1.1 Identify the features of effective partnership working
The policy of person-centred care has become a mainstay of efforts to reform health care in the UK. Government policy is now built around this core concept, a major aspect of which is enabling individuals to participate in decision-making about their care at every level. By concentrating on the individual’s wishes and aspirations and placing them at the centre of a ‘planning circle’ the care team can assist with goal setting to ensure their best interests are maintained and the individual’s preferences are considered.

They will have details about the individuals past history and life which will all be taken into account when establishing the individual’s plan of care and support. An example might be if there are communication difficulties. A carer or family member can share information with me about how I can best communicate with an individual. This enables the individual to be listened to and supported in ways that they desire and choose.
1.3 Analyse how partnership working delivers better outcomes
Health and social care organisations increasingly need to work together in partnership to get better value from the available resources and improve services and outcomes for service users, by improving quality of support for service users. Good relationships between partners, a common vision and understanding of expected achievements and what outcomes need to be delivered are critical to the success of partnerships. This will enable a more responsive service, which is well co-ordinated approach to the service delivery, and better value for money by ensuring reduced duplication of services.
Multi-disciplinary and multi-organisational working allows for a holistic approach to problems. With good communication channels and relationships with people from different organisations and disciplines, the team is made larger and therefore more resources are available to call upon and use, which benefits the teams objectives, to provide the best
This presentation is going to talk about person- centred care, confidentiality, respecting privacy and dignity and protecting from risks and harm. The common core principles are important to every Health and Social Care setting as they provide a basis for a general understanding of promoting good mental health and recognising signs of poor mental health among everyone receiving care and support. The aim of care home are to meet the identified needs of individuals who live in that home. An older person might need to live in that care home for years. It is important for staff to be aware of responsibilities in delivering care to support the individuals who live at that home. These common principles of health will help develop the workforce that respond confidently to the individuals and supporting the life they are leading.
Effective partnership amongst care home workers and other professions such as health care workers relies on a recognition that all parties care about the same goal: that of promoting the health of the service user.
Person centred care could be defined as focusing on a person’s individual needs, wants, wishes and where they see their target goal. Person centred care also takes into account delivering person centred care to the patients family and carers, as well as the multidisciplinary team that is working together to provide care. The service user is the most important person in decision making for their health care and the nursing process. (Draper et al 2013). Person centred care reminds nurses and care staff that they are caring for the service user, their families and care staff providing the care, this allows the patient power in decision making towards their health and wellbeing. (Pope, 2011)
1. The service user and his/her care team including any advocates, make parallel around a common understanding of what is Important to the individual now and in the future.
In this report I will be investigating how care services meet the needs of individuals by firstly analysing the needs of an individual using care services. Then I will go on to explain the roles of the care planning process in identifying needs, and explain the features of a positive care practise. After explaining that, I will analysis positive care environments and evaluate how they meet needs before finally explaining the role of legislation in promoting a positive care environment.

As a support worker, it is imperative that we look at all aspects of health and social care with a holistic approach when planning and provision is concerned. Holistic means looking at a situation as a whole and not just part of it.
I am going to discuss the care strategies that can be used to support individuals and discuss the circumstances in which they might use.
Reporting of Injuries,Diseases and dangerous occurrences regulations of 1995 requires the reporting of work-related accidents, diseases and dangerous occurrences. The Act applies to all work related activities, but not to all work related incidents. The objective of the regulations is to enable the enforcing authorities to identify where and how risks arise and to investigate serious accidents so as to prevent them from occurring in the future and thus providing a safer work environment. The enforcing authorities can then help and advise you on preventive action to reduce injury, ill health and accidental loss,the main points of our own policy that relate to this are:

Partnership is driven by a desire for collaborative advantage and can offer many positive outcomes, like benefits for staff and services such as less replication between different service providers. It helps them see others point of view and it keeps them from being selfish, it helps young people interact with others to achieve a goal, and it helps them develop socially. It’s important to work in partnership with others so that all agencies can share information and concerns about children and young people in their care, it also helps to keep the children safe and away from harm.
Partnership is when two or more organisations work together, showing cooperation and collaboration. This can provide better care and support for service users. For example, different funds working together, instead of working individually and stressing. Another example, a social worker and the health visitor will need to work together, share ideas and use different skills to develop a support plan that will benefit children and families. This also promotes multi-disciplinary working. Partnership thus can reduce conflicts and enhance team working skills as professionals can work together, sharing responsibilities and ensuring need led approaches are met and set. This can be done by reviewing care plans and offering support.
This unit develops understanding of the values and principles that underpin the practice of all those who work in health and social care. The essay consider theories and policies that underpin health and social care practice and explore formal and informal mechanisms required to promote good practice by individuals in the workforce, including strategies that can influence the performance of others. The first part of this essay will consider how principles of support are implemented by using Overton house residential care home to evaluate and explain how principles of support are applied. Key concepts such as person-centred approach and dilemmas and conflicts arising from the
