Patient K., 58 y/o, complains of weakness, muscle ache, loss of appetite, nausea, vomiting, aching bones, deterioration of memory and cramps. In anamnesis there is a record of the ulcerous disease of stomach, frequent pathological bone fractures. Objectively: the consciousness is clouded, the skin is dry of ashy gray colour, present deformity of the vertebrae bodies, "goose-stepping" gait, X-ray shows systemic osteoporosis. Heart sounds are dull, rhythmical, arterial pressure – 160/100, pulse – 56beats/minute. The level of calcium in the blood – 3.9mmol/l; hypophosphatemia, hyperphophaturia; glycemia – 4.8mmol/l. What are you going to do with this patient?A. Introducing 2-3 l of 0.9 % solution of sodium chloride and the potassium phosphate and sodiumB. Introducing 150ml of 4.5 % solution of sodium chloride, 100mg prednisolone C. Introducing 100mg prednisolone, 40g furosemide D. Introducing 1-2ml 0.5% solution of strophanthin, 100mg prednisoloneE. Mr. H is a 52-year-old male who presents to the emergency department. His left leg is in a cast, and he states that 1 week ago he was in an automobile crash and broke his upper leg. Since that time, he has had difficulty “getting around” and has mostly been lying on the couch watching television. On the evening of admission he noticed a sudden onset of dyspnea and chest pain. He denies having orthopnea, cough, hemoptysis, or wheezing. He smoked two packs of cigarettes a day for 19 years but quit 3 years ago.  The ABG analysis of Mr. H suggests uncompensated respiratory alkalosis with mild hypoxemia, with base excess of -1 in her arterial side, whereas -4 in her venous side. Part 1: Her actual arterial-venous oxygen content difference (Ca-vO2) is 5.31 mL/dL. (Normal range considered here is 3.5 to 5 mL/dL) Part 2: Patient's actual oxygen extraction ratio (O2ER) was 29%. (Say normal range is 20-28%) What is clinically happening to the patient?
J.J., age 52, presents to the E.R. after developing acute flank pain within the past 24 hours that she states “comes and goes.” She reports some discomfort with urination, which she describes as “spasms when I pee.” Laboratory tests show a small amount of hematuria and a slight increase in WBC in her blood and urine. Her pain rating was a 9 on a scale from 1 to 10 (with 10 being the highest). Select a potential diagnosis for J.J. and describe the pathophysiology of that diagnosis. How does the pathophysiology explain J.J.’s symptoms and lab results?
