CCIB Intake received a SOC 341 from reporting party (RP) Melanie Willis regarding and incident with residents Greg Ekdahl and Paige Katler. According to the RP on 1/26/17 at 9AM while being transported via van to Greg's day program (Escuela Del Rio Day Program) a peer sitting behind him was screaming and singing curse words while listen to music on the radio. The RP asked the peer to stop and Greg asked the peer to stop, in addition Greg turned off the radio. The peer (Paige) struck Greg twice on his left shoulder with an open hand in reaction to his turning off the radio stating " he turned off the radio, he deserved that." Greg was examined for injuries; none were found. He was later left at his day

CCIB Intake received e-mail from John Merrill regarding the initial complaint. The information is the same as the LIC802. LA City Fire Department of the location being problematic due to the number of request for medical assistance. There is a resident who is diabetic and a victim of strokes and kidney failure. He is reported to be in need of care and supervision. He was ordered by the Court to be placed at the location and is assigned to a Probation

CCIB Intake received a call from licensee Gurpreet Kaur who stated on 8/29/16 at approximately 3PM day care child Arya Naik DOB: 12/14/13 during nap time (2:15PM) the child began to make strange noise. The licensee stated she called out the child's name and the child didn't respond. The child's eyes were open and she was breathing. The licensee stated 911 was contacted and emergency responders responded within 15 minutes. After contacted emergency services the licensee contacted the child's parents. The child's mother arrived at the home while the emergency responders were still at the home. The child was transported to the local hospital and diagnosed with double ear infections per the child's father. The licensee will submit the written report

CCIB LPA Perryman-French received a call from a parent who was trying out this day care. She brought her one year old child to the program and then snuck around the corner to watch how the child adjusted. (Her child hung on to the gate for a few minutes and then wandered into the classroom. Caller then watch a Ms. Veronica grab a toy (or something) out of a little boy's hand (under two years old) and say "Give me that." Her grab was forceful enough and her tone loud enough that caller immediately reacted. Caller walked away for maybe a half hour, then could not handle the bad feeling she had. She returned and found her child had red swollen eyes and snot on the face, was clearly upset, but the teacher did not comforting, instead, just told

CCIB received a Corrective Plan of Action (CAP) detailing the visit to the home on 10/18/17 by Service Coordinator, Sally Cano. Ms. Cano arrived at the home, but no one was there. Ms. Cano proceeded to check Delta Home 3 (located next door) and found 2 of Delta Home Care IV residents along with residents from Delta Home Care 3 at the home. As Ms. Cano was leaving, she observed a bus arrive at the Delta Home Care 2. The bus driver reported that there was no staff at the home and that the residents were waiting outside. It was observed the Delta Home Care 2, 3, and IV to be lacking staff. On 10/19/17, RP and Quality Assurance specialist (QAS) Jesus Ozeda went to the facility to observe the staff to resident ratios and to collect the staff schedules

CCIB received a SOC 341 via email from Maria Doreen Ainsleigh (medical SW) regarding Terrylee Young (DOB 2/18/1948). RP reported that Terrylee stated that some of the nurses treat her and other residents bad (did not state how). RP reported that Terrylee stated that the staff (names unknown) yells and scream at her and other residents (LPA contacted the SW and Terrylee neither parties could tell me what staff says). RP reported that Terrylee also stated that the Administrator Joe Johnson curses at her and the other residents when they are having problems in the facility. CCIB Intake received a call from owner/administrator Therese Kragness who disclosed that the local Law Enforcement was present at the facility. According to the caller, an employee who was having difficulty with her boyfriend received a text threatening to come to the facility. Fearing for her safety, the employee notified Law Enforcement and was escorted to the Police Station to file a restraining order against the boyfriend. Conversely the boyfriend did not appear at the facility. The Administrator will fax the hard copy of the report identifying the employee within the required time

CCIB LPA received a call from Theodore and his girlfriend, Linda Lubert. Theodore and Linda were in a skilled nursing (rehab) facility until May 9th, 2015. They were given 24 hours to move and ended up at this facility. When they arrived, they were not appraised, given any care plan or schedule. Both need assistance getting up, dressed and with hygiene, they have to beg for staff to help them. The doors to their rooms are locked at night and they are not allowed out of them. This happens during the day sometimes as well. Since he has arrived he has had to wait five days for assistance with showering, staff say they do not have time or the towels are in the dirty laundry. CCIB Intake received a formal complaint letter from California Advocates For Nursing Home Reform (CANHR) Long Term Care Justice and Advocacy stating the facility failed to maintain the required liability insurance to cover injury to resident or guest. CCIB LPA C. Perryman-French received a call from Gina, her son (2 years old) attended the program from 02/01/15 until 0714/15. When Gina's son was admitted, Nova (licensee) assured her that the ratios would not be more that 1:8. Gina understands that CCL requires a 1:12, however, on several occasions in May, June and July there were more than 24 children to the two teachers. When she approached Nova, Nova stated that she counted in the ratios even though she was in the administrative office. Gina's son received a scrap from a cinder block that is used to hold up a play structure. He was in a wading pool near the block and apparenly slipped on the wet floor/ground and scratched him arm. Gina could not get a specific cause for the scratch or

CCIB received a complaint via phone call from Anonymous female (employee) regarding the facility. RP stated that the facility has been hiding children to stay in ratio. RP stated that the facility hides about 20 to 25 children (ages 2 to 3) when CCL comes to visits. RP reported that the administrator hides the extra kids at the Otimal Christian Academy (another school on the grounds). RP also stated that class rooms 1, 2 and 3 have fraudulent role sheets with kids missing on the sheets. RP stated that the classrooms 1, 2 and 3 have a head count at 10 am on the children in the class so they can find out who keep in the classroom if CCL comes for a visit. RP mentioned that there are roaches and rats in the pantry for about 3 years now. RP stated

CCIB received a telephone call with an allegation against the facility. RP called to report that there is drug use and domestic violence happening in home due to the licensee runs a daycare. RP reported that on 10/9/2017 Veronica and Juan went to pick up children from school and Veronica asked staff at the school to call the police. RP reported that staff told her to contact the police to report what is happening. The RP reported that the police came to the school and arrested Juan in the parking lot. RP stated that it was stated on the police report that Veronica was hit in the face and kicked (unknown when incident happened). RP reported that the police report stated Veronica had a cut lip and red marks on her legs. RP also mentioned on 10/10/2017

What do people think of when they first hear the word nap? Everyday sayings such as snatching a nap, being caught napping, snoozing, or stealing forty winks are all beneficial daily activities we need to add to our busy life. If we do, we will be more awake and capable to reach our full potential as an individual. People don’t realize how important a nap can be. Although popular beliefs suggest that naps are a waste of time, naps actually increase productivity by resting the brain.
