CCIB Intake received a SOC 341 from reporting party (RP) Melanie Willis regarding and incident with residents Greg Ekdahl and Paige Katler. According to the RP on 1/26/17 at 9AM while being transported via van to Greg's day program (Escuela Del Rio Day Program) a peer sitting behind him was screaming and singing curse words while listen to music on the radio. The RP asked the peer to stop and Greg asked the peer to stop, in addition Greg turned off the radio. The peer (Paige) struck Greg twice on his left shoulder with an open hand in reaction to his turning off the radio stating " he turned off the radio, he deserved that." Greg was examined for injuries; none were found. He was later left at his day

The reporting party (RP) stated resident Arev Dabbagh DOB: 12/27/00 disclosed on 4/15/16 she inappropriately touched by a male peer. According to the Arev on 4/14/16 a peer Ibraheem Mamoori DOB: 9/2/98) placed his hand on her shoulder and continued downwards towards her breast. The peer touched her breast over he clothing. The resident was distraught while she recounted the incident. Arev alluded to their being other incidents with the peer and other unknown males. Arev didn't disclose details or clarification. Arev was placed voluntarily in the facility by her mother and will continue her placement for a few more weeks. The peer is currently placed on a 5150 hold and will not have access to Arev in the future.

CCIB Intake received referral number 0560-8525-1774-8086170 from reporting party Amy Rodriguez mother of Penelope Rodriguez on 4/2/16. The reporting party believes the black eye to have occurred from physical abuse do to "personal experience." She believes the black resulted from being hit or by being hit with a toy. The reporting party brought Penelope to a doctor (date unknown) due to the child's hair loss. The doctor suspects the child has Alopecia, however the doctor is not certain about the diagnosis. It was presumed the child's hair was falling out due stress. The doctor advised the child be seen by a specialist to confirm the diagnosis.

Reporting party (RP) called CCIB to cross report that resident Gerald Gilliland (DOB: 4/22/41) reported that while he resided at this facility, unknown male staff locked him in a room against his will. It is unclear what room the resident was locked in because resident was unable to convey any details about the room. Resident then stated that it was all of the nursing staff who locked him in the room. Resident was hospitalized at Kaiser South Sacramento on 9/18/16 through 9/22/16 and is currently residing at Eskaton Greenhaven. RP stated that it is unclear if the resident has Alzheimer's or not. RP stated that the original reporting party is Sacramento Sheriff's Department (Report#16-286176) and RP will forward the SOC 341 to CCIB.

On June 20, 2014, the Department obtained emergency custody through C&P of Ryan due to neglect by mother Heather Martin. Mother was arrested by Burlington Police on 6/16/14 for larceny; shoplifting and three counts of receiving stolen property. Heather was taken to at MCI Framingham. The family was placed in a hotel through Department of Transitional Assistance. It was against the hotel policy for children to be left in the hotel unsupervised, and Ryan and Bria were staying alone in the room for five days. This was deemed concerning for the two children to not have an appropriate caregiver. When the Department was made aware of the situation father Brian Scopa 's whereabouts were unknown to the Department. Family members were contacted that could possibly take the children in. However, uncle Stephen Martin, aunt Cindy Fiore and Grandmother Carroll Clinton were unable to do so for various reasons. Given that there were no family members to take Ryan and Bria in, the Department placed them in a temporary Hotline foster care placement from the 6/20/14 to 6/23/14 until a more suitable living arrangement was made available. On 6/23/14 Ryan and his sister were placed in a kinship home of a family friend.

