CM conducted a Face to Face meeting at the family’s home. In attendance were Jasmine Alexander (CM), Mrs. Christine Edman (mother), Destiny Tyler (youth), Freddie Tyler (father) and Ms. Reyes (DCP&P). Destiny informed the team that she started 3rd grade last week and she is excited about school. Mrs. Edman explained to the team that Destiny bedtime is at 8:30pm and the only TV channels Destiny is allowed to watch are Wild Life and Ocean Life. Mrs. Edman stated that Destiny has been spreading rumors about her and her brother sharing the same room in the past. Mrs. Edman and Mr. Tyler explained that Destiny did not share a room with her brother due to the age and gender difference. CM was informed that Destiny used to sleep in her mother bedroom when her brother was living with the family. Mrs. Edman and Mr. Tyler wants Destiny to tell the truth. CM and Ms. Reyes suggest for the family to attend family counseling to work on communication. Mrs. Edman and Mr. Tyler informed CM that family counseling is not a good fit for them due to their past experiences. Ms. Reyes informed the team that she is waiting for the evaluation to come back from Audrey Hepburn. Also, Ms. Reyes will seek information regarding Project Safe with her supervisor. During this assessment, I interviewed a single-family home with a 32 year old mother, M.S, with a 4 year old daughter, M.S. II, from Costa Rica. The daughter was in pre-school and the mother had graduated high school and immediately worked with event planning. The family does not attend church, but practices under the Catholic traditions. I would place this family under the preschool stage of the family life cycle due to the oldest child is 4 years of age and the mother is adapting to her child’s needs in growth-producing ways. M.S and her daughter love spending time outdoors taking walks and visiting gardens. M.S. sews various items, including lunch boxes and aprons in her free time too.
CM spoke to Justin about his recent behaviors in school, his inappropriate dressing and leaving school grounds. Justin informs CM that he left school grounds to visit his adoptive parents because he misses them. CM inform Justin that he did not have permission from DCP&P to visit the Williams. CM informs Justin that his safety comes first. CM also informs Justin that Joslin was also aware of his behaviors in school and according to her if he continues displaying unsafe and oppositional behaviors she can violate him. Justin states that he does not want his probation to be violated. Mrs. Monestime reports that Justin is having a better day today. She reports that he completed his English assignment and he has not been wondering the halls. CM
Family health assessment is an integral part of the formula used in creating a customized plan of care for the families’ health care. Family health assessment is also a tool that can be used to identify and evaluate the family’s health concerns, their life style and also helping families make good decisions regarding their family’s health. Family’s perception towards health and health promotion could be very different and unique, which makes the Family health assessment even more challenging for the nurses and health care professionals. The Purnell Domains of Culture assessment gives a vast amount of information in regards to a persons cultural background, family and heritage that gives care provides a better understanding of their client or clients family. As a future care provider, I took the time to speak to Mrs. Smith about herself and family to gain insight on whom they are as a family and what influences them to be who they are today. Over the course of the domains of culture I have been able to increase my perception of who and what drives this family and how their beliefs came about.
They are on a regular diet with few restrictions on fried foods and fat intake. The children eat school lunch and the parents from outside restaurant choices. Mom tries to cook a meal a day after work or they seldom eat Subway or Pizza Hut. No variation in weight gain or weight loss reported. They try to eat dinner as a family at home on a regular basis. However, this was not feasible all of the time due to dad’s late night work hours and CH basketball practices after school. They generally get 6-7 hours of sleep per night and denied any sleep deprivation. No exercise program has been implemented by this family. CH is the only physically, athletic member. DH stated that he likes to ride his bike. The children are active in Sunday school and participate in summer camp.
Different assessment examines different features of an individual, or similar assessments examine similar features in different ways. Three assessments that regard family conditions and that are considered important are, the Family Adaptability and Cohesion Evaluation Scales (FACES III), the Family Assessment Device (FAD), and the Self-report Family Inventory (SFI).
As the society we live in continues to transform, nurses need a comprehensive tool to assess family’s health patterns. Family units are influenced by environment, biological, mental, social and spiritual factors. Assessing these areas for health promotion and disease prevention will take all of these influences into consideration (Edelman & Mandle, 2010). A priority to a family focused health assessment approach is to adequately question, assess and examine these areas. Half the children in this country live in homes in which one or both parents work. Twelve million children in this country do not have health insurance, and over 4.5% of all children are victims of suspected child abuse or neglect (Doktor and Poertner). Believe it or not, these are all indicators of the demand for Family Resource Centers within our school systems. Many question whether Family Resource Centers are worth the money the state pours into them. However, in serving students, parents and teachers, there should be no question of their significance. Due to The Kentucky Education Reform Act in the early 1990's we have seen the development of Family Resource Centers in Kentucky, and they are fulfilling
