Life Care Planning Overview
A life care plan is basically a comprehensive document that provides for the future care and associated costs of a person facing a serious illness or injury. Full life care plans result from a stringent medical billing audit, medical record analysis, and several interviews, however there are also lower level life care plans that can help you evaluate the potential value of a case in earlier phases of litigation. These needs-based documents project costs in present day value, allowing all parties involved to truly understand the financial implications of a medical condition.

It's difficult to properly analyze all aspects of an injured party's condition. A professional life care planner can help you access the current needs of the patient and project future complications with a systematic approach to analyzing the injured party's current and future conditions. Care plans are developed by the service users, and when needed with help and assistance from friends and family. These plans are then to be agreed by a social worker or senior care manager e.g. the nurse or senior care worker depending on if the person is nursing or a residential client. By finding about a person’s preferences, life history and wishes then the care plan can be written to reflect this and the best care can be given. By knowing their life history may explain why they do things in certain ways. It also encourages discussion and leads to building relationships. They may like things done in a certain way to remind them of loved ones who are no longer here.
It is necessary to involve the individual in the plan of care and support. Encourage the individual to make choices. This includes their needs, their culture, their means of communication, their likes and dislikes, wishes and feelings, advance directives, beliefs and values, involvement of their family and other professionals. This should be considered and documented. Also, there must be evaluation in assessing effectiveness in the plan of care.
This helped me create a specialized plan of care for my patients and their families.

Upon completing a care assessment, the assessor will work with you to develop a care plan. Each plan is tailored to provide you, or your loved one, with the support needed to continue living in your current situation, if this is the best option.