CCIB Intake received a referral #0849-8183-9064-4018185 RE: victim Corey Montjau Toney DOB: 10/23/97 DOP: Unknown SCHOOL: Oak Hills High 7625 Cataba Rd. Oak Hills CA 92344

A report was received on 06/05/2017 alleging that the mother (Hermionne) left Ashante (C-V 17) with a non-relative since 02/2017 without any legal rights. According to the report, the mother refuses to take her child back home and will not engage with Ms. Aarons (caretaker) to provide legal documentation for the child to be enroll in school and taken to a Primary Care Physician. Ashante has not been is school for the past 4 months and are unable to enroll in school without paperwork. According to the report, Ashante self mutilates her arms, and the mother refuses to get counseling. The report indicates Ashante was to follow-up with a Cardiologist for a chest pain and the mother refuses to take her to the doctor.

CCIB Intake received a call from owner/administrator Therese Kragness who disclosed that the local Law Enforcement was present at the facility. According to the caller, an employee who was having difficulty with her boyfriend received a text threatening to come to the facility. Fearing for her safety, the employee notified Law Enforcement and was escorted to the Police Station to file a restraining order against the boyfriend. Conversely the boyfriend did not appear at the facility. The Administrator will fax the hard copy of the report identifying the employee within the required time

Alejandro C. is a 36-month-old Hispanic male who was placed in foster care after being taken from his mother, Ms. C. Ms. C’s family had anonymously called the Department of Children and Family Services (DCFS) reporting allegations of severe neglect. The family stated that Ms. C., a methamphetamine addict, would leave Alejandro alone for long periods of time without any supervision.

CCIB received SOC 341 reporting that during a meeting with the resident, James Brown (DOB: 09/18/84), social worker from Solano Adult Day Healthcare Center, the resident stated that staff Marco has hit him on multiple occasions. The resident kept repeating that he did not want to return to the facility. RP consulted with resident's previous social worker who affirmed that the resident has a history of making allegations that are not true due. Resident also has a history of Schizophrenia/Delusions. Facility staff stated that the resident has a history of making allegations whenever he is assigned a new social

The RP stated Nicole revealed that Dayjiontay put his hands in her pants and touched her pubic hairs. The RP stated Dayjiontay was not outside with Nicole and based on information received from staff Nicole and Dayjiontay were not in contact with one another. The RP stated she made the report due to Nicole's allegation. Additionally referral #1148-5502-1861-10383333 dated 4/24/15 is also associated with the facility. On 4/24/15 Nolan Scaduto age 15 DOB: 1/27/15 disclosed to a Bayfront youth counselor Jamar Harbin that resident Dayjiontay George for him to have sex with him sometime in late 2014. Nolan reported the incident happened in a bathroom stall at the Bayfront facility when staff member Named Mike Taylor enter the stall and said "what are you two doing in there?" It is unclear if the staff member did anything further. Per the RP Dayjiontay George was a bully and was

Reporter stated the following: He is in DHS custody. He (Issac) came to me on October 20th. He (Issac) was in foster care. He (Issac) told me this the other day and it was about 2 or 3 days ago, “I guess”. I have never reported it. I called today because Ms. Judith with MYPAC came in my home today and told me to make a report. Per intake worker (Gail) was on the phone also. Per Ms. Judith I was his (Issac) therapist with Youth Villages.

Javon was placed in therapeutic foster by order of the court on October 2, 2015. His previous guardian, Ms. Terrell had sought psychological testing for Javon on numerous occasions, but never felt she was able to access the community resources and after-care appropriate help for Javon’s diagnoses. She reports not being able to obtain SSI benefits for Javon that would allow for additional treatment and supervision in the community. There is no prior involvement with the Department of Social Services or Child Protective Services.

CCIB received a telephone call with an allegation against the facility. RP called to report that there is drug use and domestic violence happening in home due to the licensee runs a daycare. RP reported that on 10/9/2017 Veronica and Juan went to pick up children from school and Veronica asked staff at the school to call the police. RP reported that staff told her to contact the police to report what is happening. The RP reported that the police came to the school and arrested Juan in the parking lot. RP stated that it was stated on the police report that Veronica was hit in the face and kicked (unknown when incident happened). RP reported that the police report stated Veronica had a cut lip and red marks on her legs. RP also mentioned on 10/10/2017
