Circumstance: Ms. Smalls (CSP) made face to face contact with Ms. Smith (MHS) and Breanna for initial visit.
Action: MHP introduced herself to Breanna. MHP explained the program, visits and addressed the youth’s concerns. Breanna only concern at the initial visit was returning home to great grandmother and not attending school. Breanna disclosed information of threats of self-harm and cutting her left fore arm. MHP email Breanna DSS worker for more information and confirm the youth information. MHP comfort the youth about school
Plan: CSP will continue to ensure Breanna well-being and safety as she gets adjusted to the home.
Focus: Ms. Smalls (MHP) administered the Casey Life skills assessment, the Circle of support and the Child and Adolescent Needs and Strengths assessment with the assistance of Ms. Givens (MHS) and Antazia.
UCM:CPSW received a text from Morgan (Fits) reporting that she has received the referral from Amanda and that she is available on Monday's and Friday's at 11-1pm. Amanda will start on 1/28/17 at 11am Brookdale location. Also, CPSW called foster parent and Amanda about Monday appointment and that Morgan will contact them regarding supervisied visit. Goal 1-2
Intervention: Ms. Smalls (MHP), Santonias and Ms. Givens (MHS) discussed Santonias complying with the rules of the home. MHP and Santonias had a discussing about his NFL football team as a way to build rapport. MHP requests feedback on possible goals from Santonias and MHS for Individual Plan of Care. MHP, Santonias and MHS discuss and process an incident that happened at the school’s gym.
On this date worker visited Shadescrest NH, for the purpose of monitoring Ms. Salter's current placement and delivering Christmas presents. When worker arrived, Ms. Salter was lying in bed watching TV. Ms. Salter was happy to see worker and excited about her Christmas presents. She informed worker that her sister was checking on having her transferred to the Cordova NH because they did not like the Administer, Chester at Shadescrest. Ms. Salter stated she had begun to experience small seizure due to the stress of Kathy Harville, her ex-significant other and Chester. A, B) MSTT met with the caregiver to examine how she plans to increase the supervision and monitoring of the youth while out in public. Caregiver expressed while the youth is at home she is going to speak with her mother and his aunt to help while she's at work. MSTT encouraged caregiver to reach out to the school and ask them if they would be able to supervise and monitor him. Caregiver contacted the school and is waiting to hear a response. MSTT will continue to work with the caregiver in increasing the supervision
Prior to meeting for the Problem Identification, Stage 1, Ms. Bellot and I spoke on the phone and I briefly explained what consultation and the collaboration efforts that would be needed in the process. She agreed to participate in the consultation process and she was asked to complete a Social History form to gather additional background information on Brianna. Ms. Bellot understood the questions and did not omit any answers.
Discussion: Ms. Roger reported that Corey's Medicaid hasn't been reinstated, as a result, his therapy and all medical appointment is on hold. She stated that Corey has enough medication and is not at risk of running out. Ms. Roger expressed concern with Corey's Maladaptive behavior (substance Abuse), truancy and medication refusal. She asserted that Corey's addiction to this substance is affecting him significantly. Corey denied it, purported that he's not addicted to it but uses it as a coping mechanism. Corey stated that he's been absent multiple times from school because he's lethargic in the AM. Ms. Rogers indicated that his Maladaptive behavior is contributing to it. Ms (Family growth) informed Corey about their Substance abuse program

CM spoke to Kiara Gelin (youth) regarding a follow-up on services. Youth reported she continues to attend the Partial Hospitalization program at the Jersey City Medical Center. CM was informed that youth stopped attending the TASC program due to time conflict and youth will resume the program in September. CM and youth disused employment; youth had a job interview at Wendy’s, Fast food restaurant. CM encouraged youth to apply at local jobs in the mall, CM verbally provided youth with a list of jobs that are hiring in the community. CM inquired about the Cook, Eat and Talk program. Youth reported the family will follow-up with the program once the family phone is working. CM agreed. CM and youth discussed mentor
Clinician provided support to youth during this transitional period. Clinician assisted the youth with positive ways to communicate with MHS and foster sister in the home. Clinician encouraged the youth to get involved once enroll into the summer enrichment program and community. Clinician will assist the DSS worker and MHS
UCM: CPSW did a home visit to Ms. Brenda Borkovec's home. Both children were in the living room playing together. Also, Ms. Brenda was in the living room holding Aydden and feeding him. Ms. Brenda stated that she has not received any papers regarding her foster care license. Ms. Brenda stated that she is frustrated with the process and that it has taken a longer time. Ms. Brenda mentioned that it has taken a long time for her to complete her fingerprint. She reported that currently she is waiting the background study and fingerprint forms to be sent to her. Ms. Borkovec's phone ring and it was Elizabeth Borkovec. Ms. Brenda asked CPSW if Elizabeth can come and join the meeting. CPSW approved it and Elizabeth Borkovec showed up less than 5 minutes.
There has been a delay in completing this wellness check. Ms. Bell has been out of the State of Michigan for the winter. Coordination of the visit was determined by her return to Michigan. This is a onetime visit that per attorney Mr. Goodwin needed to be coordinated with the prior nurse case manager Joyce Lucas from Wellspring Case Management.
