I, Cpl. Lessane, along with Deputy Ayer, responded to Hampton Regional Medical Center, to assist Deputy Smalls, with a runaway mental patient. Upon arrival, Deputy Smalls stated a mental patient, Travis Johnson, ran away from the Emergency Room with a set of black handcuffs on one of his arms. Deputy Smalls stated he wasn’t sure of Travis exact location at the time. Deputies along with assistance from the Varnville Police Department and the Hampton Police Department patrolled the area in an attempt to make contact with the patient. No contact was made at that time.
I, Cpl. Lessane, contacted Sgt. Smith, and advised him of the current situation. Sgt. Smith along with Caption Russell and Deputy Green, all shortly responded to the Emergency Room.
I notified dispatch that Officer Albert and I were out at the RSC. I parked our patrol car on the north side of the RSC and entered the north door. I heard Officer Smith on the radio say he was out with the subject near the Credit Union. I observed from across the RSC Officer J. Smith and Officer R. Faison standing next to a black male all the way by the Book Store. As I walked closer, I could see the black male with both of his hands up in the air and he was getting down on both of his knees. When I got to the Credit Union, I saw a set of crutches and a black bag laying on the floor. I told Officer Albert to stay here with these items as I continued to go behind the black male. On December 11, 2016 at approximately 1330 hours, Detention Officer Harden reported duty and was assigned to work 1200 Baker Street Jail, 3rd Floor, A-Pod Control Center. At approximately 1420 hours, Detention Officer Harden advised she was not feeling well. At approximately 1430 Hours, Detention Officer Harden had her blood pressure checked in the 1200 Jail Clinic and was advised it was elevated. At approximately 1440 hours I called for Houston Fire Department. At approximately 1445 hours Houston Fire Department Medic 17 arrived at 1200 Baker Street Jail. Detention Officer Harden declined transport at approximately 1500 hours. At approximately 1505 hours, Sergeant Chad King took Detention Officer Harden to her husband’s work location Lieutenant
Mr. Jones stated, he had never seen the individual before and was unable to provide an accurate physical description. At 0650 hours Orange County Deputy Stephen Williams (Badge #25569) arrived on site. D/S Williams conducted an initial investigation with the Stab Wounds Victim and after questioning, he determined there was no immediate threat to Florida Hospital staff. He then contacted Security Supervisor Steven Evans and concluded the lockdown could be lifted. Supervisor Evans then radioed Dispatch and the Emergency Lockdown was lifted at 0705 hours at which time both Supervisor Nurse Administrator and Security Manager King were notified. Security stood by and was informed D/S Williams that the incident would be turned over to Orlando Police Department (OPD) which has jurisdiction over the incident. On Tuesday, June 6, 2017, at approximately 2134 hours, Trooper Jason Brown, Unit 522, conducted a traffic stop at the junction of US 460 and Camargo Leevee Road in Montgomery County. Jimmy White was pulled over for driving with no headlights. When Trooper Brown made contact with Mr. White, dispatch advised Mr. White had active arrest warrants. When Trooper Brown advised Mr. White that he had active warrants and was under arrest, Mr. White began to flee on foot. Trooper Brown chased him approximately 300 feet giving him loud verbal commands to stop running. Mr. White tripped over some debris in the yard he was running through and fell to the ground. When he got back up to continue running, Trooper Brown attempted to place him in hand cuffs.
On Thursday, 05/18/2017, at about 9:27 p.m., partner Officer Griffin (#3135) and I were on uniformed patrol in a marked patrol vehicle (Unit #21) as 3A21. We were dispatched to an injury traffic collision at Lake Ave. and Maple St. Upon our arrival, we saw three vehicles that appeared to be involved in a traffic collision. While we were assisting with the scene, we saw Victim Captain Konrad struggling with the suspect, later identified as Kevin Hendrix. Officers Griffin, Riehl (#7454), Spatz (#8058) and I grabbed Suspect Hendrix and took him to the ground. I placed properly adjusted, double locked handcuffs on Hendrix. Officer Griffin and I placed Hendrix in our patrol vehicle.
On April 7,2018, I was assigned the duties as Field Training Deputy. At approximately 0420, Arrestee Hedman, Roy (#1749220) was refusing to follow any orders given to him by Deputies when he entered the Intake Area. While in Holding Cell B116, Arrestee Hedman attempted to kick Deputy Holliday twice. I took control of Arrestee Hedman's legs to prevent him from kicking any staff members. While attempting to remove Arrestee Hedman's pants, Inmate Hedman attempted to stand up twice on the bench. Inmate Hedman refused to comply with any commands being given to him. On 11/07/17, I D/O B. Bailey #D1200 was assigned to the Medical Clinic as a floor officer. At approximately 0230 hours. Inmate Bowman, Torry BKN #1720190 refused medical treatment. As a result, he was given a refusal to sign so he could be returned to his floor. Inmate Bowman was given a pen so that he could sign the refusal. However, Inmate Bowman did not return the pen. Inmate Bowman was given several verbal commands to return the pen; however, he failed to comply. Lt. Steele was notified. Consequently, 8 male inmates in 3LB 23 were searched also a strip search was conducted. No pen was recovered.

On Monday, 11/30/2015 at approximately 1755 hours, Officer Lauritzen and I responded to intersection of Holt Blvd and Plum Avenue to assist Sergeant Ortiz to subdue a suspect who was fighting him. Upon arrival, I saw other OPD officers trying to handcuff the suspect, later identified as Kevin Larry. Larry was on his stomach and he was yelling that he did not do anything. Larry was resisting arrest by turning his hip side to side and was trying to pull his hands to the front of his body. Larry was grabbing our handcuffs and he was not letting them go after being told to let go. We told Larry to not resist arrest and stop fighting. On 02/06/2017, at approximately 1715 hours, while I was conducting meal distribution and ID Headcount in Durango Jail, 3225 W. Gibson Lane, Phoenix, AZ 85009, Housing Unit D-3, Pod D. When Inmate Peterson, Jordan T324870, cell 57-2 received his meal tray, I noticed he had dried blood stains on his shirt, his left eye, left cheek and left corner of his lip were bruised and swollen. At that time I asked the inmate what happened and he replied, "It’s nothing." I told him to step out of the pod and have a seat in the multipurpose room of the housing unit.
On Sunday November 24, 2016 at approximately 16:02 hrs. Officer L Wilkerson and I Ofc. R. Holcomb were dispatched by dispatcher B Coleman to 304 Hopkins Drive in reference to a disturbance. Upon my arrival I observed a white female identified as Whitney Anderson who exited her vehicle as I attempted to park my patrol unit on the side of the road. As I was exiting my patrol unit Whitney began to yelling at Darrell Anderson. As I was approaching the two subject I instructed Whitney to stop multiple times.
