Focus: Reighn will understand and communicate her feelings in an appropriate manner on a daily basis
Intervention: CSP and Reighn discussed ways the youth is expressing her feelings at home and school. CSP assisted Reighn in processing her thoughts about upcoming court date and adjusting to having another youth in the home. CSP addressed MHS concerns the youth behaviors are regressing.
Response: Reighn was hyper during the visit. She expressed school is getting better and her grades are increasing. All her make up work was corrected and turned in to the teacher. The youth hopes she can return home soon or live with a family member. She misses her family and feels family can help her heal. Reighn does not think her behavior has changed.
Focus: Ms. Smalls (MHP) administered the Casey Life skills assessment, the Circle of support and the Child and Adolescent Needs and Strengths assessment with the assistance of Ms. Givens (MHS) and Antazia.
Intervention: MHP and MHS debrief and process of Santonias’s disregard for appropriate boundaries. MHP challenge MHS beliefs of comparing her own child learning from past mistakes to Santonias repeating the mistakes. MHP asked how limits and boundaries are enforce in the home, community and school. MHP request for MHS to monitor Santonias with total supervision and obverse to identify patterns.
stays focused on reality and her idea of perception as well as the friendships she acquires in her two year stay at McLean Hospital and her recovery period once she is released.
Intervention: CSP and Aliyah discussed her week at school and home. CSP assisted the youth in processing incorrect information about court and returning home. CSP praised the youth for identifying the source of frustrations. CSP and MHS explained Charleston County DSS had a hearing not formal court. CSP, youth and MHS discussed concerns of uncooperative and lack of receptiveness. CSP encouraged the youth to comply with the rules of the home and school.
CM spoke to Kiara Gelin (youth) regarding a follow-up on services. Youth reported she continues to attend the Partial Hospitalization program at the Jersey City Medical Center. CM was informed that youth stopped attending the TASC program due to time conflict and youth will resume the program in September. CM and youth disused employment; youth had a job interview at Wendy’s, Fast food restaurant. CM encouraged youth to apply at local jobs in the mall, CM verbally provided youth with a list of jobs that are hiring in the community. CM inquired about the Cook, Eat and Talk program. Youth reported the family will follow-up with the program once the family phone is working. CM agreed. CM and youth discussed mentor
CM conducted a face to face meeting for Kiara Gelin (youth) at the Jersey City Medical Center Partial Hospitalization Program. In attendance were Jasmine Alexander (CM) and Kiara (youth). CM and youth discuss therapeutic services; youth reported she enjoys attending the program. Youth admitted to using substances and noted she will stop. CM and youth discussed employment. Youth is a part-time employee at Honeygrow restaurant in Hoboken, NJ. CM explained to youth Hudson Partnership CMO contact policy. CM requested for caregiver to contact CM to schedule CFT meeting and to discuss services. CM provided youth with CM’s contact information to provide to caregiver.
Case management services were provided through a face to face interaction in Costa Mesa. Present at the meeting were WYP (Wraparound Youth Partner) Andy Ngo and the youth. The service minutes are high, because WYP Ngo assisted the youth to remove bloatware on the computer. WYP Ngo role modeled for the youth and guided the youth through each steps. WYP Ngo discussed with the youth about school. The youth is doing well in school. The youth had a great Thanksgiving with his siblings. The youth is excited for Christmas. The youth can't wait to be off probation in January. WYP Ngo encouraged the youth to stay motivated until his court date. The next one on one will be on December 13th, 2016.
Intervention: MHP discussed the importance of being responsible. MHP advised Charles to focus and pay attention in class. MHP recommended that Charles utilizes the study skills that were taught. MHP encouraged Charles to avoid confrontations with his peers. MHP discussed anger management techniques with Charles. MHP instructed Charles to always ask for help when he does not understand something. MHP advised Charles to continue applying the calming techniques when experiences feelings of anger or frustration. MHP provided positive reinforcements for improved academic performance and behavior.
CM conducted a Face to Face meeting at the family’s home. In attendance were Jasmine Alexander (CM), Mrs. Christine Edman (mother), Destiny Tyler (youth), Freddie Tyler (father) and Ms. Reyes (DCP&P). Destiny informed the team that she started 3rd grade last week and she is excited about school. Mrs. Edman explained to the team that Destiny bedtime is at 8:30pm and the only TV channels Destiny is allowed to watch are Wild Life and Ocean Life. Mrs. Edman stated that Destiny has been spreading rumors about her and her brother sharing the same room in the past. Mrs. Edman and Mr. Tyler explained that Destiny did not share a room with her brother due to the age and gender difference. CM was informed that Destiny used to sleep in her mother bedroom when her brother was living with the family. Mrs. Edman and Mr. Tyler wants Destiny to tell the truth. CM and Ms. Reyes suggest for the family to attend family counseling to work on communication. Mrs. Edman and Mr. Tyler informed CM that family counseling is not a good fit for them due to their past experiences. Ms. Reyes informed the team that she is waiting for the evaluation to come back from Audrey Hepburn. Also, Ms. Reyes will seek information regarding Project Safe with her supervisor. The team was informed that Destiny will continue attending Christ Hospital outpatient services until DCP &P find a psychiatrist that specialize in sexual assault for the youth.
Intervention: MHP educated Timothy about socialization. MHP taught Timothy techniques for anger management. MHP discussed with Timothy the importance of education. MHP provided additional educational worksheets on peer engagement. MHP demonstrated the difference between appropriate and inappropriate behavior. MHP instructed Timothy on the benefits of improving his academic performance. MHP provided Timothy with strategies to improve his social skills. MHP provided strategies to improve his study habits. MHP provided positive reinforcements for improved behavior and better social skills.
The team conducted a school conference meeting via phone regarding school placement and safety plan for Treshawn (youth). Phone conference attendees were Jasmine Alexander (CM), Mrs. Borders (resource parent), Dr. Mayo (CST), School’s principal, youth’s behavioral aid and Cathy (teacher). The team discussed safety concerns and talked about youth asking staff for money in and out the school building. CM was informed that youth continues to beg for money outside the local 7 eleven store. Mrs. Borders explained difficulties of getting youth on the bus in the morning. Mrs. Borders requested for a bus aid; the school came up with a plan for Mrs. Boards to walk youth to the bus and wait for him to get on the bus. Mrs. Boards agreed.
Discussions: Ms.Erickson reported that Nathaniel is doing well in his new school and at home. She said that Nathaniel is doing better with the new medication. Nathaniel presented calm, respectful and compliant during the visit. Ms.Erickson reported a decrease in appetite and weight.

The social worker uses CBT to problem solve how the family can improve handling the client’s behaviors. By delving into the client’s reasons for certain behaviors attributed to her coping techniques, challenging the parent’s negative cognitions about her substance use. Then, replaced by positive thinking and therefore better able to provide support to the client. This depicts an improvement in the communication between client and parents. And, again normalizing why they may question her behaviors, but also acknowledging the strength in improving their understanding of her behaviors, reinforcing the healthy interaction. At this point, you observe the father involved in problem solving with his daughter, reemphasizing the enhancement of the family function. Observation: Jason report he was feeling great, He also informed the MHS that he has the better behavior in his class. Jason said he is doing good at home and he has been helping out around
