CCIB received a call from DCFS. They received a call from a member of the staff of LA County Board of Supervisors (Ofuna). Elizabeth reported that FFA SW Claudia Cuevas was driving her personal vehicle (with no foster children in it) when she was involved in a vehicle accident. The other driver was a Miss Cummins, was concerned and convinced Ms. Cuevas to take cash for the damage repair. MS. Cuevas accepted $450 in cash (in two payments.) Miss Cummins is apparently under the age of 18 and her mother, Nobie Cummins became aware of the situation and confronted Ms. Cuevas at the FFA. (Karen cash, director of the FFA was present). Nobie Cummins accused Ms. Cuevas of intimidation and manipulation of her daughter. (Daughter was called and on speaker

RP stated law enforcement received a call from an anonymous caller stated an employee, Shemaiah Messengale grabbed the child Marley's arm which is fractured. Law enforcement contacted the child's mother to verify injuries. It was disclosed the child had suffered 4 fractures to her elbow and wrist. The RP stated law enforcement spoke with owner/licensee of the facility and the employee accused of causing the injury. They reported Marley sustained the injuries accidentally when she fell from slide. The RP stated the child was treated at Children's Hospital Los Angeles (CHLA) by Dr. Lindsey Andras who determined the injuries are consistent with a fall. CCIB Intake received referral #1113-8073-1224-4053654 from reporting party (RP) Counselor/Therapist Kristy Bynum with Child Guidance 3628 Stockdale HWY, Bakersfield CA 93309 (661) 322-1021. Former foster child Jacob Daniel Halbrook DOB: 12/09/01 who is the brother to foster child Sharra Russelle Halbrook DOB: 5/23/06. Jacob disclosed that he witnessed his former foster father Jose Borbon in the same room with Sharra while she was dressing. Sharra was observed dressed in her underwear only. Jacob observed this incident as he passed, walking down the hall. This incident occurred approximately 2-3 months

The reporting party (RP) stated her son Da' Rell Jones DOB: 10/12/85 has lived in the facility for approximately 13 years. The RP stated on a visit she observed that her son's right eyebrow was swollen and escorted her son outside of the home. The RP stated her son is non-verbal but understands and is able to shake his head yes and no. The RP asked her son if someone had hit him and he nodded yes. The RP stated she named off the names of the caregivers and when she came to a caregiver named Chips, her son identified the caregiver by nodding no. CCIB Intake received a call from Patricia Parker the anonymous complainant who disclosed that this morning while smoking a cigarette staff person Stephanie (last name unknown) called her a "bitch." The reporting party disclosed the administrator Kendall discloses her personal/confidential conversations with other staff persons causing the staff persons to respond to her differently than the other residents. On 9-17-17, the unknown male child's grandmother Faye; asked me to look at the child back. The child's left shoulder blade had a scar. The scar appeared to like someone used their two middle fingers and dug into his skin. The child was bleeding and scared. The child's grandfather (unknown) did this allegation. Faye called the child to come to her. The child was afraid to come to the grandmother. Faye stated told the child "I am not going to do anything to you." The child finally came to Faye. The child did not seek medical attention. The child's mother *(Santana) was not at home. It is unknown if Santana knows about this incident. A report was received on 06/05/2017 alleging that the mother (Hermionne) left Ashante (C-V 17) with a non-relative since 02/2017 without any legal rights. According to the report, the mother refuses to take her child back home and will not engage with Ms. Aarons (caretaker) to provide legal documentation for the child to be enroll in school and taken to a Primary Care Physician. Ashante has not been is school for the past 4 months and are unable to enroll in school without paperwork. According to the report, Ashante self mutilates her arms, and the mother refuses to get counseling. The report indicates Ashante was to follow-up with a Cardiologist for a chest pain and the mother refuses to take her to the doctor.

