CCIB received a referral #1659-2526-7518-9002203 via email from La Vette Norris (Staff Member) regarding Keishaunna Ross (16) (DOB 1/15/2001). RP reported Keishaunna was placed in her home “about three hours ago” by assigned CSW Kendra Jones and RP wants her removed immediately. RP reported stated Keishaunna “poses a risk to the other children”, but did not state what kind of risk. RP reported that Keishaunna “is real angry,” “doesn’t want to be at the home” and “she going back and forth with me.” RP reported Keishaunna has a history of AWOL and Keishaunna could something to her or one of the children in the home (RP did not say what that might be). This officer has had contact via face to face and phone with the mother, Ms. Evelyn Edwards. Discussions have taken place with her regarding Marquel’s Comprehensive Re-Entry Case Plan. She has participated in a video conference with Marquel since his commitment. She is aware of his placement in the Virginia Beach CPP Program. She has not visited him at the facility, but she has had contact with him via phone. She is currently unemployed. Upon Marquel’s release from DJJ, the anticipated parole plan (CRCP) will be for him to return to her home with intensive supportive services via 294 funding. CPSW did a home visit to meet with Ms. Berner and to discuss about her safety plan since the children are moving with her on10/29/16 from the foster provider. Ms. Berner was late for her appointment and CPSW waited a 40 minutes for Ms. Berner. Ms. Berner apologized for being late. CPSW explained Ms. About safety plan. Ms. Berner understood and she signed them. Ms. Berner stated that she will be doing a house arrest for two weeks and the recommendation of her criminal court is to obey law and continue taking her medication on time and seeing her therapist. INCIDENT/EVENT: This writer received a call on Tuesday, August 25, 2015, from civilian Ms. Vera Worsley of the Metropolitan Police Department, 6th District. She advised a Ms. Benneilla Willis was there with a court order 2014 DRB 001276 that law enforcement from any jurisdiction shall assist Defendant Ms. Benneilla Willis in retrieving the minor children.
The case was brought to DCFS attention when a reporter stated on 7/11/17, OPWI attempted to obtain an emergency order of protection on behalf of Kiara (age 7). Reportedly Ashley (mother) is actively using METH and also methodone she gets from a clinic, and is also involved in prostitution. Ashley told OPWI that Ashley ''can't wait outside for the bus to go to drug court because she's done bad things to people (robbed people, committed home invasion, taken people's money and drugs) and these people are after her.'' Reporter states that one of these people came to OPWI's house looking for Ashley and scared Kiara. Ashley recently got out of jail, stayed with OPWI for a couple of days, left last Friday telling Kiara she would be back in a couple of hours, didn't return

Ms. Floyd received information from Ms. Goods stating she’d put her mother out of her home because of her crack use. Brittney was not put out of the home and is currently residing with her sister. Brittney’s basic needs are being met by her sister and Ms. Goods appears to be a better and more responsible caretaker. Brittney has been in and out of the court system and Ms. Higgins has legal custody of the child at this time; Brittney is without a legal guardian since her mother was put out of the home. It’s unknown what date Ms. Higgins was put out but the information was reported on 08/11/15.
RCG Rose Campos reports no health problems. Ms. Campos also reported that she has had no problems with Elijah at home. However, I am concerned that Elija is not safe in his present home with, the maternal grandmother due to Austin’s behavior. Ms. Campos reports no physical harm to Elijah by Austin, but Elijah has witnessed Austin’s violent and abusive behavior to Ms. Campos. I am concerned about Austin’s exposure to this violent and abusive behavior to his grandmother.
We got there at 11:30 am. The young lady at the front desk looked up Ms. Cortes’ case and informed us that her case was closed, but the case worker was still willing to meet with her. The case worker came out and told us the director wanted to speak with us, but we would have to wait a while. We sat down and waited until 1:30 p.m. at which point I told Ms. Cortes to remind them she was still here. The case worker came downstairs and told us to come into her office. She as still waiting for the Director. The case worker asked, Ms. Cortes what happened at Project Home. Ms. Cortes recap the events that led to her being discharge on April 12th 2016. On 06/01/2016 at 8:55 AM I along with Cpl. Pries, R. was escorting the above named inmate from the BHU Interview Room back to his assigned housing, BHU-13. Once we entered Section II of the BHU the inmate attempted to kick and head-butt Cpl. Pries and me. Because of this we had to place the inmate on the ground in order to stop his behavior and to gain control of him. Once the inmate was under control he was taken back to assigned cell. On March 8, 2016, I was assigned this case to follow up. This case involves Ms. Talia Karina Barillas a sixteen year old T.C. Williams High School student. I had previously been assigned runaway cases involving Talia in 2015. Talia left for over 30 days during the summer and was located at school unharmed. During previous follow up an LBGQT issue was the school was going to assist her with services.

CCIB received a telephone call with an allegation against the home. RP reported she received a text on Tuesday 8/8/2017 from Holly Harris (licensee assistant) stating to contact her. RP stated that she did not contact her back until 8/9/2017, and that’s when Holly told the RP that Gloria (licensee) would hit RP son Fisher in the face, and give Fisher Tylenol or Benadryl to make him go to sleep. RP stated that she was appalled to hear to hear such things are happening to her son. RP went on to state that Holly told her that the Gloria would force feeding Fisher. PER REPORTER: Kanosho said she works for the foster care and licensure department in CPS, and she said she received a report from a caller by the name of (Kelly) whom called into the agency. She said Kelly told her that she is worried about two children in her family that live in Mississippi due to different family members calling an informing her about things involving the children. However, Kanosho said Kelly told her that no one else in the family will report the things going on with the children besides her. According to Kelly, the family’s home is dilapidated and she said if someone were to go into the home they would consider it condemned. The reporting party (RP) stated resident Arev Dabbagh DOB: 12/27/00 disclosed on 4/15/16 she inappropriately touched by a male peer. According to the Arev on 4/14/16 a peer Ibraheem Mamoori DOB: 9/2/98) placed his hand on her shoulder and continued downwards towards her breast. The peer touched her breast over he clothing. The resident was distraught while she recounted the incident. Arev alluded to their being other incidents with the peer and other unknown males. Arev didn't disclose details or clarification. Arev was placed voluntarily in the facility by her mother and will continue her placement for a few more weeks. The peer is currently placed on a 5150 hold and will not have access to Arev in the future.
