On June 19, at 12402hrs, Nurse Manager notified VA police about an employee who entered the 1L109 (female) bathroom and was peering into her stall from both sides of the door. Investigation revealed that the employee is a Transgender and identifies as a female as the individual peering into the stall. VA Police advised the Nurse Manger to complete a report of contact and turn it into the Supervisor for MSA. On Jun 21, 2017 at 1337 via email, VA Police was notified by the MSA supervisor that management needed Police Service to conduct an investigate into this matter. On June 23, at 1305 hrs. I met with Union Stewart, Jesse Cole and employee Steven Johnson (who goes by Stephanie). I advised both Cole and Johnson that I was not conducting a Criminal Investigation. I had received a complaint in writing concerning Johnson peering into the stall in female bathroom 1L109. Further, I needed to get her side of the story. I asked Johnson if she remembered entering the bathroom 1L109 on June 19, 2017 at 1230 hrs. Cole at this point requested to see the report of contact from Raina Alejos. After they reviewed the report of contact I continued my questions with Johnson. Johnson stated she typically uses the first stall. She noticed a that the first stall was being used and entered the second stall. She denied peering into the stall. I asked her why she used that bathroom when there are three bathrooms closer to her work station in the Call center. Johnson explained that is the bathroom she started using in March of this year is comfortable in use the bathroom.

On 3/13/16 SO EMT Perez was notified that an engine had come on campus. SO EMT Cunningham responded along with SO EMT Perez and caught up with the engine by lakeside commons. The driver of the engine stated that he was responding for a lift assist to 3144/3146 apt 220 but that he wasn’t sure which one he was going too. On 2/17/16 at approximately 8:30 P.M, Shift Supervisor (S/S) Enmanuel Cabrera was notified by Safety Specialist Matthew Marsicano that IH services associate Elizabeth Andrisoqie was asked to be removed by IH services Project Manager (P/M) Courtney Berger via cell phone. Upon arrival, S/S Cabrera spoke with IH services associate Elizabeth Andrisoqie and advised her that IHS P/M Berger asked for her removal. On Friday 09/16/2016 at 2300 hours, Security Officer Michelle Gallegos was badging visitors in the E.D. Lobby when the sister of a patient in E.D. # 32 lunged over the counter, knocking down a computer screen and a cup of water, then striking Officer Gallegos in her left arm. Security Officer (SO) Omar Alonso, was inside the E.D. Security Office, and responded immediately verbally instructing the visitor that police were going to be notified and that she needed to leave property immediately, and he also called Dispatch for immediate assistance. The female visitor was with 5 other females and her mother so they all followed Officer Alonso, outside of the E.D. Lobby. They were screaming and hollering that they were being treated disrespectfully by the badging officer who would not let them go back and visit her sister. Security Officers Ariel Weiland, Demetrio Flores, and Supervisor Steven Evans all responded by 2303 hours and helped to keep all 6 visitors outside while SO Alonso, conducted an investigation into the visitor’s allegations that they were being unfairly treated and were given erroneous information. The reporting party (RP) stated on 5/31/16 at 6:20PM the RP responded to a call from Rialto Police Department regarding a conflict between resident Dorothy Goforth DOB: 8/12/52 and her roommate (name not provided). The administrator Willicia Fuller was observed yelling at Dorothy and giving her an attitude. According to the RP Dorothy was having difficulty pulling up her pants and required assistance. The administrator refused to assist the resident with pulling up her pants. Brenda Castro expressed that the Collections department would not be able to services his current account and instructed the member to call customer service.
