On December 7, 2016, Investigator Sergeant Martin Marler contacted me via phone and advised of an incident that he and Investigator Emmitt Reyer were involved in at 46th Street and Judy. I noted it was daylight with no rain. Sergeant Marler’s 2013 Dodge Durango (002RSN) was in good condition. The incident occurred on a two lane county road with a posted speed limit of 35mph. There was an AT&T telephone box damaged during the incident. Sergeant Marler’s vehicle was not damaged during the incident. The suspect’s vehicle exhibited extensive damage. The incident was not called out over the radio therefore the duration can only be estimated; approximately 1 minute. The incident covered a distance of approximately 1500 feet. Mr. Montgomery said he was in the area of Highway 161 driving , in his own words “stupidly”. Mr. Montgomery said he was attempting to catch his girlfriend cheating on him and he was in a hurry to do so and then return the vehicle he was driving to his mother. Mr. Montgomery said he had no reason to run stating that he knew he had no warrants and he was unaware of his ARDL being suspended. Mr. Montgomery said he was unaware that Sergeant Marler and Investigator Reyer were attempting to stop him and that he never remembered seeing blue lights. Mr. Montgomery said he could not get his vehicle to stop and believed it was a good idea to exit the vehicle while it was traveling down the roadway. Mr. Montgomery said he had no intentions of running from Sergeant Marler and Investigator Reyer, My investigation revealed that Sergeant Marler’s actions where within General Policy 04-003 Pursuit Driving-General. Though Sergeant Marler and Deputy Reyer’s actions did not violate policy, their actions where a violation of the Uniform Standards of Conduct 06-Committing Unsafe Acts or Endangering Self or Others. Upon arrival, Officer Wheeler activated his (BWC1) body worn camera which was later uploaded to evidence.com. As our patrol vehicle pulled up to St. Luke’s emergency room entrance, I viewed a male later identified as (RV1) Devinelle Lavaughn Broussard with a cellphone in his hand attempting to prevent a female, later identified as (B1) Eleasia Ashanti Fraise, from throwing a (EVD1) red brick. I next saw Fraise throw the brick at a parked (D1) silver BMW, located along the south side walk. The brick fell to the ground and Fraise went to picked it back up. Fraise picked the brick up and moved in the direction of our patrol vehicle with the brick at shoulder height. I parked the patrol vehicle and immediately got out drawing my firearm to a low ready. At approximately 1:28 a.m., I arrived at the scene and met with Trooper Otten, who advised what had transpired prior to my arrival. Trooper Otten stated a blue 2014 Ford F-150, driven by Eric A. Wenzel, was traveling northbound on Illinois Route 4 south of Norton Lane. While traveling northbound, the blue Ford veered to the left and entered the southbound lane of the roadway. Once in the southbound lane, the front passenger’s side of the blue Ford struck the front passenger’s side of a southbound silver 2003 Ford F-150, driven by Alan L. Schilling. Trooper Otten stated both Mr. Wenzel and Mr. Schilling sustained incapacitating injuries in the collision Trooper Otten advised the silver Ford’s front seat passenger, Sarah L. Wagner-Schilling, sustained life-threatening injuries in the collision and was not expected to survive. Trooper Otten said Mr. Wenzel admitted to having been drinking prior to the collision and was suspected of driving under the influence. All three occupants had been transported from the scene
In March 2001, a Georgia deputy attempted to pull over 19-year-old Victor Harris for driving 73 miles-per-hour in a 55 miles-per-hour zone. Instead of slowing down, Harris increased his speed, which initiated a chase at speeds exceeding 85 miles-per-hour. The deputy radioed for backup to pursue him. Deputy Timothy Scott, the petitioner in this case, joined the group of officers pursuing Harris. The officers attempted to box Harris into a shopping center parking lot, but he made a sharp turn, collided with Deputy Scott, and sped off again. Deputy Scott took over as the lead pursuit officer in the chase.
On Thursday, July 14, 2016, at approximately 0847 hours, Patrolman David Stamets had a vehicle stopped on the 100 block of East Oak Street. Immediately after Stamets stopped the vehicle Your Affiant positioned his vehicle on the 100 block of South White Street near the Oak Street intersection facing southbound. When exiting my vehicle your Affiant observed a gold Pontiac sedan accelerating northbound on South White Street at an unsafe speed towards Stamets and your Affiant's location.

On August 23, 2014 Sgt. Lavelle was assigned to the 83 Precinct. She was present for duty at 0800 hours as the domestic violence Sergeant and she conducted several home visits. At 1450 hours she was post changed to the 83rd Precinct desk and she assumed the duties of the 83 desk office. Sgt. Lavelle remained as the Desk Sergeant for the entire tour. In summary, on 01/04/17 at 1745 hours, Ofc. Kelley #262 and I were patrolling the area of 5600 W 16th St., at which time I observed a vehicle with tinted windows and no rear registration light (05' grey Acura IL Q336438). A traffic stop was conducted on the vehicle.
On Sunday November 24, 2016 at approximately 16:02 hrs. Officer L Wilkerson and I Ofc. R. Holcomb were dispatched by dispatcher B Coleman to 304 Hopkins Drive in reference to a disturbance. Unit 2 was driving eastbound in the left lane of 167th St. between Orchard Ridge Ave. and Anthony Ave. in Hazel Crest, IL. Unit 2's approximate speed was 35 miles per hour. A large puddle of water had formed from a very recent rain. It covered the right lane and part of the left lane. The driver of Unit 2 noticed the puddle and proceeded to slow down to a speed of approximately 20 miles per hour. The driver of Unit 2 then noticed Unit 1 coming from behind in the right lane at a high rate of speed. Unit 1 tried to avoid the puddle without slowing down and impacted Unit 2. On 10/28/2015, at approximately 0232 hours, Captain O'Neill and your Affiant observed a red Chrysler PT Cruiser (PA Registration JWA5451) traveling south on State Route 924. The operator of the vehicle was observed and identified as Dawn HERRING. HERRING'S driver's license was expired and suspended for driving under the influence; the information was verified through a certified Pennsylvania Department of Transportation record. HERRING was observed traveling under the posted speed limit and repeatedly swayed her vehicle throughout the lane. On March 3rd 1991, Los Angeles Police chase a vehicle that was driving erratically. When the vehicle came to a stop after reaching speeds of nearly 120 mph, a total of four cruisers arrived to the scene, including six patrol officers and a sergeant.
