Intervention: Ms. Smalls (MHP), Mrs. Clark (MHS) and Taliana discussed Taliana adjusting to the home. MHP encouraged Taliana to ask questions if she does not understand something. MHP praised for willing to communicate with MHS and family. MHP, MHS and Taliana discussed Taliana’s emotions about family visits and no contact with biological mother. MHP praised Taliana expresses her feelings. MHP explained to Taliana and MHS that the DSS case worker will visit next week. MHP provide an update from DSS worker about clothing and eyeglasses. MHP administrated the Casey Life skills. MHP allowed Taliana to share stories about her biological mother and family.
Action: MHP and MHS discuss Donovan’s behaviors in the home and community. MHP and MHS review monthly paperwork that is needs to be submitted. MHS and MHP complete a walk-through of Donovan’s bedroom and play room. MHP and MHP discuss upcoming medical appointments. MHP email Ms. Greene at the MUSC Foster Care Support Clinic to assist MHS with verifying upcoming medical appointments. MHP and MHS discuss MHS contact with Donovan’s DSS worker.

Social Services Meeting: On 11/21/2016, Ms. Medina and her children met with her assigned Case Manager for the family ILP Document Review. Ms. Medina is expected to meet with assigned Case Manage bi-weekly. Ms. Medina’s next ILP Document Review is on 12/05/2016. Case Manager encouraged Ms. Medina to continue attending meetings. Ms. Medina stated that she was a sad and upset, due to that her doctor informed her that she is not going to be able to work as a Home Health Aid due to a back injury. Ms. Medina stated that her doctor recommended that she would benefit from physical therapy and that she should avoid to lift anything heavy until further notice.

Social Services Meeting: On 03/01/2017, Ms. Hawkins and her daughter Emoni met with her assigned Case Manager for the family ILP Document Review. Ms. Hawkins’ next ILP Document Review appointment is on 03/14/2017. Ms. Hawkins is in-compliance with the terms of her ILP. Ms. Hawkins was reminded that she is expected to attend all scheduled meetings with assigned Case Manager and failed to do it would considered non-compliance and warning will be issued. Ms. Hawkins stated that she is aware. Case Manager asked Ms. Hawkins if there are any issues or concerns that she will like to discuss, Ms. Hawkins stated no.
Action: CSP and MHS discussed changes in daily schedule because of Ayden’s surgery and foster brother’s surgery. Ayden has medical appointments during the week foster brother’s plan recovery from heart surgery. Ayden has upcoming appointments to replace his feeding tube, pulmonologist and pre-op for ear surgery on 11-23-15. CSP encouraged the MHS to continue the great work getting the children to all medical and DSS appointments. There are no family visits scheduled with bio-parents. Ayden continues to respond to the school staff and his motor skills are improving.
Case management services were provided in garden Grove. Present at the meeting were WYP (Wraparound Youth Partner) Andy Ngo. Service minutes are high due to WYP Ngo attempting to meet the youth. WYP Ngo reminded the youth of the meeting during the after noon through a phone call. The youth acknowledged the meeting and will be waiting for WYP Ngo. WYP Ngo arrived at the youth’s home. WYP Ngo contacted the youth through a phone call, but the youth hung up on WYP Ngo. WYP Ngo waited for the youth to show up. WYP Ngo contacted the youth again, but the youth did not pick up. WYP Ngo contacted WCC (Wraparound Care Coordinator), WPP (Wraparound Parent Partner) Lyneth Torres, and SUP (Wraparound Supervisor) Sheila Gaston-Cruz to inform them the youth
UCM: CPSW did a home visit again and talked with Annette regarding her relationship with her mother Ms. Clifton. Annette stated that some times that they both disagrees on things and that and than Annette gets upset with her mother. CPSW talked about therapy with Annette and Ms. Clifton. Annette looked at her mother when this therapy talked about family therapy with her mother Ms. Clifton. Ms. Clifton refused to participate with Annette any family therapy. Ms. Clifton stated that Annette has anger issues and that they don't need family therapy at this time. Annette was very quite and mostly was looking her mother's responses regarding family therapy. She would like to see what her new residence looks like and meet the people. She is glad that she can continue to call and check on her. She tries her best to speak to her everyday, mostly after program. She would also like to get her hair done Thursday. She wants to try something different, but she is unsure of what she would to do. As well as get her nails done this weekend. She stated that she will be getting a new library from her Day Program. She is excited about it and heard that she can use it as identification. Maria stated that things are going well between her boyfriend and her. This makes her happy and she hopes that it remains like this. Goal 2: Kayla will comply with the treatment recommendations of her medical and mental health providers. The director informed the QP, "I have completed the paperwork to refer her and I am waiting on the foster mother's portion of the paperwork." The director said, "I have asked her for the paperwork because it's just basic information, but she stated the social worker has to complete it."
Clinician provided support to youth during this transitional period. Clinician assisted the youth with positive ways to communicate with MHS and foster sister in the home. Clinician encouraged the youth to get involved once enroll into the summer enrichment program and community. Clinician will assist the DSS worker and MHS
Collateral interview with Ms. Jennifer Small-Felix/Child Advocate and therapist was conducted via phone to discuss child’s behavior and follow up in reference to status of her medication. Child’s advocate was informed of change in child’s behavior reported by the foster mom.
I think you did a great job summing up your opinions and also making them easy to understand at the same time. I agree with you that the relationship between both the victim and the offender is important when trying to understand what went horribly wrong and also what led to the murder suicide situation. I also agree that the fatality review team approach should hold offendersaccountable for their actions as well. However I do not agree with you as far as your opinion on officers not being in danger when responding to domestic violence calls. The third priority issue the social will address is getting assistance to Maria with her son Marco and employment. Marco was diagnosed with autism at age 4 and now is no longer in school having Maria “overwhelmed.” Maria has lost her job and her medical insurance. The first thing will be getting Marco evaluated by a doctor to make the decision of what care is appropriate for him. Having the communication with Marco primary doctor will be important. Maria stating she does not know “what to do with him” confirms her unknown knowledge of the resources in her community that can be a help for her.
