Narrative As I (Ofc.S.Diaz) was walking from store to store I (Ofc.S.Diaz) notice a person on the ground in front of Nando Peri Peri when I approached the person (Ms.Janet last name not given 65yrs old female) she was laying down on her face when she looked up I (Diaz) notice her nose was bleeding severe I ask another pedestrian to call 911 for Ms.Janet as the pedestrian was on the phone with 911 Ofc. P. Diddy and I (Diaz) kept talking to Ms.Janet. Ms.Janet had told us she was walking from carter store when she trip on her own foot and fell she believes it was her own shoes being loose that cause her to fall.Laurel Volunteer Rescue Squad came and took Ms.Janet to laurel hospital..
On 3/13/16 SO EMT Perez was notified that an engine had come on campus. SO EMT Cunningham responded along with SO EMT Perez and caught up with the engine by lakeside commons. The driver of the engine stated that he was responding for a lift assist to 3144/3146 apt 220 but that he wasn’t sure which one he was going too. SO EMT Cunningham and SO EMT Perez decided to respond to FR-220 first being that they were the engine came from gate 2 and they were closest to that building. SO EMT Perez and SO EMT Cunningham knocked and announce their presence at FR-220’s door and were both verbally invited in. The resident a Mrs. Helen Neff stated that she could not get out of her chair because it was stuck and needed help getting out of it. SO EMT Perez
On 10/21/2015 SO EMT Perez was dispatched to HS-318 regaurding a general assistance call. SO EMT Perez knocked and announced his presnece at the door and was verbally greeted in by the resident, a Mrs. Netha Hopson. Mrs. Netha Hopson stated that she needed help retrieving her purse because her motorized chair was caught on it and she could not get it untangled on her own. SO EMT Perez managed to untangle the purse and retrieve it for Mrs. Netha Hopson. After insuring that Mrs. Netha Hopson needed nothing else SO EMT Perez cleared the scene without further incident.
RP stated law enforcement received a call from an anonymous caller stated an employee, Shemaiah Messengale grabbed the child Marley's arm which is fractured. Law enforcement contacted the child's mother to verify injuries. It was disclosed the child had suffered 4 fractures to her elbow and wrist. The RP stated law enforcement spoke with owner/licensee of the facility and the employee accused of causing the injury. They reported Marley sustained the injuries accidentally when she fell from slide. The RP stated the child was treated at Children's Hospital Los Angeles (CHLA) by Dr. Lindsey Andras who determined the injuries are consistent with a fall. Law enforcement did not speak with treating doctor to confirm the information. The RP disclosed
S/L Walker was dispatching for evening shift at 08/29/15. S/L Walker receive a call for a welfare check at KC-219. The daughter, Karen, usually talk to her mother at around 2100 hr. But tonight could not reach her. S/L Walker took down the information and send an officer up to the apartment. S/L Walker called the apartment while the Officer was en route. There was no answer by phone. S/L Walker notified the Officer. S/O EMT Funes was the reporting officer. When S/O EMT Funes arrived on the scene, there was no answer by knock. S/O EMT Funes entered via her master key because the door was lock and search the apartment. There was no one home and S/O EMT Funes left a white tag #28798. S/O EMT Funes cleared. S/L Walker called the daughter twice
As I was completing my initial investigation I was returning items (driver’s licenses, registration, and insurance information) to the drivers. I approached Mitchell who was sitting in her vehicle. She appeared slumped over. I knocked on the window several times. There was no reply. I opened Mitchell’s vehicle door. She appeared unconscious. I shook her by the shoulder. She did not respond. I called for Goodwill Ambulance, who had just left with Feeney to return because I had an unresponsive person. I pulled Mitchell out of her vehicle and laid her on the street. I put a blanket that she had in her vehicle under her head. I shook her again, no response. I performed a sternum rub on Mitchell. She moved her head and asked “What’s going on?” I told her she passed out that she was in and accident. I told her to keep her eyes open and waited for Goodwill Ambulance to arrive back on scene. After the Paramedic treated Mitchell he informed me that he was unsure of her injury or what could have cause her

The resident, a Mrs.Martha Doyle was sitting in her living room chair and was bleeding from the forehead. Mrs. Martha Doyle was washing her face when she attempted to remove a bandage over a previous wound and she wanted it re-bandaged as she stated. SO EMT Perez asked Mrs. Martha Doyle if she wished to go to hospital before he assesed her to which she denied. Mrs.Martha Doyle also denied any care besides the application of a new bandage. On 7/6/2015 S/O EMT Perez was dispatched to WC-407 regaurding a pendant alarm. S/O EMT Perez knocked and annoounced his presence at the door, when there was no answer S/O EMT Perez proceeded to search the apartment. S/O EMT Perez found the resident, a Mrs. Mary Beiter toppled over her walker by her bedside. Mrs. Mary Beiter was trying to get inter her bed when she lost balance and fell and could not get up under her own strength. Mrs.Beiter did not complain of any hip pain during the pelvic exam but did state that she has a bad knee and hip that makes it rather hard to stand on her own. S/O EMT Perez asked Mrs.Beiter if she wanted to be transported to the hospital before he picked her up which she declined. S/O EMT Perez performed a lift assist
I the writer V. De La Garza was assigned to 1P21 at the Bayshore district unit. I was informed by dispatch to quickly respond to a 911 call. The victim advised her that she was being attacked by her husband at 1234 13th St. Bacliff Texas. Dispatch then hears multiple gunshots and no further response from the victim. Dispatch hears the suspect reload a magazine and continue to fire. Dispatch sends EMS and animal control, after hearing aggressive dog barking turns into the dog squealing as if it was shot or injured.
Medic 1 was dispatched by 911 to 1234 Main St for a possible code blue for a 5 year old Male. Fire Dept was on scene as this was the location of a structure fire. Along with Fire Dept the local first responders was on scene. The first responders arrived on scene and requested ALS from dispatch. Medic 1 arrived on scene to find a child lying supine across the street from where the fire was located. There was four to five fire firefighters located around the patient assisting first responders. As Medic 1 arrived on scene was was given a history on the patient from first responders. They noted that the boy was pulled from the house that was on fire by firefighters and started mouth to mouth ventilation's. They said when they arrived on scene they
The SC arrived at the group home Bo Waffer. The SC was greeted by the nurse Marie and CIS Jentrelle Moore. Both Jentrelle and Marie note knocked on the door and there was no answer by Bo. SC and Jentelle had the nurse to call the owner. The owner reported that Bo should be in the home. It was reported that the nurse did not have a key to get into the home and that there was no staff inside of the home with Bo. Jentrelle and ECC continued to knock on the door and windows. SC asked to Jentrelle call by Mesquite PD to come out and do a Welfare Check call by Mesquite PD. The staff was able to get into the home by a window after checking the entire outside of the home.
