On this date worker visited Walker Rehabilitation in Carbon Hill Al., for the purpose of monitoring Ms. Phillips placement. When worker arrived Ms. Phillips was in bed sleeping but woke when her name was called out. Ms. Phillips was dressed appropriately with good personal hygiene. Ms. Phillips did not remember worker from previous visits but was friendly with worker. During visit, Ms. Phillips talked about her roommates and appears to have adjusted well to her new living arrangement. This was a concern for Ms. Phillips because she went from a room for two into a room of four. Ms. Phillips stated she enjoys the food but doesn't participate in activities. Worker spoke with Ms. Ana Dyer and learned the family was getting assistance with housing and transportation from a local church. Ms. LaVone Yarbrough and her husband run a ministry that helps family reunite. However, Ms. Dyer is concerned about discharging Ms. Blankenship to the home with narcotic and according to Ms. Dyer, Crimson's boyfriend lives with them. Worker has not been able to conform. Ms. Dyer stated when Ms. Blankenship discharged from the NH she would need a doctor and transportation to the doctor appointments. RT came into the MOP office in a great mood and greeted writer nicely. However, RT appears to be a socially awkward person. RT communicates with writer but he don't usually talk much. Writer inquired about RT's wellness and RT informed writer that he was doing well and feels like a normal person. RT stated that he just went grocery shopping and was at the neighborhood garden earlier planting. RT informed writer that he no longer goes to venture housing day program because he knows himself and wants to stay out of house; therefore, RT believes that the best thing to do is not to go to the program. R-Staff EW met with NV to work on some of his recovery goals. Staff EW observed NV drive to Woodman’s grocery store where he purchased some groceries. After Woodman’s, staff EW observed NV drive to the Dollar tree store. At this store, staff EW showed and discussed with NV the many cheaper options there are when it comes to purchasing food and not just going to Woodman’s. Staff EW noticed that’s NV seemed to be a lot more confident in driving and he discussed with staff that he wants to try and drive on the highway in the near future. NV talked
On November 20, 2015 worker visited Selby Ridge Rehab and NH, for the purpose of monitoring Mr. Parker’s situation. Mr. Parker is still in the physical therapy section of the NH but will be moved to a long-term bed soon. When worker arrived, Mr. Parker was lying in bed watching TV. He was dressed appropriately with good personal hygiene. Worker introduced herself and explained the reason for the visit. Mr. Parker remembered worker from the previous visit but could not remember worker’s name.
On this date worker visited the residence of Mr. William Teske, for the purpose of monitoring his situation. When worker arrived, Mr. Teske was heating up a microwave dinner. He stated he was his favorite, spaghetti and meatball. Mr. Teske remembered worker from previous visits. Mr. Teske was appropriately dressed with acceptable hygiene. There was mail lying around the apartment and there the odors of cigarette smoke. During visit Mr. Teske keep O2 on and did not smoke. Mr. Teske thanked worker for helping him get Medicare part A. He stated he still receives Elder Care and home health but HH switched from Amedysis HH to Alacare HH and Alacare is in the process of changing him from HH to Hospice. In chapter three of our book it talk about the basic function of families meeting the needs of their disabled child which includes day-to-day task such as cooking, cleaning , laundry, transportation and health care. Brandy meets all of Marks’ needs staring her day with a walk with the family dog at 5:30 am. After her walk with the dog is completed she gets Marks’ medication ready before she wakes him up. Brandy prepares the medication the night before. Brandy wakes Mark up at 5:50 she gives Mark a shower and she dress him for school because Mark has grown this process takes about twenty minutes.
Betty Jo reluctantly made the decision to move to an assisted living community, due to high pressure from her healthcare providers and family. She is an 85 years old and a widow of 3 years from her beloved husband of over 50 years. Her husband made sure his financial affairs were in order before his death, and thus managed to provide financial comfort and security for Betty Jo. She has two children, both live out of state. Her daughter Karen, has two adolescent children, and visits her mother once a month. Her son, Adam, is a recovering alcoholic who has trouble keeping a job and frequently seeks financial assistance from his mother, yet rarely visits her at Sunrise Pines. Betty Jo often has to sit down for a minute, due to “dizzy” spells and becomes fatigued at a greater frequency than she did before her husband passed away. Her decision to move came after falling twice in her home, upon having hip replacement surgery. She had trouble keeping her physical therapy appointments, and managing her pain medication. Prior to her move, Betty Jo was active in her church and in the Red Hat Society. Now that she is living in Sunrise Pines, she has become more withdrawn and rarely eats her meals in the main dining area. When she does participate in social activities, she acts out and becomes hostile with her fellow residents. The community director received word that she is refusing to take her antidepressant and blood pressure medication. The director reached out to the local
Kathy and Raymond have visits one day per week for two hours per day. Worker supervises these visits once per month. The first visit this worker will be supervising is on November 23, 2016. These visits occur at the Godfrey, Illinois McDonalds play place, but were taking place outside in the community prior to this when the weather is nicer. During the last supervised visit the case assistant observed Raymond reading the newspaper. The tried children interact with the case assistant; the case assistant redirects the children to interact with Kathy and Raymond. Johnathan spent most of the visit in the play place and Zoey spent most of the visit on the tablet. Raymond left the visit and returned 20 minutes later telling Kathy “they (the children)
