The members of the interdisciplinary team are the physiatrist, occupational therapist, physical therapist, psychologist, social worker and rehabilitation nurse, and more. The physiatrist is a physical medicine and rehabilitation physician. The occupational therapist is the therapist that helps the patient with daily living actives, upper extremity movement and function including fine motor skills. The physical therapist is the member of the interdisciplinary team who is responsible for improving the patient’s strength, range of motion, balance and mobility. The psychologist and social worker both help to work with a newly disable person. The psychologist helps the patient in adjusting to the new disability and communication with the rehabilitation team. The social worker will help in providing background information on the patient and helping the patient have a smooth transition back home. I have worked for the West Virginia Division of Rehabilitation Services (WVDRS) for over three years as a Rehabilitation Service Associate. During that time I have been given the opportunity to work with the clients that we serve on a regular basis. One of the first patients that I have met in the Fellowship Community rehabilitation care was an elderly gentleman with a post polio syndrome diagnosis that had seriously affected his functionality in early nineties. Lately, he was submitted to the short-term rehab unit due to the infection and was deconditioned by the bed rest and antibiotic treatment. The therapy goal was to return him to the previous level of the independence and function with the use of his motorized wheelchair. However, goals were not met in the certain time frame and the patient's family was complaining of prolonged rehabilitation and stated that all they need for him is to return home.

Elderly people often begin to lose their physical ability to do certain tasks, so there are occupational therapists working in nursing homes or providing in home care. Many athletes suffer sports injuries that cause them to lose their ability to do daily activities, and occupational therapists are available to them in rehabilitation centers and hospitals.
