Description
The academic year has been very intensive and ran quicker than I imagined, however, it has been a very challenging but exciting both academically and on practice. Lectures have been well organised and delivered compared to year one where on many occasions there were disruptions due to last minute change of lecture theatres or change of timetable. We had equal sessions of field learning and theory at the school.

Feelings
I feel the year two modules have helped shape my perception about nursing practice. Being on placement in a mental health/learning disability hospital in module three and in gastro ward in module four have consolidated my learning and experience about patients with long term condition. I feel more compassionate and more caring for patients and their needs.
I feel that I have learned a lot during the second year of my studies.
I have learned to communicate better with both my colleagues, patients and their families and how to develop therapeutic relationships. I have also improved in academic writing. I feel I have made positive impact on both patients and their families during module three and four placements. I safely practiced the clinical skills acquired during theory and on placement and have developed more confidence in delivering these skills.
Evaluation
As a learning disability nursing student, I have learned about the importance of the nurse’s role in empowerment and health promotion with individuals with learning disability. The modules have helped develop my knowledge and skills on working in partnership with individuals and their families/carers using person-centred approach. making information accessible to them.

Valuing People (Department of Health 2001) it was introduced by the labour government who were keen to promote independent living. This white paper on learning disabilities was for the first in England in 30 years. It made direct payments available to more people with a learning disability and was the first paper where we officially come across the term ‘Person Centred Planning’. It stresses the importance of Personal Centred Planning in helping people with learning difficulties take charge of their own lives. This paper has been ‘refreshed’ in 2009 titling it Value People Now and is a new three year strategy for people with learning disabilities, and will lead to better lives for people.
In looking at this model I have found that many people have views that based on discrimination and prejudice they are embedded in today’s society, the attitudes and the surrounding environment often focuses on what a person lacks in terms of disability and focuses on condition or illness or a person’s lack of ability. To combat this prejudice within making space we use person centred care planning it is my responsibility to promote the use of this and to ensure that the support workers who deliver it are fully trained to do so. Personally, for the duration of the health and social care course I believe that I have changed. This course has given me the additional skills and knowledge in which I can use for the future. My presentational skills especially have improved as before when I had to present to an audience I would become very nervous, however in various units presenting in front of an audience was essential. From the encouragement from both the teachers and my peers I am able to now confidently speak in front of a small audience and be comfortable with my work and present it - I have also used their feedback as a way to improve myself.
People with learning disabilities say that they want to live an ordinary life in their communities. Person centred planning can help Partnership Boards make this happen.
In addition, the course material and modules have allowed me to adjust the various aspects of my professional practice for the best outcomes. In fact, I have applied the course material to adjust my patient care plans with the result that I now appreciate the different treatment and prescriptive therapies, and the legal implications for more awareness and understanding. I am inspired to embark on this path because I want to gain an in depth understanding within the Learning Disability and Mental Health field for example professional aspects of care, biological basis for health and well being, identifying knowledge for nursing practice, origins and expressions of Mental Health and wellbeing, I want to be able to carry out the tasks of providing care and support for people with mental in capabilities, knowing that I am doing so with the full comprehension and grasp of what each patient needs.
After this interview, my perspective on patient care has changed. In the future, I will work to increase patient autonomy. Although the patient was a minor, his voice should still be heard when deciding a care plan. As an OT, I will work on creating a role for the patient on their own care team. A Person centred approach is about ensuring someone with a disability is at the centre of decisions which relate to their life. A person centred process involves listening, thinking together, coaching, sharing ideas, and seeking feedback. By working in a person centred way we ensure that the individuals we are working with are listened to and that we focus on what matters to the individual and that their families pay attention to how staff support. This can be done with one page profiles, person centred reviews and full PCP’s.
It has given me the tools and confidence to continue my quest to gain unlimited knowledge and experience with a career in Nursing.
Initially I thought that I had little research experience except for gathering information regarding falls and looking for solutions to prevent them. Then I began thinking about all of the times that I have turned to journal articles when looking for information related to best practices in patient care. I hope to learn to develop a better understanding of the research process and to conduct my own research in courses to come.
Looking back to the first day of clinical to the last day I have changed a lot during this time. During the whole clinical experience I had to use a lot of the growth mindset in-order to get fully through the whole process. My clinical teacher and I didn’t quite have the same opinion on certain things. While we did struggle through are different mindsets, it was a great learning experience. I will have people I work with that I don’t always agree with and I will have to find a way to compromise with them. This was something we talked about with each other and worked through. So, Im grateful in that aspect in learning how to deal with those situations. I also changed in that I’m more confident in giving my lessons to my students, along with I tried to not be as scared in-front of them.
