On 08/20/2017 at 1002 hours FTO Wheeler #4361 and I responded to St. Luke’s Hospital emergency room entrance, on 3555 Cesar Chavez St., regarding a women in a hospital gown trying to break the caller’s vehicle window with a brick. Officer Wheeler and I were in full police uniform driving a marked patrol unit when we responded.
3D15A Officer’s Perez #4305 and Sutton #2446 and 3D11A Officer’s Perez #4305 and Holdbrook #708 responded to provide assistance.
Upon arrival, Officer Wheeler activated his (BWC1) body worn camera which was later uploaded to evidence.com. As our patrol vehicle pulled up to St. Luke’s emergency room entrance, I viewed a male later identified as (RV1) Devinelle Lavaughn Broussard with a cellphone in his hand attempting to prevent a female, later identified as (B1) Eleasia Ashanti Fraise, from throwing a (EVD1) red brick. I next saw Fraise throw the brick at a parked (D1) silver BMW, located along the south side walk. The brick fell to the ground and Fraise went to picked it back up. Fraise picked the brick up and moved in the direction of our patrol vehicle with the brick at shoulder height. I parked the patrol vehicle and immediately got out drawing my firearm to a low ready. Officer Broussard stepped out of the vehicle and drew firearm pointing it at Fraise (View Officer Wheeler’s statement for further details). On March 17, 2016, at 1:41 P.M., officer T. Cramblett CPD#1191 was dispatched to he area of Sharon Woods Blvd. and Brookhurst Ave on a narcotics complaint. Radio was advised by a known caller that a white Chevrolet Malibu, occupied by two white females, was parked on the street and were involved in the sales of narcotics. Officer Cramblett was responding to the area when he observed a white Chevrolet Malibu traveling south bound on Beechcroft Rd. Officer G.Meyer CPD# 2333 had also been dispatched on the run, and was traveling north bound on Beechcroft Rd., when he observed the vehicle ,traveling south bound, and aired with radio that it was occupied by two white females. On the afternoon of November 14, 2015 at approximately 5:07 P.M. Deputy Singleton responded to a domestic disturbance call at 4547 Old Salkahatchie Hwy in Early Branch SC. When I Deputy Singleton arrived on scene I interviewed Sheree Washington who stated that after Deputy Ayer had left the scene from the previous call that her boyfriend Christopher Williams who resides at the residence said "bitch I should kill" you then proceeded outside, and retreived a light with a piece of pipe attached to it, and then struck her in the chest. At this time Washington called dispatch, and asked for a Deputy to come back out to her residence. On Thursday, 05/18/2017, at about 9:27 p.m., partner Officer Griffin (#3135) and I were on uniformed patrol in a marked patrol vehicle (Unit #21) as 3A21. We were dispatched to an injury traffic collision at Lake Ave. and Maple St. Upon our arrival, we saw three vehicles that appeared to be involved in a traffic collision. While we were assisting with the scene, we saw Victim Captain Konrad struggling with the suspect, later identified as Kevin Hendrix. Officers Griffin, Riehl (#7454), Spatz (#8058) and I grabbed Suspect Hendrix and took him to the ground. I placed properly adjusted, double locked handcuffs on Hendrix. Officer Griffin and I placed Hendrix in our patrol vehicle.
On 12-05-17 at approximately 1017 hours, I responded to 442 N. Los Robles Avenue regarding a psychological evaluation. PPD Dispatch advised that a female, White was yelling incoherently at the above location. Officer Gawlik #3116, Officer Herrera #3595 and Officer Killgore #7231 were on scene prior to my arrival.
On 26 June 2017 at 1516 hours, Officers were dispatched to the area of 305 East Penn Street for a theft of a vehicle. Officer were advised the actor was a Rhiannon Meyer and that she had stolen her dad Dodge truck green in color with a plow on the front. and was last seen heading towards Main Street. Officer began to check the area of Main Street, when Unit 191 called out with the vehicle at 100 Pennsylvania Ave, in Shenandoah Heights. Your Affiant (Patrolman William Moyer) along with Patrolman Tyler Dissinger responded to Unit 191 (Chief Livergood) location. At arrival Chief Livergood had MEYER in custody along with the stolen vehicle. MEYER'S was still yelling about someone having a gun in the window of the residence and was not making sense
On 08/07/15 at approximately 1820 hours, my partner (Ofcr. Vanlenzuela No.38161) and I (Ofcr. Nunez No.36181) were working North Hollywood patrol, assigned to unit 15A84 W5. At that time, we responded 10849 Fruitland Dr for an “Attempt 211 Suspect.” Comments of the call indicated the P/R was the wife of the victim Melvin Salinas. Suspect was described as a male black “transient” armed with a knife tried to take a vehicle from the parking garage Inc# 150807003722.
I, Cpl. Lessane, contacted Sgt. Smith, and advised him of the current situation. Sgt. Smith along with Caption Russell and Deputy Green, all shortly responded to the Emergency Room.
On Sunday November 24, 2016 at approximately 16:02 hrs. Officer L Wilkerson and I Ofc. R. Holcomb were dispatched by dispatcher B Coleman to 304 Hopkins Drive in reference to a disturbance. Upon my arrival I made contact with Ms. Shawntavia Lucky (complainant) who stated that her son Mr. Jousha Lucky (victim) got into a physical altercation with two black males.
I was dispatched to 113 W. Orange Street in reference to a domestic call. Upon arrival, I made contact with Brenda Colvin. Colvin advised that he husband picked up a chair and threw it at her feet. Officers did notice swelling to Colvin left ankle and a flipped over wooden chair located on the front porch. Colvin advised that her husband(James Wigfall) had been drinking and that’s what was making him angry about everything they were talking about. Colvin advised that she confronted him to talk about him drinking so much and things that needed to be done around the house. Wigfall then got an attitude. Colvin advised that she told Wigfall that he needs to leave the house for the night and he refused so she called the police.
On 9/12/2016 at approximately 0740 hours Deputy Chavez B1871 and I responded to a Assault DV in 9615 South 7th avenue Phoenix, Arizona 85003. When we arrived at the residence we heard Amy Frieberg scream, we entered the house and Ron Frieberg and his wife where having a verbal argument and where in each other faces. We separated them both I took Amy outside to talk to her and my partner stayed inside to talk to Ron. I noticed that Amy had a swollen right eye and a red mark above her right cheek. I asked her if she wanted to be checked by medical she said “Yes”. I called medical and asked her what has happen. She said “ I don’t remember it happened really fast, I don’t want him to stay here”. I responded to 4198 AL HWY 87 in reference to a suspicious circumstance call. Upon arrival, I made contact with Gilbretta Thompson walking in the grass heading towards Troy on US 87. Dispatch advised that the subject had mental issues and possibly under the influence of drugs. I then attempted to get out the vehicle and make contact with Gilbretta, but she kept walking. I then notified Sgt. Wingard and he came out to the scene. Upon Sgt. Wingard arriving, Gilbretta was walking in the roadway. Myself, Sgt Wingard and Off. Irsik then pushed the subject on the ground to prevent Gilbretta from getting hurt by on coming vehicles. Gilbretta was then escorted back to her residence. At the residence, Gilbretta did advise that she was going to kill herself. East Central Mental Health was notified and Haynes ambulance came out to check Gilbretta out.
