North Coast OB/GYN: A Case Study

For this assignment, I had the opportunity to interview Samantha Hage De Reyes, family nurse practitioner, currently working at the University of California, Riverside (UCR) Health Center in Riverside, CA. Family nurse practitioners are described as health professionals with analytic skills for evaluating and providing evidence-based, patient-centered care across settings, and advanced knowledge of the health care delivery system (Hamric, Hanson, Tracy & O’Grady, 2014). My objective was to ask a series of questions pertaining to the role of a family nurse practitioner, challenges concerning this nursing role, opinions regarding the future of family nurse practitioners, and more. On April 221, 2017, at 1602hrs, VA police received a Duress alarm in 2F511. Investigation revealed that the Veteran was upset needing an appointment to get transition lenses. VA Police made contact with the Veteran at the Woman’s Clinic to obtain his side off the story. The Veteran became standoffish with officer and kept stating there was no problem. Organizations like the Institute of Medicine (IOM) and Centers for Medicare and Medicaid (CMS) all agree to allow nurses to practice to their full abilities to make healthcare more accessible and affordable, especially for the aging baby boomers and less accessible rural neighborhoods, and densely populated urban areas. Research has proven that NPs that provide primary care have similar health outcomes to primary care physicians (DeNisco et. al., 2016). NPs particularly take pride in their holistic approach, forming therapeutic relationships between other providers of the healthcare team, patients, and their families, aiding the informed decision making process, use of the evidence based practice approach in health management (Brown, 2005). As a family nurse practitioner, I plan to work in a medical office in Palo Alto Medical Foundation (PAMF). PAMF is a multi-specialty group practice, comprising of physicians, nurse practitioners and physician assistants from all specialties. It has more than 1000 health care providers and has more than 2 million patient visits each year (Palo Alto Medical Foundation, 2015). The group’s administration is headed by a Chief Executive Officer and is led by the PAMF board of directors. I will be working in the family medicine department in PAMF. Family medicine department team consists of family medicine doctors, nurse practitioners, physician assistants, nurses and medical assistants. I interviewed Beth Lindung via Skype who is a RNC-OB assistant nurse manager at the Labor and Delivery center at Franklin Square Hospital in Baltimore, Maryland. Beth has been a RN for 16.5 years with an emphasis in Labor and delivery 15 years. She got involved in Labor and Delivery because she never knows what her day can bring, and that excites her. Her day can vary greatly due to different circumstances and birth plans. She can come in and be put in the position of needing to be a triage nurse, ICU level nurse, OR nurse, a PACU nurse of simply a newborn nursery nurse. She decided to go into a leadership role after have having poor nurse managers and someone told her If you can do it better, Why don 't you (B. Lindung, personal communication, September 25, 2014). What she does everyday inspires me to be a greater nurse and be an advocate for women 's pre and post natal care through nursing.

A key recommendation for transforming nursing practice from the IOM is improving quality of care. There is an ample amount of research that associates midwifery care as being of comparable or higher quality to obstetrician/gynecologists with increased patient satisfaction and lower cost (American College of Nurse Midwives, 2012).
