IRTC spoke with assigned CPS, Myra Townes, voiced that a joint response occurred at the case address with NYCACS and NYPD. CPS stated that Detective Gomes was present with two additional police officers. CPS indicated that the subject child was observe with the injury; the alleged subject explanation for the injury was that the child ran towards her direction while she was cooking food in the kitchen and the grease from the pot burned her face. CPS stated that the alleged subject mentioned that she placed the child in another room to watch television while drinking juice and did not expect the child to enter the kitchen. CPS said due to the severity of the observed injury, Detective Gomes contact his captain for directives regarding this matter.
On 02/11/2017 I was Dispatched to Smith Springs Elementary School for an investigation. A student Cire Kistner, was reported as being visibly distraught upon her arrival at school. She was taken to Lance Forman's office the schools principal and was questioned. The student reportedly told Forman that she and her sisters walked into the bathroom and witnessed their father, Eric Kistner, with a needle in his mouth and blood on his forehead. He was passed out on the floor. There was a spoon with "black watery play dough" on the counter. Principal Forman contacted DCS and informed them of the situation as well as MNPD. Metro P.D held the students at school, while also performing a welfare check on their father at the Kistner residence and calling

RP stated law enforcement received a call from an anonymous caller stated an employee, Shemaiah Messengale grabbed the child Marley's arm which is fractured. Law enforcement contacted the child's mother to verify injuries. It was disclosed the child had suffered 4 fractures to her elbow and wrist. The RP stated law enforcement spoke with owner/licensee of the facility and the employee accused of causing the injury. They reported Marley sustained the injuries accidentally when she fell from slide. The RP stated the child was treated at Children's Hospital Los Angeles (CHLA) by Dr. Lindsey Andras who determined the injuries are consistent with a fall. Law enforcement did not speak with treating doctor to confirm the information. The RP disclosed
CW went to 1261 Everton Dr, Apt 9 Akron, OH 44307 because they were called out by Akron Police Department (APD) for allegations of abuse towards 2 children. CW Jonathon Henry and CW Sonya Cole pulled up to the apartment complex and observed that there were over 15 people sitting outside. Both CW could tell that this was the correct address because there were 3 police cars that were sitting outside. As the CW's were walking towards the apartment complex an APD officer asked if the two workers were from CSB. CW Cold responded that they were. The officer then told CW's that there were not 2 children, however, there was 3. The officer stated that there was one child in the bedroom sleeping that they did not know was in the home. CW's were also told by the officer that Mr. Brian Burrell (AP) was in the home and that he was very agitated. The officer told the CW that they had called him down, however, suspected that he would get upset after entering the home. The officer also said that he already knew that the kids were in the custody of CSB and that he would most likely not be happy that the two CW's were at the home. The officer the led both the CW upstairs to apartment 9.
Dispatch alerted Central Pd. Officer C. M. Early about a white male 25 years old with a gunshot to the chest and abdomen. Officer Early arrived on the scene at 0630. Officer Early talked to the witness, Custodial staff Jody de Cleaner, who said that he heard popping sounds at 0620 and decided to walk over where he found the body. The EMS arrived shortly after at 0650 to transport the victim to the hospital. Ten minutes after the EMS arrived I arrived at the crime scene.
The reporting party (RP) received call from Diego Baca with Kings County CPS, 1400 W. Lacey Blvd., Hanford, CA 93230 (559) 852-2345 who reported that on 12/21/16 at approximately 3PM while conducting a placement visit for youth Andrew Yanez DOB: 12/18/01 the youth and the social worker took a walk. During the walk Andrew disclosed that he and a staff member named Shawn Wilcox were involved in an altercation. According to Andrew at approximately 2:45PM he was in the office of the ground home with 2 other staff members named James Boston and Nicole Juarez. Medic 1 was dispatched by 911 to 1234 Main St for a possible code blue for a 5 year old Male. Fire Dept was on scene as this was the location of a structure fire. Along with Fire Dept the local first responders was on scene. The first responders arrived on scene and requested ALS from dispatch. Medic 1 arrived on scene to find a child lying supine across the street from where the fire was located. There was four to five fire firefighters located around the patient assisting first responders. As Medic 1 arrived on scene was was given a history on the patient from first responders. Mrs. Drew stated “Saniya was at her great-grandmother Norma Drew’s home Mrs. Drew noticed Saniya had a knot on her forehead being concerned she asked Saniya how she got the knot on her forehead. And Saniya said she was hit. Norma the asked Gerald Daniels how Saniya got a knot on her forehead and Gerald said she fell playing with her cousin. Beforehand I asked Saniya too and she told she fell. Well my mother didn’t like the reason she was giving by Gerald and reported the incident to DHS. Upon D/CPL. Case’s arrival at the crime scene there is no documentation in the crime report that he had contact with the first responder, PFC Malcolm. The first responder should brief the primary investigator as it is the only opportunity for the next in command to obtain initial aspects of the crime scene prior to subsequent information (Technical Working Group on Crime Scene Investigation, 2000). On April 05, 2016 at approximately 1518 hours, I was dispatch to 2004 Pine Street in reference to a death investigation. Upon my arrival, I met with Detective Campbell who was the lead Investigator at the scene. Detective Campbell stated that the victim neighbors reported a strong smell coming from the victim’s (Cortiz Lazarz) residence. The neighbors called 911 and Officer Volk was the first responded officer on scene. Defective Campbell and officer Volk stated that the victim located on the living room floor, fazing the south from the residence. Detective Campbell requested I process the scene for evidence and documentation.
