In order to provide effective care to a community a nurse must understand the complex interacting factors within a community, such as social determinants of health, which affect the health of clients (Yiu, 2015). Community assessments are one important way nurses can promote effective community health strategies, encourage public participation and develop community action plans (Lotfi, 2014). There are four components that make up a community assessment; environmental scan, needs assessment, problem investigation and resource evaluation (Yiu, 2015). Rock Bay is considered part of the downtown Victoria core. The boundaries of which run along the upper harbour waterfront to Bay Street along Dowler Place down Blanchard Street to Chatham Street back to the waterfront (City of Victoria, 2011b). Rock Bay takes its name from the inlet of water that was once the outfall of a creek drawing from the Oaklands area (D’ambrosio architecture, urbanism and New landmarks, 2011).

The data collected from the Census and National Household Survey from 2011 showed the population of Burnside Gorge neighborhood, including Rock Bay, is 5860. Furthermore, this industrial and employment area, including the Burnside-Gorge community is home to a diverse mix of nearly 1200 employers. Some developers may wish to gentrify the area, realizing the potential for luxury housing close to the water and downtown core. The gentrification process would mean creating housing for the upper and middle class citizens while displacing low-income residents (City of Victoria, 2014). Another possibility for Rock Bay is that the First Nations community would buy the land after remediation and partner with developers to establish a commercial industry (Wilson & Dedyna, 2015). Additionally, the local government has proposed Rock Bay as one of the potential sites for a sewage treatment center in Victoria (Wilson & Dedyna, 2015). The suburb of Pyrmont on the shores of Sydney Harbour has been transformed by the processes of urban renewal into a thriving cosmopolitan residential area, an efficient and sophisticated business centre, and a popular recreational and tourist hub. Through my own observation of the Pyrmont area, I have seen how the painstaking urban planning efforts for the area have come to fruition, and a focal point of the Harbour foreshore created as a result of this.

A community health assessment is a fundamental instrument of public health practice. Its objective is to depict the health of the community, by presenting information on health standing, community health needs, resources, and epidemiologic and other studies of present local health problems. It seeks to recognize target populations that may be at augmented risk of poor health results and to increase a better understanding of their needs, as well as evaluates the larger community surroundings and how it relate to the health of people. It also identifies those areas where better information is desired, particularly information on health differences amid different subpopulations, quality of health care, and the incidence and severity of disabilities in the population. The Community Health Assessment is the foundation for all local public health development, giving the local health component the instance to recognize and network with key community leaders, businesses and concerned residents about health priorities and concerns. This information shapes the foundation of improving the health status of the community by way of a strategic plan (The Municipal Public Health Services Plan Community Health Assessment Guidance and Format, n.d.).
Community health nursing is a vocation; striving to improve the health of the community by promoting, stabilizing and protecting the health of individuals, families and identified aggregates. We are able to have the furthest reach when we can isolate a specific deficit of an aggregate through assessment and subsequently the development of a community support program. Community support programs promote health, access to services and self-advocacy.

Population health as a framework for examining health is not a new concept. Historically, epidemiologist have used this to formulate etiologies of disease by measuring variations within a population and the impact of environmental factors (Radzyminski, 2007). Governments have also applied this concept to the implementation of interventions geared at improving the health of nations (Radzyminski, 2007). Although the concept is not new, the term population health has only been recently defined and differentiated from the definitions of community health, public health, and population-focused care.
