On 04/22/16, Ken'Dell came to school with a red scratch mark on his face. The reporter stated that the child isnt complaining of pain or needs any medical treatment. According to the reporter, Ken'Dell told her the scratch on his face came from running into a pole. According to the reporter, the child told he ran into a pole while chasing his baby brother, that chased a cat. The reporter stated that the children were not properly supervised and was outside the home alone. Ms. Pendleton stated there are not any prior concerns and the child's basic needs are being met. Ms. Pendelton stated her primary concern is the proper supervision.
RP stated law enforcement received a call from an anonymous caller stated an employee, Shemaiah Messengale grabbed the child Marley's arm which is fractured. Law enforcement contacted the child's mother to verify injuries. It was disclosed the child had suffered 4 fractures to her elbow and wrist. The RP stated law enforcement spoke with owner/licensee of the facility and the employee accused of causing the injury. They reported Marley sustained the injuries accidentally when she fell from slide. The RP stated the child was treated at Children's Hospital Los Angeles (CHLA) by Dr. Lindsey Andras who determined the injuries are consistent with a fall. CCIB LPA Perryman-French received a call from a parent who was trying out this day care. She brought her one year old child to the program and then snuck around the corner to watch how the child adjusted. (Her child hung on to the gate for a few minutes and then wandered into the classroom. Caller then watch a Ms. Veronica grab a toy (or something) out of a little boy's hand (under two years old) and say "Give me that." Her grab was forceful enough and her tone loud enough that caller immediately reacted. Caller walked away for maybe a half hour, then could not handle the bad feeling she had. She returned and found her child had red swollen eyes and snot on the face, was clearly upset, but the teacher did not comforting, instead, just told
On 09/10/15 Ms. McClain voluntarily admitted her son to Parkwood for treatment; the child has not been given a discharge date. During the assessment with the child and his mother, Jameraqua disclosed to the reporter that he and his mother have had multiple physical altercations in the past and a several months ago his mother whipped him with an extension cord. The child did not have any visible marks or bruises and indicated to the reporter he doesn’t have any. Jameraqua feels his mother fights him because she dislikes him; Ms. Mitchell is unaware of the reasons. The reporter witnessed a volatile relationship between the child and mother and they had to be separated during his assessment.
On 1/6/16 at approximately 12:05PM the reporting party (RP) was alone with foster child Michael Martinez as they worked on a puzzle on the floor of the resource room. The reporting party (RP) stated she spoke with the foster child's maternal grandmother (name unknown) regarding an incident involving the foster father. According to the RP several months ago (exact date unknown) the foster father pulled the foster child up the stairs by her arms. The incident occurred when the foster child refused to go to her room therefore the foster father grabbed the child by her arms and dragged her up the stairs. The foster child stated she did not suffer any injuries or bruises during the incident. The foster child disclosed that she is not fearful of the foster father and has no desire to leave the home. The reporting party (RP) stated his daughter has had several falls resulting in injuries. According to the RP the falls were due to the teacher's lack of supervision. On 11/1/16 while walking down steps the child turned away from the RP 19 month child and loss contact causing the child to fall down the steps. Subsequently on 10/11/16 the child was bitten on 3 occasion on her left arm and elbow in one day. A month prior the RP reported his daughter fell while outdoors on concrete with insufficient cushioning. Again the child fell on a toy and injured her eye. Yes, Tavion’s mother’s statement of concern about the suspicion of abuse should warrant a valid reason to lunch investigation in the child injury. Moreover, the emergency department staff had suspicion about the Tavion’s injury due to the fact injury is not consistence with an injury sustained in park playground. Hospitals are mandate by law to report any suspicion of child abuse; the Child Abuse Prevention and Treatment Act enacted in 1974 was designed to encourage the reporting of child abuse cases, and prevention. In addition, most states have enacted laws to further protect abuse children, and most states protect the individual required to report cases of suspected child abuse; the following individual are eligible to report suspected cases of abuse include healthcare administrators, physicians, interns, registered nurse, chiropractors, social service workers, psychologists, dentists, osteopaths, optometrists, podiatrists, mental health professionals, and volunteer in healthcare facilities (Pozgar, 2014).
Reporting party (RP) stated that her son Hudson Satterfield (DOB: 06/02/15) attended the daycare since August 2015 through June 2016 (RP is a teacher so the child remained at home during summer months). Since the child's return to the facility in January of 2016, he has returned home with scratches on his face, neck, arms and near his eyes on a daily basis. RP was told by staff that Hudson was being retaliated against by the other children because Hudson had been grabbing on the other children. RP reported this to the administrator and inquired why they were not informed of Hudson's behavior (RP has only received 3 incident reports). 1:10pm- IRTC called the source who validated the narrative and expressed that the injury was observed yesterday (11/27/17). Source voiced that the injury appeared to be more like a rash; since the child has a history of eczema, they didn’t think much of it. Source uttered that the subject child’s mother called inquiring about the injury and wanted to know if the subject child fell at the daycare. Source stated that the child did not fall, but it was recommended for the child to be brought for a medical assessment. Source indicated that they received a return call from the subject child’s mother stating that the medical provider reported that the injury appears to have been sustained by a fall or possibly by another child. Source reiterated that the child did not fall at the daycare. Source added that besides the subject child, there are two active children residing at the case address.
The reporting party (RP) disclosed that during a supervised visit with foster children Jessen Allen and Caden Allen, Caden appeared lethargic. When the RP retrieved the child from a baby bouncer she observed the child's tongue handing out of his mouth. The child appeared to be wheezing. The RP brought the wheezing to the attention of the attending social worker (no other information); Caden disclosed to the RP that Caden sleeps in the foster mother's (FM) bedroom in a crib. Caden has not been observed crawling around. According to the RP the foster mother keeps Jessen and Caden apart. The two children are not allowed to play together. The RP disclosed that the FM does not provide dinner to Caden. Consequently he is provided waffles for lunch
The complainant stated that on the Friday of April 28, 2017, she and her 13 year old sister (Kimani Gandy) visited their father’s residence, located at 1702 Gales Street, NE #4. The complainant stated that they were visiting five weeks old new born stepbrother (Demarcus Gandy). The complainant stated that during her visit, her sister Kimani was complaining of pain to her foot. Kimani injured her foot when her mother threw an object and it accidentally hit her foot. The complainant stated that her father (David Gandy) made a comment as if her mother intestinally threw an object at Kimani’s foot. The complainant stated that she told Mr. Gandy that it was not true, and they get in a verbal altercation about Mr. Gandy’s comments about their mother.

During individual session today, 10/4/17, the child admitted his stepfather jumped on his sister. Reporter stated it sound like this has been going on for a couple of months. Reporter stated it has not been long since the last incident took place. The child did not go into details as to why the stepdad jumps on the sister. The child did not stay if the sister have any marks or bruises. The child said he is tired of the stepdad jumping on his sister and he is getting angry. He said he want to do something to the stepdad. Mom don’t know about the situation. The child did not say why mom does not know, reporter believes she is not at home during the time.
