INVESTIGATION: On 08-08-2016 at 0525 hours, I was notified by Detective M. Carter # 181968, of a traffic crash fatality that occurred at the intersection of Memorial Highway and Kelly Road. at 0451 hours. I responded to the crash scene and upon arrival at approximately 0703 hours, I was assigned to forensically map the scene. I also interviewed two witnesses to the crash, (name & name) (See interviews) Approaching the scene from the east, HCSO Deputies had the entire intersection of Memorial Highway and Kelley Road closed to traffic. I saw a maroon in color Ford E150 cargo van in the inside through lane of eastbound Memorial Highway, facing in a southeast direction. The van had sustained damage to the front of the vehicle. I viewed video surveillance from LRHS camera #7.24 at approximately 3:04 p.m. and observed a Red pick-up while backing up strike the vehicle in question left rear back fender near the back tire. At approximately 3:05 p.m I observed the suspect getting out of the truck and walking up to the vehicle in question and kneeling down near the left rear area as if he was assessing the damage. On Thursday, 05/18/2017, at about 9:27 p.m., partner Officer Griffin (#3135) and I were on uniformed patrol in a marked patrol vehicle (Unit #21) as 3A21. We were dispatched to an injury traffic collision at Lake Ave. and Maple St. Upon our arrival, we saw three vehicles that appeared to be involved in a traffic collision. While we were assisting with the scene, we saw Victim Captain Konrad struggling with the suspect, later identified as Kevin Hendrix. Officers Griffin, Riehl (#7454), Spatz (#8058) and I grabbed Suspect Hendrix and took him to the ground. I placed properly adjusted, double locked handcuffs on Hendrix. Officer Griffin and I placed Hendrix in our patrol vehicle.
At approximately 7:27 a.m., Detective J. Figueroa arrived Cape Fear Valley Medical Center 1638 Owen Drive Fayetteville, North Carolina 28304 in-reference to an additional victim suffering from a gunshot wound. The individual was identified as Kenneth Alexander Shaw (black male, 7/5/1988) who arrived in a gray Ford Fusion North Carolina Registration PAB5727. Detective J. Figueroa was contacted by Officer Catlett who advised an Amos rashaad Shaw (black male, 10/10/1989) was in the parking lot and had brought his brother Kenneth to the Hospital.
I noted it was daylight with no rain. Sergeant Marler’s 2013 Dodge Durango (002RSN) was in good condition. The incident occurred on a two lane county road with a posted speed limit of 35mph. There was an AT&T telephone box damaged during the incident. Sergeant Marler’s vehicle was not damaged during the incident. The suspect’s vehicle exhibited extensive damage. The incident was not called out over the radio therefore the duration can only be estimated; approximately 1 minute. The incident covered a distance of approximately 1500 feet.
On Thursday, August 31, 2017 at approximately 2232 hours, I was dispatched to a residence across the street from 2220 University St in reference of a suspicious person inside of a black truck. Upon arrival I spoke to the complainant Ms. Delisa Smothers. Ms. Smothers stated that at approximately 2230 hours, an unknown black male went inside her neighbor's truck located across the street at 2221 University street.
On Thursday, July 14, 2016, at approximately 0847 hours, Patrolman David Stamets had a vehicle stopped on the 100 block of East Oak Street. Immediately after Stamets stopped the vehicle Your Affiant positioned his vehicle on the 100 block of South White Street near the Oak Street intersection facing southbound. When exiting my vehicle your Affiant observed a gold Pontiac sedan accelerating northbound on South White Street at an unsafe speed towards Stamets and your Affiant's location.
On February 9, 2011, at approximately 7:15 AM, I, GBI Special Agent Abigail Wilkes, arrived at 293 Stone Road to process a crime scene involving two deaths. When I arrived, the crime scene was secured with yellow tape. I met with Deputy Sheriff Ray Chandler. Chandler told me that the two victims, Steve Campbell and Sherry Campbell, had two gunshot wounds each. Sherry Campbell died in the master bedroom of the residence, but emergency medical services (EMS) took Steve Campbell from the scene. Chandler informed that the EMS personnel entered the residence through the front door and placed Steve Campbell on a stretcher. Steve Campbell 's injuries created a blood drip trail when he was transported to the ambulance. The drip trail began in the master bedroom, went through the hallway, out of the front door, and into the yard of the residence. Steve Campbell died on the way to the hospital.
At approximately 1:28 a.m., I arrived at the scene and met with Trooper Otten, who advised what had transpired prior to my arrival. Trooper Otten stated a blue 2014 Ford F-150, driven by Eric A. Wenzel, was traveling northbound on Illinois Route 4 south of Norton Lane. While traveling northbound, the blue Ford veered to the left and entered the southbound lane of the roadway. Once in the southbound lane, the front passenger’s side of the blue Ford struck the front passenger’s side of a southbound silver 2003 Ford F-150, driven by Alan L. Schilling. Trooper Otten stated both Mr. Wenzel and Mr. Schilling sustained incapacitating injuries in the collision Trooper Otten advised the silver Ford’s front seat passenger, Sarah L. Wagner-Schilling, sustained life-threatening injuries in the collision and was not expected to survive. Trooper Otten said Mr. Wenzel admitted to having been drinking prior to the collision and was suspected of driving under the influence. All three occupants had been transported from the scene
Trooper Cox completed the Illinois Traffic Crash Report 11-16-00806 to document the collision. A copy of Trooper Cox’s report is available in Appendix 2 of this report.
Brace yourselves everyone! This is Dave Smith here at the scene of the wreck on I-92. A semi pulled in front of them, and they swerved to avoid hitting it. If I were them, I would take the back roads like Route 66. Route 66 has small town restaurants, stores, and everyone knows everyone. It does not have those huge semi's that zoom past you. The article on the tragic crash at Michigan speedway was truly shocking. It goes more into the aftershocks of the accident but does in fact touch on what happened during the astounding event. The article seemed to be created so it can shed some light on how the whole ordeal was handled. This is very important due to its relation with broadcasting to the people watching which is an important job. The article did a good job of showing a mistake that happened to all the sports fans and people wondering why the situation was ignored. On April 05, 2016 at approximately 1518 hours, I was dispatch to 2004 Pine Street in reference to a death investigation. Upon my arrival, I met with Detective Campbell who was the lead Investigator at the scene. Detective Campbell stated that the victim neighbors reported a strong smell coming from the victim’s (Cortiz Lazarz) residence. The neighbors called 911 and Officer Volk was the first responded officer on scene. Defective Campbell and officer Volk stated that the victim located on the living room floor, fazing the south from the residence. Detective Campbell requested I process the scene for evidence and documentation.
