CCIB received a call from DCFS. They received a call from a member of the staff of LA County Board of Supervisors (Ofuna). Elizabeth reported that FFA SW Claudia Cuevas was driving her personal vehicle (with no foster children in it) when she was involved in a vehicle accident. The other driver was a Miss Cummins, was concerned and convinced Ms. Cuevas to take cash for the damage repair. MS. Cuevas accepted $450 in cash (in two payments.) Miss Cummins is apparently under the age of 18 and her mother, Nobie Cummins became aware of the situation and confronted Ms. Cuevas at the FFA. (Karen cash, director of the FFA was present). Nobie Cummins accused Ms. Cuevas of intimidation and manipulation of her daughter. (Daughter was called and on speaker

Upon arrival, I spoke with the petitioner of the order of protection (Muro, Christine DOB 08/15/55), who related her daughter (Corona, Christina DOB 08/15/88) was inside her bedroom and that she has an active order of protection. Muro provided Cook County Order of Protection, which was valid and served. Corona was prohibited from entering or remaining at the residence/household inhabited by protected person(s). I made contact with Corona inside her bedroom. Corona was checked through L.E.A.D.S., which revealed she an active order of protection and warrant from Cook County. Corona was placed into custody at 0016 hours and transported to the Cicero Police Department. Muro signed one misdemeanor complaint for violation of an oder of protection. Muro was issued a Maybrook Court date (10/19/15 at 9:00 A.M. in room 104) and a Domestic Violence packet on the scene.

On June 20, 2014, the Department obtained emergency custody through C&P of Ryan due to neglect by mother Heather Martin. Mother was arrested by Burlington Police on 6/16/14 for larceny; shoplifting and three counts of receiving stolen property. Heather was taken to at MCI Framingham. The family was placed in a hotel through Department of Transitional Assistance. It was against the hotel policy for children to be left in the hotel unsupervised, and Ryan and Bria were staying alone in the room for five days. This was deemed concerning for the two children to not have an appropriate caregiver. When the Department was made aware of the situation father Brian Scopa 's whereabouts were unknown to the Department. Family members were contacted that could possibly take the children in. However, uncle Stephen Martin, aunt Cindy Fiore and Grandmother Carroll Clinton were unable to do so for various reasons. Given that there were no family members to take Ryan and Bria in, the Department placed them in a temporary Hotline foster care placement from the 6/20/14 to 6/23/14 until a more suitable living arrangement was made available. On 6/23/14 Ryan and his sister were placed in a kinship home of a family friend.

UCM: CPSW did a home visit to Ms. Brenda Borkovec's home. Both children were in the living room playing together. Also, Ms. Brenda was in the living room holding Aydden and feeding him. Ms. Brenda stated that she has not received any papers regarding her foster care license. Ms. Brenda stated that she is frustrated with the process and that it has taken a longer time. Ms. Brenda mentioned that it has taken a long time for her to complete her fingerprint. She reported that currently she is waiting the background study and fingerprint forms to be sent to her. Ms. Borkovec's phone ring and it was Elizabeth Borkovec. Ms. Brenda asked CPSW if Elizabeth can come and join the meeting. CPSW approved it and Elizabeth Borkovec showed up less than 5 minutes.

A report was received on 06/05/2017 alleging that the mother (Hermionne) left Ashante (C-V 17) with a non-relative since 02/2017 without any legal rights. According to the report, the mother refuses to take her child back home and will not engage with Ms. Aarons (caretaker) to provide legal documentation for the child to be enroll in school and taken to a Primary Care Physician. Ashante has not been is school for the past 4 months and are unable to enroll in school without paperwork. According to the report, Ashante self mutilates her arms, and the mother refuses to get counseling. The report indicates Ashante was to follow-up with a Cardiologist for a chest pain and the mother refuses to take her to the doctor.

EREPORT # 19021 stated the following: The incident happened last night, 2/8/2016. Destiny's mother has "taken off" as evidenced by empty drawers and closets. Destiny stayed home alone all night by herself. The concern is that Destiny is not supposed to be alone with her dad, per DHS. Dad never came home last night, but the concern is 1) dad comes home and she is alone with him or 2) dad doesn't come home and she is alone again by herself all night. I found out abou the situation because the teacher told me. estiny disclosed to her teacher about being home alone, mother "taken off" and that dad may come home and she has to be alone with him. Terry, APCW Cowsette received a hotline on Reported Adult Barbara Henson with the allegations being caregiver Neglect medical, Caregiver Neglect Physical, & Financial exploitation. APCW Cowsette was able to obtain enough evidence to pursue guardianship & conservator. The Ra was placed at Creve Coeur Manor with the understanding that DHSS would be pursuing guardianship & conservator. APCW Cowsette was informed by OGC Courtney Chaplin that she would not be pursuing guardianship because the Ra is in a protected environment. OGC failed to understand the placement was on the condition

The case was brought to DCFS attention when a reporter stated on 7/11/17, OPWI attempted to obtain an emergency order of protection on behalf of Kiara (age 7). Reportedly Ashley (mother) is actively using METH and also methodone she gets from a clinic, and is also involved in prostitution. Ashley told OPWI that Ashley ''can't wait outside for the bus to go to drug court because she's done bad things to people (robbed people, committed home invasion, taken people's money and drugs) and these people are after her.'' Reporter states that one of these people came to OPWI's house looking for Ashley and scared Kiara. Ashley recently got out of jail, stayed with OPWI for a couple of days, left last Friday telling Kiara she would be back in a couple of hours, didn't return

Stephanie was referred to FACT for CMO services by Trinitas Hospital – CCIS unit. Stephanie was admitted to CCIS on 4/18/17 for running away from her mother’s home and making threats to hurt self if she were to return to reside with her mother. While in the hospital, Stephanie has also been refusing to visit with mother. At the time of the FCP meeting, Stephanie (Youth) resides with her Natalia Ramos (Mother), Elsa Sanchez (Maternal grandmother) and Johnathan Sanchez (Uncle) who disabled. Mother and stepfather recently divorced. Her father Hamilton Ramirez (biological Father) lives in Elizabeth with his girlfriend and younger daughter. Each biological parent has children with other people. Parents do not get along at all. Youth wants to live with father.
