Any alterations are recorded and reviews and changes are highlighted to the team.
There are always key elements to assessments and reviews, including the family and friends. Everyone has a responsibility to support individuals and bearing in mind ‘need to know’ information. The aim being able to achieve the highest goal to maintain effective open channels for everyone.
In my role partnerships with:
• Family and friends • Care Staff • GP’s • Nutritionalists • Speech Therapists • Social Workers • Physiotherapists • Consultants • Other Nursing Home Managers • Central Office
All the above are people I may work with in order to gain information, which supports me in my role in making the best decisions and judgments’.
Everyone needs educating and supporting including myself when looking at the individuals preferred method of communication, and everyone in the above list can give valid input in assessing for example swallowing by the speech therapists. This can include communication with the RN, GP for referral, to speaking with the catering chef and carers. By doing this with effective communication and documenting accurately we can ensure that everyone’s input is maximized and as well as being written and passed on verbally it is put into practice. This results in the clients personalized care needs being met

During an initial assessment an individual’s ability and communication methods are established. This is done when an individual arrives into care. Everyone involved in the care of this service user is made aware of their needs and preferences regarding communication and any changes are recognised during reviews and shared with the team to ensure the individual’s needs are met.

There are various types of communication in today’s world where technology is embedded and manifest in all of them without us feeling it as a pervasive and hankering presence. Most used among these are verbal, non-verbal and visual communication. Verbal communication refers to the spoken word. The information is transmitted through a verbal medium like words, speeches, presentations and more. For instance, in educational institutions and professional organizations, individuals communicate verbally with each other through dialogues, lecture, presentations, discussions and even conversation. The clarity of the message conveyed cannot be achieved unless the message is well-spoken and concise, unambiguous and definite. Slowly and gradually technology is pervading and conquering our daily lives. With verbal communication being substituted by technological interface people are being more involved in the virtual and digital world than being present in the real world. According to Emily Drago ( EJSPRING 15) in a study of verbal communication with and without the aid of mobile devices, it was elucidated that the use of technology inherent in these mobile devices affects human relationships during communication. People not using such devices to communicate showed higher levels of empathy and concern and people using such devices showed less of the same quality. Researchers found that conversations in the absence of mobile communication technologies were rated as significantly superior
I felt helpless as I didn’t know how to help Elsie understand what I was communicating and for me to understand Elsie. I felt more anxious when Elsie became tearful because I felt that my inability to effectively communicate with her had caused her to become upset.
