There are different types of abuse that can occur within a healthcare setting: physical, verbal, sexual, emotional, psychological, and financial (Duggleby, 2014). The vulnerable populations who are at risk for being abused are women at any stage of their lives, persons with disabilities, of First Nations descent, the elderly, and anybody within a same-sex relationship (Chow, 2013). Abuse can occur in home, in public, and even within the healthcare setting. For the purpose of this paper, we are going to look at physical and verbal abuse within the healthcare setting.

Working with victims of domestic violence can be an extremely rewarding and fulfilling endeavor. One of the most crucial aspects for a paralegal working with victim of domestic violence is adequately engaging in the task of educating oneself to understand the commonalities of such victims and the ordeals that they have been through. Such clients are drastically different from other individuals who have suffered other physical and violent crimes. Understanding the background of someone who has lived through domestic violence is absolutely central to being able to provide adequate and sensitive legal care. To work in the human services field, it is imperative that you can see and understand your own bias and personal prejudices. In my work as a Case Manager working with the homeless population, it is fairly common for me to come across women, and occasionally men, who have been victims of domestic violence. Often times when we cross paths, they are actively fleeing from it. One of my struggles as a case manager has been to support and protect these women while helping them to find self reliance and independence. Occasionally I find myself feeling frustrated by behaviors and thought processing of my clients that I perceive as being weak and subservient. For me, I believe this intolerance comes from my mother, who in her own right is a strong independent woman, but still found herself faced with domestic violence in her marriage. My mother’s story is a rare one, the abuse happened, she reported it to the police, and after a brief separation, my parents were able to work on their marriage through therapy sessions with a counselor. Intimate partner abuse is a sensitive social topic as well as an important topic with respect to health care and community nursing (Svavarsdottir, 2010). Nurses should be well aware of the signs and symptoms of intimate partner abuse, but its detection can often be difficult. Specific assessment protocols are often useless in detecting abuse because each situation of intimate partner abuse varies (Svavarsdottir, 2010). While identifying victims of physical abuse is difficult, it must be done to implement interventions to assist these victims. Nurses and health care professionals must be prepared to identify these individuals. In addition to identification of victims of intimate partner abuse, healthcare professionals must be diligent in education, which could prevent future incidences of intimate partner abuse.

Before reading the first two parts of "Til Death Do Us Part" I knew that domestic violence was a growing issue in the United states, but it was a surprise to find out that the state that we live in for more than fifteen has been place in the top ten nationally in the rate of women murder by men (Pardue, Smith, Hawes, & Hauff, 2014). The first part gave the statics on how many victims were turned away in 2012-2013 from shelters around South Carolina which was a total of 380. The reason why this was done because there was no rom they said (Pardue et al., 2014). Since this information was taken there should have a increase in the number of shelters in South Carolina, but there is only 18 domestic violence shelters. There is about 36,000 incidents of domestic abuse in our state. That number is shocking, many people wouldn 't have thought that there were that many incident. It has been recorded that approximately “2,000,000 injuries, and 1,300 deaths are caused each year as a result of domestic violence” (Centers for Disease Control and Prevention). This harm to and loss of life is due to the negligence of all Americans, in regards to the monitoring and managing of domestic violence in the United States. In order to combat the blatant disregard for domestic violence victims, Americans must support, aid, and assist those who fall victim to this crime, as well as educate those who are oblivious to the matter at hand. These actions will be essential in the eradication of domestic violence, as well as the preservation of the health and overall safety of all American people.

Self-reported rates of screening for domestic violence are low even among women at higher risk of abuse. The study provides nationally representative estimates of lifetime Self-reported domestic violence screening in health care settings for United States Women, described the characteristics of women who reported being screened, and described the settings in which reported screening occurred. Researchers found that reported lifetime domestic violence screening was more likely among women who had characteristics associated with higher risk for domestic violence: being younger, unmarried, or living unmarried with partner. In addition, screening was much more likely among women who had witnesses someone being beaten or killed in the past 12months, which may be a marker for being a victim of violence.

There's a growing trend of domestic abuse in the United States, and large cities like New York can see more than their fair share of abuse cases. There are people who desperately need to leave their current abusive situations, but unfortunately, many cases are being brought before the courts that are not real abuse cases.

For domestic violence, medical providers can be as much of a first responder as law enforcement or emergency medical services. The medical provider who is going to note that the victim has been seen several times for the same injury. One of medical providers’ core functions is to investigate the well-being of the patients. If a patient presents with multiple concussions in a year without a good explanation of the circumstances that caused the concussion, this would raise the red-flags for the provider. Additionally, with new governmental regulations providing incentives for providers to transition care to another provider, the whole medical record for a patient can now be in the hands of the provider. (Centers for Disease Control and Prevention, 2016) At Santa Clara County Health and Hospital System, the primary care provider will receive documentation of all their patients who are admitted or seen in the emergency room. Information sharing about patients will only increase with time. If a patient is seen on one side of the country in the emergency room for a concussion, their primary care provider on the west coast can receive a report of this concussion. The awareness can be a matter of life or death for a patient.

In the United States, approximately 1.5 million women report some form of intimate partner violence (IPV) each year and of those an estimated 324,000 are pregnant (Deshpande & Lewis-O'Connor, 2013). According to Deshpande and Lewis-O’Connor (2013), IPV is defined as abuse that may be actual or threatened by an intimate partner that can be physical, sexual, psychological, or emotional in nature. It is important for health care providers to realize IPV does not only include physical or sexual abuse but also includes name calling, financial control, constant criticism, and isolating women away from their families and friends (Deshpande & Lewis-O'Connor, 2013; Smith, 2008).
