For Example: When they were at their house if they would read the news paper and drink a cup of coffee then get dressed they should be able to still do their routine like they did before going into a long term care facility. If it’s the client that decides how their day is going to go, it will make them have a good day and they will be happy. For example: If you have a client that always liked to take a relaxing bath around 9-10 at night before going to bed, you should give them a bath around 9-10 and they will be happy and in a good mood. These facilities are usually targeting individuals who are of geriatric age or need around the clock care (mental health and physical health). The residents in these communities have access to individually-tailored levels of individual or group-centered activities, programs, and assistance whether it is with home or personal care. Long term care facilities are generally for those who are unable to manage independently in the community. Specific types of long-term services include nursing homes, hospice care, home health agencies, and residential care facilities. Taking care of the individuals that are getting older takes many different needs. Most of these needs cannot be given from the help of a family. This causes the need of having to put your love one into a home and causing for the worry of how they will be treated. It is important for the family and also the soon to be client to feel at home in their new environment. This has been an issue with the care being provided for each individual, which has lead to the need of making sure individuals have their own health care plan.
There are many forms I use to assess an individual’s needs. The first bit of the information comes from Derby City Council, which is called a outcome based support assessment. This is what they use to identify someone’s needs and how much care they require. The information on this document is great for Derby City to use, but I also need to do my own and adapt it so it’s easier for a care worker to understand as they are the ones who will be doing the care. As a way of helping Client X she was given as much independence as possible to try and make the transition easier. As she became used to the carers coming into the house she soon became glad of the company due to being unable to get out of the house on her own. This then grew an element of trust between the client and the carer which meant that she was then more willing to participate with the carers and would communicate with them on a regular basis.

During the Personal Wellness Assignment, various factors influenced my fitness, nutrition, energy, and sleep numbers. I noticed that my nutrition, energy, and sleep scores were higher on Saturdays and Sundays because I didn’t have school on those days so I had more time to eat and sleep. My energy scores were high on those days because I was happy that I didn’t have school. I also notice that my fitness scores are low most of the time, except on Tuesday. This is because I have an extracurricular activity only on Tuesday. My graph shows that when my sleep scores are high, so are my energy scores. This indicates that I have more energy when I sleep for a longer time. My graph also shows that the nutrition and energy scores are exactly the same.

I agree that valuing the satisfaction of clients and providing, validating information and care contributes to the positive experience of client within health facilities. Because if there is a limited access of care to clients, especially when they were waiting in an emergency or the surgery ward for a long period of time. They become anxious and frustrated as they are people just like us. on top of that the limited information provided to families and patients causes them to become uncertain about the plan of treatment. therefore, perceive that they have no control within the health facility and feel unvalued.
Lily was a 65 year old lady with stage 5 CKD, she had recently begun hemodialysis treatment three times a week as an inpatient and had been responding well to treatment. During dialysis treatment on the morning of the first day, Lily’s observations showed that she was: tachycardic, hypotensive, tachypnoeaic, had an oxygen saturation level of 88% and was becoming confused and drowsy.
