I experience two great learning opportunities, where my preceptor helped me understand how population requires unique care, different from inpatient nursing. The first example was of a mother who brings her disable son to the clinic and was having problems with the visiting nurse not showing up to the appointments on time. The mother explained that she works fulltime, attend school and do not have a home health aide all day. Her son is disable and so is her husband. When she provides the time for the visiting nurse to come to the house, she must be on time and not consistently late. As she is unable to sit around waiting to open the door, as the nurse is constantly late and keeps missing the scheduled time. The now causes the mother to have …show more content…

Prior to starting my practicum, from the readings I learned that the PCHN not only deals with the patient, but families in the community and in their own home environment. I was looking forward to gaining experience in community health nursing. As all my experience prior was from my practicum experience for NURS 400 Leadership and Management in Nursing. All my professional nursing experience is in acute care and I hoped to build my patient interaction skills in a non-acute care setting. My expectation was that I will learn how socioeconomic problems are dealt with on a community level.
Now that I completed my practicum hours, I gain such a better understanding of what community nursing entails. I have more respect for the community nurse because they at times must reach out to so many more people and organizations to get the patient the help they need. Unlike in the acute care setting, help and resources are more easily accessible. I was very pleased that I had many one on one interactions with patients and able to discuss socioeconomic issues they were having. I gain vital knowledge by watching my preceptor deal the examples discussed above. My expectation of my practicum experience prior to starting, matching with the result. I had a very rewarding and knowledgeable experiences and I will be able to apply the interaction skills learned to my acute care
Nurse practitioners are advanced practice registered nurses who have received special courses and training. They usually work closely with doctors and can perform many high-level primary care tasks. They often specialize in specific types of practice such as pediatrics, psychiatry, or obstetrics. Some establish private practices; however, most work in doctors' offices, hospitals, or neighborhood health centers. Their duties often include taking detailed medical histories and performing complete physical exams, providing diagnoses and recommending treatment plans, treating common medical conditions, illnesses, and injuries, prescribing limited medications, and counseling patients and families. 1. This year, my community nursing placement is at Donview Middle School Health and Wellness Academy. It is a fairly large Middle School with over 400 students, 19 classrooms and 29 teachers (N. Vohra, personal communication, September 9, 2014). As the name of the school suggests, Donview focusses on the health and wellness of its students. However, was not always a Health and Wellness Academy, Donview has only recently been changed into a Health and Wellness Academy. The school is located in a quiet residential area of North York with the building itself being fully equipped with lockers, a cafeteria, a gym, a large outdoor field and a fitness studio. The school has been in this location since the 1970’s (V. Bradbury, personal communication, September 16, 2014).
I agree with Walden University’s (2013a) view on social change that “Walden supports social change through the development of principled, knowledgeable, and ethical scholar-practitioners, who are and will become civic and professional role models by advancing the better of society” (para. 4). I plan to incorporate social change in my profession by applying my past work experiences and knowledge from school to become a better professional role model to my patients, co-workers and community. An example of one learning outcome for the Master of Science in Nursing is to “exhibit ongoing commitment to professional development” (Walden University, 2013b, para. 3). I am constantly trying to learn new ways to improve and better my knowledge and education. I recently obtained my Certified Emergency Nurse certification and will soon be taking the Certified Pediatric Emergency Nurse exam. I only hope that with my past experiences and my future knowledge gained at Walden University, I will be able to become a professional advocate and role model for the future of pediatric care and the patient population thereof.
The patient tells me that she is unsure when her last menstrual cycle was. Her periods are very irregular and she had not been keeping track of it. She and her husband have been attempting pregnancy for the last year and a half and she tells me that as of Thursday, August 27th, she and her husband had gone through the procedures for adoption. She said that she knows she ovulated on August 18th. She was using an ovulation kit. She took a home pregnancy test on the 28th, which was positive, repeated with a different brand of test on the 29th and that was also positive. My practicum setting will be an associate degree nursing program, Joliet Junior College (JJC), one of the oldest community colleges in Illinois. The mission of the school is focused on offering students an affordable, accessible valued education for a lifelong profession in nursing, and to serve the community (Joliet Junior College [JJC], n. d.). Accordingly, the school’s philosophy is based on preparing students to be exceptional providers in the growing healthcare system, providing safe and high quality care (JJC, n. d.). The foundation of the JJC nursing program is based on four major principles: client, environment, health and nursing (JJC, n.d), while their conceptual framework is grounded in the fundamental structure of three major theories which forms the basis of the concepts, skills, and principles of the nursing practice (JJC, n. d.). In 1965 the first Nurse Practitioner (NP) training program was created by a physician Henry Silver and a nurse Loretta Ford. There was a shortage of primary care physicians due to increased specializations in medicine. The shortages were very evident in rural areas and the creators of the program realized nurses had the potential to fill the primary care gap within their communities. This NP training was informal and lacked credentials, leading to criticism and mistrust of those providing care. The first master’s program for NP was established by Boston College in 1967.

The national league for nurses defines critical thinking in the nursing process as “a discipline specific, reflective reasoning process that guides a nurse in generating, implementing, and evaluating approaches for dealing with client care and professional concerns” (Kozier, 2008). This definition is imperative to help a nursing student learn how to think in terms of nursing care. Nursing students must achieve a comprehensive understanding of critical thinking in order to understand the nursing process. Over the course of this preceptorship, I have written a few different journals with the nursing process as my topic, the reason for this is because the nursing process is everywhere and has become a part of my day to day life, not just during my different clinicals over these past couple of years, it has spilled over into work, school work assignments, and home life, as well as. I now find myself assessing everything that needs to accomplish throughout my day and developing a plan of action, setting priorities to works towards their completion. Another topic that I feel is very important for a nurse is professionalism and important in developing a trust with the patients in your care, as well as, to earn the respect of your fellow nurses and other members of the hospital staff. The first week of preceptorship went fairly well. It made it a little difficult to have two different nurses, as they both expected things a bit differently. One nurse wanted me to shadow her the first day, and not do much, and the second nurse, which I had the second day asked me to be more hands on next time. If I knew this before the start of the shift I would have done that from the beginning. However, I will just be that much more confident in myself the next shift with that nurse. The second nurse did allow me to do more things throughout the shift, and actually preform an assessment on a patient, which I think helped me feel more comfortable and gave me confidence and experience for the next time.
For this assignment, I was required to interview one of my professors, including questions that were specific to this assignment as well as questions that I wanted answers for. I chose to interview Professor Amy Edmison, my Adult Health 2 professor because I found out during lecture that she, like myself, had a passion for mentoring.
The main focus in nursing school is to care for individuals physically to improve their health in the hospital setting. This view changes in community and public health nursing because the emphasis becomes communities, groups, populations, and aggregates that have certain needs. It becomes an instability for many students, because it is difficult to focus and view communities as clients rather than a single person. Many students do not want to serve communities because they have failed to see that groups of people are clients in desperate need of education, medical guidance, and emotional support.
