BCH Executives have agreed to begin the new touring motorcycle project with larger motors for long distance driving. The proposed target audience suggested for this product would maintain an income up to 100,000 annually. The project completion goal is within five (5) years of the marked beginning date. The company will use the current facilities where the medium-sized manufacturing are created with modification of the equipment and as well as additional staff. With an estimated goal of 100 larger motor motorcycle production at a cost determined by the approved budget, this project will utterly build additional profit for the company.
Mr. Wooten is a 33 year old male who presented to the ED following a visit to his primary care provider. Prior to Mr. Wooten coming to the ED his provider contacted TACT with concerns of Mr. Wooten mentioning suicidal ideation with a plan to use a gun to shoot himself and experiencing depression. At the time of the assessment Mr. Wooten denies suicidal ideation, homicidal ideation, and symptoms of psychosis. He states having no suicidal ideation today, however mentioned to his provider a previous thought of harming himself. It should be noted Mr. Wooten was seen on 4/22/17 here at Randolph Hospital for reporting similar statement, however retracted his statement after reports a hidden agenda of only seeking anti-depressant medication to alleviate
Mr. Cinton is a twenty-three-year old, Hispanic of Puerto Rican descent male. He was referred to the Evolutions Treatment facility for his Substance abuse usage. Mr. Cintron reported his struggle with substance abuse has impacted his daily functioning. The purpose of the current evaluation is to (1) assess present level of functioning, and (2) identify relevant treatment and service needs.
Mr. LePape stated that his reason for the evaluation is because his wife has promised him that he will never see the children again. He indicated that she is trying hard to stop him having a relationship with his children.
On 09/26/16, Mr. Trotter began attending the TEECH Foundation, 1750 W. 103rd St. and has attended daily as verified by the executive director at the TEECH Foundation. On 10/13/16, Mr. Trotter met with the executive director at the TEECH Foundation who suggested that he begin one-on-one counseling sessions on-site. On 10/14/16, he began counseling sessions. The executive director at the TEECH Foundation informed Mr. Trotter’s DOC case manager that if deemed necessary, Mr. Trotter will be referred for extensive counseling after his one-on-one sessions conclude on 11/25/16.
