Reporting party (RP) called CCIB to cross report that resident Gerald Gilliland (DOB: 4/22/41) reported that while he resided at this facility, unknown male staff locked him in a room against his will. It is unclear what room the resident was locked in because resident was unable to convey any details about the room. Resident then stated that it was all of the nursing staff who locked him in the room. Resident was hospitalized at Kaiser South Sacramento on 9/18/16 through 9/22/16 and is currently residing at Eskaton Greenhaven. RP stated that it is unclear if the resident has Alzheimer's or not. RP stated that the original reporting party is Sacramento Sheriff's Department (Report#16-286176) and RP will forward the SOC 341 to CCIB.

The reporting party (RP) stated that 92 year old resident Delora Lovelock DOB: 1/15/24 was residing in the facility from 5/15/2015 to 6/4/2015. Prior to admission the facility was informed that the resident had a broken femur and could not bear weight. The resident required two caregivers to transfer. Initially the facility assessed the resident and accepted her into the facility. Subsequently the resident was not provided the care necessary. The resident was transferred by one caregiver that resulted in the resident's trip to the emergency room where she was diagnosed with contusions to her ribs. The RP stated that prior to admission that the caregivers were trained in transferring resident however the caregiver had no idea how to transfer a resident from bed to

The Department of Human Resources received a report on Ms. Sue Salter on 9/17/14 alleging Ms. Salter a paraplegic who is paralyzed on her left side was living in a home with no water, power, and had an inadequate caregiver. During investigation, utilities were reconnected and Hospice service provided. Hospice stated Ms. Salter was losing notable weight and found lying in urine during visits. Due to this, several bedsores increased and the doctor ordered a catheter and low air mattress. Ms. Harville, the caregiver was educated on wound care numerous times and demonstrated a working knowledge but choose not to administer wound care.

MICU 15A was dispatched to 30 West Ave, @ Genesis Healthcare, Wayne Center in the Wayne Business District in Radnor Township, for an ALS Emergency, Assault Victim. The weather conditions were cloudy, cool and dry. MICU 15A responded with care per protocol to the stated location with EMT Straub driving. On arrival, the EMS Crew proceeded to the nurse's station, and the individuals there did not know anything about an incident, there was a female standing in one of the hallways who advised that there was a person sitting over where she was. The EMS crew proceeded to that area and found a 43-year old female who was a registered nurse and was conscious and alert x4 and was sitting on a chair, the EMS crew asked her what was happening, "she replied that she was in a patient room and stated that the patient had an IV in her arm and became disturbed and picked up a pocketbook and then assaulted her by the swing it and striking her in the right frontal region of the head", the nurse then exited the room and went and sat down in the hallway, where the EMS crew found her.

The reporting party (RP) stated resident Sofia Miramontes age 27 DOB: 11/23/88 has developmental delays, deaf in right ear and is bipolar. The RP stated the resident was issued a government cell phone, however the caregivers take it away from her and refuses to let her charge it. On 6/13/16 the police were called to the facility due to resident's inability to enter the facility. The North Hollywood Police Officers Winter (#38878) and Officer Radke (#39569) arrived to the home. The Officers removed the resident due to her being battered and concern for her safety. The Officer informed the Director of agency Sherry Winston that the resident was coherent and walking to her boyfriend’s home. The RP contacted the facility and spoke with a staff

The reporting party (RP) stated she held a telephone interview with Aurora Drake DOB: 9/1/98. The RP stated Aurora disclosed shortly after her placement in the facility on 9/24/14 she observed the residents smoking in the back yard of the home. According to the RP Aurora began smoking shortly after the encounter. Aurora stated a staff person named Kate would supply her with cigarettes. Aurora stated she observed a staff person named Shawna share the same cigarette with a resident named Austin. The RP stated Aurora was constantly belittled and harassed. The RP asked Aurora of incidents of her interactions with staff. Aurora stated around last Christmas after returning from a visit with her mother, staff person Kate called her a "spoiled little bitch.' The

It was reported that when staff Elizabeth Gomez of UCP-Work day program (Facility #425800283) arrived to Sloan Terrace, she noticed 3 bruises on client Thomas Pankratz's (DOB: 04/09/71) upper left arm. Two of the bruises were the size of a quarter and the other was a size of an egg. The bruises were yellowish and green in color. Staff asked the client what happened and the client stated that he was horse playing and a staff member (unknown) from Novelles Day Program hit him in the arm. No additional details were

Patient was here with cousins to be seen. This occurred at end of the visit approximately 1:15 pm. I was called by Madelyn Ramirez (PCT) that there was an incident in the exam room. I spoke to the mother, she said he was fine that he did not hit his head. No injuries noted, mother decline being evaluate by provider. But, patient was evaluated by Dr. Ritchin after fall.

The reporting party (RP) stated on 11/10/16 the biological mother of David Jiron DOB: 3/6/11 reported he was transported to his visit with her on 11/8/16 without a booster car seat by social worker Liz Cardenas. The mother also stated the social worker grabbed David's arm and hand to prevent him from swinging a rake. Attached is an email from the social worker explaining the reason for not providing the booster car seat. The social worker stated due to the other two car seat there was not enough room for the booster seat therefore the child was secured with a seat

The reporting party (RP) stated minor Anthony Silver DOB: 6/19/99 resided in the facility until when he was given notice on 10/4/16. According to the RP, Anthony went AWOL from the facility on that date. While being transported into custody to Juvenile Hall he disclosed that staff members brought drugs and alcohol into the facility. The RP stated she spoke with the minor privately regarding his allegation and disclosed several staff members were involved in bring in pills, tobacco, alcohol, and meth into the facility. When the minor was asked which staff members, he replied he didn't know and could not remember. Consequently the RP continued to speak with the minor and he revealed the names of Gary Stanley, who no longer works in the facility.

The possible physical abuse section includes the assessment of bruising that appeared on several places on the patient’s body including right periorbital area and right maxilla along with swelling, redness and warmth. There was

Shockley’s oral report, Mr. Wood related the occurrence to Dee Showalter, our client Human Resources Director, who in turn reported the matter to Mary Miller, our client’s Chief of Living and Learning. Ms. Showalter also contacted our client’s nurse, Janice Young, and requested that she perform a skin assessment on Allison and also arrange for a physician to examine her per Ms. Miller’s request. A 32 year old woman was admitted to the Trauma Intensive Care Unit following a motor vehicle accident; she had multiple injuries and fractures, with several complications which continued to develop over the first couple of weeks. The patient rapidly developed Adult Respiratory Distress Syndrome, was on a ventilator, and was continuously sedated. Shortly after the patient's admission, her parents were contacted and remained vigilant at her bedside. The parents reported that the patient was one month away from having her divorce finalized. The patient's husband was reportedly physically and emotionally abusive to her