EREPORT # 18573 statedthe following: The incident took place on today. DFCS worker accompanied Investigation Terry Smith to 317 West Main St, Sardis, MS due to reports coming to the Sheriffs Department alleging that Dakota, his girlfriend Kynli, and Dalton are staying in a home alone with no adults. The concerns individual also stated to Invest. Smith that the children were not in school. Kynli is pregnant and will be having her baby within the next month. After going to the home Invest. Smith and DFCS worker were able to assess that the allegations were true and the home was filthy and no adults were in the home and the children were not going to school. Per Reporter: Misty has custody of Colena, but gave her to Thomas at the age of 11. Misty doesn’t care for Colena. Misty lives down the street from the family’s home in an apartment. Colena primarily lives with Thomas and Carl (paternal grandfather). Colena previously reported to Thomas that Misty’s boyfriend (Damien) touched her; no further details known. Thomas did not report the allegations; however, since then Colena doesn’t care to be at Misty’s home. Misty is at work the majority of the time. Thomas is at work most of the time and when he’s not he’s elsewhere. Colena is left home alone the majority of the time with Carl who is legally blind. CCIB received a call from reporting party (RP) resident Jillian Gilbert, (626) 342-4818 stating that on 8/3/16 at approximately 5 or 6PM, she contacted local Sheriff Department (LE) because the facility placed her personal belonging on the curb. RP stated that LE told staff to bring her items back into the facility. RP also mentioned that the facility would place her belongings into storage, but the facility placed her belongings in the living room. RP stated that resident Bill relayed the message from staff to remove her belongings from the living room but storage has not been provided at this time.

Reporting party (RP) stated that mother Guen Mencias (818) 693-0534 contacted their office today to report that child Xyza Evangelista (DOB: 12/30/13) had a hand print on her bottom. When mother asked licensee about the injury, licensee stated that she was unaware of any injury because she was on vacation and there were 2 staff members left with the children in her absence. RP also stated that on 3 separate occasions, the licensee failed to pick up her son from school (RP did not have the name of the male child). The licensee told RP that she thought her daughter was going to pick up the child up from school (please not that it is not clear if the licensee's daughter or the mother's daughter failed to pick the child up from school). EREPORT # 19130 stated the following: The child is at Diamond Grove Center. Mya reported that 7 years ago her cousin Charron raped her when she was 8 years old. They stayed overnight with their relatives and Charron took off her clothes while they all slept in the same bed. Onautyca, Deja, her, and Charron slept in the big bed together. Mya reports thst she told her mother, but her mother didnt do anything about it. Mya reports that she sees Charron often at church or the weekends, and her mother continues to take her and her sisters around him. The reporting party (RP) stated referral #1143-3655-1102-7078895 was generated from the following referral #0534-9062-5752-307279 dated 1/28/15 regarding resident Nicole Morris age 14 DOB: 6/1/00. Per the referral Nicole is an alleged victim of sexual abuse, physical abuse and general neglect by an unknown perpetrator. According to the RP on 1/28/15 a staff member named Rebecca took Nicole to Long Beach Memorial Hospital clinic to be examined after she disclosed having suicidal ideation. Nicole revealed to hospital staff that she planned to hang herself. While waiting at the hospital Nicole disclosed being "fingered" by two different boys (names unknown) at the home. Nicole also disclosed that a girl (name not provided) beat her up. Referral 0290-1132-4581-1002176 dated 3/13/15 reported by Tatiana Garcia, Clinician with Bayfront Youth Group home.

CCIB received a telephone call with an allegation against the facility. RP called to report that there is drug use and domestic violence happening in home due to the licensee runs a daycare. RP reported that on 10/9/2017 Veronica and Juan went to pick up children from school and Veronica asked staff at the school to call the police. RP reported that staff told her to contact the police to report what is happening. The RP reported that the police came to the school and arrested Juan in the parking lot. RP stated that it was stated on the police report that Veronica was hit in the face and kicked (unknown when incident happened). RP reported that the police report stated Veronica had a cut lip and red marks on her legs. RP also mentioned on 10/10/2017
