Park neighborhoods and it¡¯s nice. For all of the questions that I asked these two men, they had just as many questions for me. Heading in to this assignment, I hadn¡¯t really considered that someone would be asking ME questions. In the midst of all that happens in nursing homes, it¡¯s important to remember that these residents aren¡¯t just dependent, older people, but that they are still people just like you and I.
