On March 14th 2018, at 16:00 hours I witnessed on the CVV camera #72, that employee #10070 Janet Smith had fallen from a ladder at the Eaton store campus and crashed and fell onto the glass coffee table below her. I saw that she had fallen off the ladder while over reaching to hang a display sign. Once I got to the scene of the accident, I noticed Janet Smith holding onto her left wrist and was covered in blood noticeably in the face, forehead and left abdomen. Her co-worker Janice Kaschuk had witnessed the accident and rushed over and was helping Ms. Smith. A petition attached to the home study request dated May 20, 2015 indicated that the mother, Amanda Chamberlain was originally investigated on February 20, 2015 for neglectful supervision, physical abuse, and physical neglect. The child being placed, Izaiah Chamberlain was found to have four belt marks on his body. Izaiah indicated that one of his mother’s roommates hit him when he could not find a pencil to complete his homework. The mother was later arrested for drug and weapon charges. At 5am Officer Singh called SA Lyn Brumaire who was on duty at Coral Tower because Jordan Horvat was worried a resident of 902, Stephanie Lee, was missing. Upon keying into the room with PSO Tabiri, the resident in questions, Stephanie Lee was found to be missing from her apt. Officer Tabiri then decided to take a statement from SA and resident Macayla Caso. Ms. Horvat was also not present in the room. Ms. Caso was under the assumption that Ms. Horvat had gone home since she was packing earlier in the day and the sheets were stripped off her bed.

As I was completing my initial investigation I was returning items (driver’s licenses, registration, and insurance information) to the drivers. I approached Mitchell who was sitting in her vehicle. She appeared slumped over. I knocked on the window several times. There was no reply. I opened Mitchell’s vehicle door. She appeared unconscious. I shook her by the shoulder. She did not respond. I called for Goodwill Ambulance, who had just left with Feeney to return because I had an unresponsive person. I pulled Mitchell out of her vehicle and laid her on the street. I put a blanket that she had in her vehicle under her head. I shook her again, no response. I performed a sternum rub on Mitchell. She moved her head and asked “What’s going on?” I told her she passed out that she was in and accident. I told her to keep her eyes open and waited for Goodwill Ambulance to arrive back on scene. On 7/6/2015 S/O EMT Perez was dispatched to WC-407 regaurding a pendant alarm. S/O EMT Perez knocked and annoounced his presence at the door, when there was no answer S/O EMT Perez proceeded to search the apartment. S/O EMT Perez found the resident, a Mrs. Mary Beiter toppled over her walker by her bedside. Mrs. Mary Beiter was trying to get inter her bed when she lost balance and fell and could not get up under her own strength. Mrs.Beiter did not complain of any hip pain during the pelvic exam but did state that she has a bad knee and hip that makes it rather hard to stand on her own. On Friday 09/16/2016 at 2300 hours, Security Officer Michelle Gallegos was badging visitors in the E.D. Lobby when the sister of a patient in E.D. # 32 lunged over the counter, knocking down a computer screen and a cup of water, then striking Officer Gallegos in her left arm. Security Officer (SO) Omar Alonso, was inside the E.D. Security Office, and responded immediately verbally instructing the visitor that police were going to be notified and that she needed to leave property immediately, and he also called Dispatch for immediate assistance. The female visitor was with 5 other females and her mother so they all followed Officer Alonso, outside of the E.D. Lobby. They were screaming and hollering that they were being treated disrespectfully by the badging officer who would not let them go back and visit her sister. Security Officers Ariel Weiland, Demetrio Flores, and Supervisor Steven Evans all responded by 2303 hours and helped to keep all 6 visitors outside while SO Alonso, conducted an investigation into the visitor’s allegations that they were being unfairly treated and were given erroneous information. He spoke to the nurse that Officer Gallegos spoke to when she needed information on the patient and permission for visitors to be given a pass to visit. Interestingly, this scenario is becoming more and more prevalent with the actions, and recent violent attacks are occurring worldwide. Further, acts of violence concerning shootings appear to be discussed more often in the media outlets and politics. While numbers and facts continue to be argued between both sides of gun control, the truth remains, the figures and facts can be woven and molded to back either side. Unfortunately, this is a terrible flaw with facts, graphs, patterns as people who understand how to manipulate the numbers are usually able to manipulate the story people hear. Surprised by the case of Stephanie Smith, who have been suffering from severe foodborne illness caused by E. coli since 2007 have made me changed my career goal. After watching the video in my food science class, one sentence keeps on repeating in my mind, “I ask myself every day, ‘Why me?’”. Truly, it is not fair for a normal 22 years old children’s dance instructor who became paralyzed because of the ground beef in her hamburger. Touched by the case, I wish I could become part of the food safety professionals who can prevent people from facing similar needless tragedies.
